Abstract Background: During pregnancy, due to reduced immunological resistance, the probability of tuberculosis infection is increased, easily leading to the occurrence of tuberculosis; thus, pregnancy with tuberculosis is a risk to the health of pregnant women with infectious diseases. However, the clinical symptoms of pregnancy with pulmonary tuberculosis are atypical, often confused with the physiological reactions of pregnancy, and there is a lack of clinical awareness, especially in the context of the global COVID-19 pandemic. Specifically, atypical lung lesions are prone to misdiagnosis or delayed diagnosis.Case presentation: A 21-year-old woman was infected with Mycobacterium tuberculosis during the third trimester of pregnancy. The diagnosis and treatment of the patient are reported.Conclusion: The interaction between tuberculosis and pregnancy is a double-edged sword. Increasing awareness of tuberculosis among doctors is essential, and early diagnosis and standardized treatment are key to improving the outcome of pregnancy.

Background. Chilaiditi syndrome is a phenomenon where there is an interposition of the colon between the liver and the abdominal wall leading to clinical symptoms. This is distinct from Chilaiditi sign for which there is radiographic evidence of the interposition, but is asymptomatic. Case Presentation. Here, we present the case of a patient who, despite having clinical symptoms for a decade, had a delayed diagnosis presumably due to the interposition being intermittent and episodic. Conclusions. This case highlights the fact that Chilaiditi syndrome may be intermittent and episodic in nature. This raises an interesting question of whether previous case reports, which describe complete resolution of the syndrome after nonsurgical intervention, are perhaps just capturing periods of resolution that may have occurred spontaneously. Because the syndrome may be intermittent with spontaneous resolution and then recurrence, patients should have episodic follow-up after nonsurgical intervention.

Abstract Background The COVID-19 pandemic has strained all medical systems, especially in countries like Ecuador, where health services were already limited. These conditions, combined with a deadly and unusual disease, like primary heart angiosarcoma, can lead to severe outcomes. Angiosarcomas represent the most common and aggressive primary malignant heart tumor; regretfully, its clinical manifestations are vague and can be easily missed. Most patients become symptomatic when there is local invasion, embolization, or metastases, leading to late diagnosis and poor survival. High clinical awareness, adequate diagnosis, and prompt treatment are vital in these rare diseases, in which time is of paramount importance. Case presentation We report the case of a 28-year-old female who had cough, hemoptysis, and ground-glass opacities in the CT (computed tomography). Since Ecuador is in the middle of this pandemic, she was misdiagnosed and mistreated. Primary heart angiosarcoma was diagnosed, and regretfully, the patient suffered multiple complications due to diagnosis and died. Conclusion To this day, most cardiac angiosarcomas are found in a late-stage with distal metastasis and advanced local invasion. Sadly, this tumor is frequently missed due to its incidence and broad-spectrum of clinical symptoms. Considering that its manifestations can be misleading, misdiagnosis can occur, especially in pandemic times. Therefore, knowledge of other pathologies prevents COVID-19 from overshadowing other diagnoses, hence preventing delayed diagnosis or even misdiagnosis and consequent adverse outcomes for patients.

Abstract Background: There is no cure for psoriasis. Early treatment using biologics is recommended to improve skin manifestations and reduce systemic inflammation, which leads to comorbidities in various organs.Case Presentation: HistobulinTM therapy was performed on a psoriasis patient who developed the disorder due to allergic rhinitis. Psoriasis was confirmed pathologically by skin biopsy. The patient responded rapidly, and the skin manifestations began to improve after just the first injection. Although the patient showed some temporary aggravation after the third injection, the clinical symptoms and signs improved continuously thereafter and disappeared after the eighth injection. Remission was induced and was evident when the patient showed no symptoms and signs during the subsequent 4 weeks during which time the patient received 4 more injections; afterwards, HistobulinTM therapy was ceased. After treatment, psoriasis did not recur for more than 6 months.Conclusions: HistobulinTM is effective and induces remission in psoriasis patients. HistobulinTM is suggested for curative therapeutics in psoriasis patients, and further basic research and clinical evaluation are necessary.

Abstract Background Metastatic choriocarcinoma in the third trimester of pregnancy is extremely rare. Case presentation A 25-year-old Chinese woman (gravida 3, para 0) who was 28 weeks pregnant was admitted for sudden convulsion, aconuresis, and unconsciousness. The decision was made to perform an emergency cesarean delivery and craniotomy, hematoma clearance, and decompression. Pathological examination confirmed choriocarcinoma with brain metastasis. The patient underwent chemotherapy with the etoposide, cisplatin (EP) and etoposide, methotrexate and dactinomycin alternating with cyclophosphamide and vincristine (EMACO) regimens. A satisfactory result was achieved. Conclusions When encountering intracranial mass or bilateral pulmonary nodules in a pregnant woman, especially one in the third trimester, metastatic choriocarcinoma should be considered.

Case presentation Though rare in pregnancy, this case report describes a 32-year-old G0 at 39 + 3/7 weeks with a history of kidney stones and a simple ovarian cyst that presented to the labor floor with left lower quadrant pain. She underwent an induction of labor for persistent pain and had a subsequent primary section for a category II tracing. Conclusion During the section, a torsion x3 of the left infundibular ligament was noted. The pathology indicated an acute infarct – only one other case of a full-term torsion was found in the literature (Li Q, Li X, Zhang P. Abstract Background This is a case report of an asymptomatic SARS-CoV-2 infection associated with new-onset nephrotic syndrome in a pediatric patient. This is the third case of new-onset nephrotic syndrome in children associated with SARS-CoV-2 infection, but is the first case report describing a new-onset nephrotic syndrome presentation in a patient who had asymptomatic COVID-19 infection. Case presentation This is a case of a previously healthy 5 year old female who presented with new-onset nephrotic syndrome in the setting of an asymptomatic COVID-19 infection. She presented with progressive edema, and laboratory findings were significant for proteinuria and hypercholesterolemia. She was treated with albumin, diuretics, and corticosteroid therapy, and achieved clinical remission of her nephrotic syndrome within 3 weeks of treatment. Though she was at risk of hypercoagulability due to her COVID-19 infection and nephrotic syndrome, she was not treated with anticoagulation, and did not develop any thrombotic events. Conclusions Our case report indicates that SARS-CoV-2 infection could be a trigger for nephrotic syndrome, even in the absence of overt COVID-19 symptoms.
