The study of the concentration of metalloproteinases, pro- and anti-inflammatory cytokines, lymphocyte activation markers in seminal plasma of men with oligosymptomatic forms of chronic inflammation of the urogenital tract (CIUT), depending on the fertility rate are presented in the article. Quantitative characteristics in male sperm with different forms of pathospermia were studied for matrix metalloproteinases (MMP) -2 and MMP-9 and their inhibitors - tissue inhibitors of metalloproteinases (TIMP)-1 and TIMP-2. It was shown, that during chronic inflammation of the urogenital tract of men are shifting levels of cytokine profile, reducing the concentration of metalloproteinase-2, chemokines - fractalkine and regulated by activation, expression and secretion of normal T-cells (RANTES), sharp increase in IL-8, MCP-1 and elevation of the CD25+ / CD95+, indicating that the disturbance of apoptosis of pathological forms generative cells and their accumulation in the sperm. The ratio of immunological indices IL-2/IL-4, IL-10/IL-12 was calculated and a significant increase in the IL-10/IL-12 index was noted in the group of individuals with elevated levels of hypercapitated form of sperm, and a reduced proportion of this ratio was observed in the microsomatic morphology of sperm. Prolonged inflammation in the genital area accompanied by depletion of the local immune system, resulting in the development of infertility. Immunocorrection therapy for men with CIUT should take into account the peculiarities of changes in local immunity and be differentiated depending on the prevalence of certain pathological forms of sperm and changes in the cytokine profile of the seminal plasma.

Introduction. One important aspect of modern research is the study of the role of the phenotypic composition of lymphocytes in the development of a variety of pathological processes. The aim of the study was to compare the phenotypic composition of lymphocytes and their intercellular cooperation in patients with sensorineural hearing loss (SHL), with vibration disease (VD) from exposure to local vibration, with VD from combined exposure to general and local vibration, chronic mercury intoxication (CMI). Material and methods. The phenotypical structure of lymphocytes was estimated by the method of indirect immunofluorescence using monoclonal antibodies to molecules CD3+, CD4+, CD8+, CD16+, CD20+, CD25+, CD95+. Results. The general regularity of changes of an immunophenotype of lymphocytes in patients with the occupational pathology induced by the influence of physical and chemical factors was established to be the hyperactivation of immune responses characterized by an increase in the number of lymphocytes (CD3+, CD4+, CD20+) of the various degree of expressiveness. Differences between the compared groups were characterized by an elevation increase in the number of mature T-lymphocytes (CD3+) in persons with SHL concerning patients with the VD from the combined impact of the general and local vibration, increase of natural killers (CD16+) in comparison with the VD due to a local vibration. The latter showed lower levels of early lymphocyte activation markers (CD25+) than BB due to combined exposure to local and general vibration and CMI. Features of pathogenetically significant intercellular interactions, manifested by a difference in number and composition of correlation pairs, are revealed. Discussion. The results show a different degree of expression of immune responses, which can be due to many factors (severity of the disease course, an initial background of immunoreactivity before starting work in harmful working conditions, work experience, etc.). Conclusion. The detected changes in the phenotypic composition of lymphocytes and their cooperation in patients with occupational pathology formed under the influence of physical and chemical factors seem to be due to the specificity of the affecting occupational factors and, as a result, the degree of expression of the pathological process.

Aim. To asses an effectiveness of assisted reproductive technologies (ART) program given characteristics of the cytokine profile of seminal plasma (SP) entering the female reproductive tract during sexual intercourse. Outcomes and methods. 33 married couples who applied for a treatment of infertility by means of in vitro fertilization /ICSI (Intracytoplasmic Sperm Injection) were included in the prospective study. Patients were recommended to have sexual intercourse with no restrictions during treatment and to have the last sexual intercourse 3 days before an intended transvaginal puncture (ITP). Testing of cytokines (transforming growth factor (TGF)-b1, interferon (IFN)-g, interleukin (IL)-33, IL-6, IL-8, IL-23, IL-10, tumor necrosis factor (TNF)-a, IL-18, IL- 17A, IFN-a, IL-12, monocyte chemotactic protein-1) levels in samples of partners’ SP obtained on the day having ITP were carried out using multiplex analysis with LegendPlex kits (BioLegend, USA). Results. When comparing a cytokine profile of SP in couples who did not become pregnant (n=25) and couples who become pregnant (n=8) increased IL-18 and reduced IL-10 levels (p=0,017 and p=0, 01 respectively) were revealed in the group which got pregnant. To assess a clinical relevance of cytokine content in SP ROC (Receiver Operating Characteristics) curve was used. It was established that determining of IL-18 concentration in SP has the greatest diagnostic significance (the area under a curve was 0.792±0.107, test sensitivity - 62.5%, test specificity - 95.24% at threshold concentration>210.43 pg/ml). Incidence of pregnancy at IL-18 concentration above threshold levels was 83,3% while at lower concentrations of IL-18 pregnancy occurred only in 13.0% of women. Conclusions. Elevated IL-18 levels and decreased IL-10 levels in SP of female patients’ partners who don’t have restriction of sexual life when treating infertility with ART are favorable factors for a pregnancy to occur.

Purpose Hodgkin's lymphoma (HL) has been demonstrated to be a good target for immunotherapy since lymphocyte activation markers such as CD30 are expressed in high numbers on the malignant cells. Thus, we developed a new radioimmunoconjugate consisting of the murine anti-CD30 monoclonal antibody (MAb) Ki-4 labeled with iodine-131 (131I). Patients and Methods The biodistribution of 131I–Ki-4 was assessed via dosimetry after preinfusion of 5 mg native Ki-4 followed by 250 to 300 MBq 131I-labeled Ki-4. Whole-body scintigraphy was performed 1 hour, 24 hours, 48 hours, 72 hours, and 6 days after the infusion. Dosimetry was calculated using the programs NucliDose ICON-IDL (version 5.0.2; Siemens, Erlanger, Germany) and MIRDOSE (version 3.1; Oak Ridge National Laboratories; Oak Ridge, TN). The therapeutic dose was given on day 8 after preinfusion of unlabeled Ki-4. Results We treated 22 patients with relapsed or refractory CD30-positive HL. Preinfusion of 5 mg native Ki-4 was sufficient to bind the soluble CD30. Imaging demonstrated localization of involved areas measuring 5 cm in diameter or more in four patients and 2.5 cm in one patient. Patients received total body doses of 0.035 Gy to 0.99 Gy. Acute toxicity was mild with grade 1 fatigue in 19 of 22 assessable patients. Seven patients experienced grade 4 degrees hematotoxicity 4 to 8 weeks after treatment. Response included one complete remission, five partial remissions, and three minor responses. Conclusion Treatment with 131I–Ki-4 is effective but can be associated with severe hematotoxicity.
