Stress and Burnout: Solutions to a Global Problem

21<sup>st</sup> Century is an era of stress and burnout. For the past few decades it has been a burning and hot topic of discussion for researchers, to evolve stress and burnout- from a global problem to global solutions, as it affects the life of individuals in an unprecedented manner and touches them at workplace across the globe. Everybody knows what stress is all about. It has become a part of life and perhaps, to some extent, necessary at work and outside work. Some people are more productive and creative when they work under stress. But if stress is intense and continuous, then it becomes a negative phenomenon leading to physical illness and psychological disorders. Stress and burnout in today's environment has become a well documented problem. Various researches have evaluated stress and burnout in workplaces. Stress is a non-specific response of body to any demand made on it. Many researchers have identified that stress and burnout has become an integral part of our daily life due to the negative aspects of job, such as, multiple responsibilities, disciplinary problems, employee's apathy, involuntary transfers, inadequate pay and perks, less chances of career advancement and lack of administrative support etc. As a result of these, individuals are likely to suffer from stress and may experience a sense of tiredness and frustration. When prolonged stress continues and it is not effectively managed, it can even lead to symptoms of burnout, a state of emotional exhaustion, depersonalization and diminished personal accomplishment in the workplace. Stress management can be done through six zones which we have discussed in detail, with a holistic approach. It includes health zone, intimate zone, family zone, work zone, social zone and spiritual zone. In this paper, an effort is made to discuss the solutions of stress and burnout in different and innovative ways through different zones which have been experienced by the author in his 20 years of professional career. Thus, stress and burnout are not a trivial problem but a major dysfunction of the 21<sup>st</sup> century, which has far reaching impact on quality and quantity of productivity. The present paper explores the concept of stress and burnout, the major differences between them, factors leading to genesis of the problem, various symptoms and how it is a serious quality concern for all professionals. Appropriate interventions for prevention and management of stress and burnout are also suggested.

Abstract Background Burnout resulting from long-term and unmanaged workplace stress is high among healthcare professionals, especially surgeons, and affects both individuals and the quality of patient care. The objective of this study was to determine the prevalence and associated factors for burnout among attending general surgeons and to identify possible preventive strategies. Methods A national cross-sectional survey using a 35-item questionnaire was conducted among members of the Turkish Surgical Society. The survey evaluated demographics, professional and practice characteristics, social participation, and burnout as well as interventions to deal with burnout. Burnout was defined as a high score on the emotional exhaustion (EE) and/or depersonalization (DP) subscales. Surgeons with high scores on both the EE and DP and a low score on personal accomplishment (PA) were considered to have severe burnout. Results Six hundred fifteen general surgeons completed the survey. The median EE, DP, and PA scores were 34 (IQR, 20–43), 9 (IQR, 4–16), and 36 (IQR, 30–42), respectively. Overall, the prevalence of burnout and severe burnout were 69.1 and 22.0%, respectively. On multivariable analysis, factors independently associated with burnout were working in a training and research hospital (OR = 3.34; P < 0.001) or state hospital (OR = 2.77; P = 0.001), working ≥ 60 h per week (OR = 1.57; P = 0.046), and less frequent participation in social activities (OR = 3.65; P < 0.001). Conclusions Burnout is an important problem among general surgeons with impacts and consequences for professionals, patients, and society. Considering that burnout is a preventable condition, systematic efforts to identify at-risk populations and to develop strategies to address burnout in surgeons are needed.

Anaesthetic technicians play a key role in the operating room, yet little is known about their levels of job satisfaction or workplace stress. A blinded, confidential single mail-out survey was posted to anaesthetic technicians in New Zealand. The survey consisted of demographic information, a job satisfaction survey, the Maslach Burnout Inventory and the Short Form 12. At total of 154 forms were returned (51% response rate). Respondents worked predominantly in public hospitals and many had duties outside the operating room. Job satisfaction was related to teamwork, practical nature of work and patient contact, while dissatisfaction was related to lack of respect from nurses and limited career pathway. High to moderate levels of emotional exhaustion (48%), depersonalisation (39%) and low levels of personal accomplishment (58%) were indicators of burnout. The Short Form 12 revealed high levels of physical impairment in 24% and emotional impairment in 35% of respondents. These data suggest that work is needed to evaluate anaesthetic assistants’ job structure and actively manage their important physical and emotional sequelae.

Abstract Territorial animals are expected to adjust their response to intruders according to the perceived threat level. One of the factors that drives threat level is the identity of the intruder. The dear enemy phenomenon theory postulates that individuals should respond with lower intensity to neighbors, already possessing a territory, than to strangers that may fight to evict them. In social species, the hierarchical status of the intruder might also mediate this response. Such behavioral adjustments presuppose a capacity to discriminate between individuals posing different threat levels. Here, we tested the behavioral response of Alpine marmots to territorial intrusions in a wild population. We compared both dominant females’ and males’ responses to scents from neighbor and stranger dominant males (dear enemy phenomenon) and to dominant and subordinate stranger males (social status-specific response). In addition, we tested for any covariance between male scents and social status. We showed that female and male dominant marmots do not adjust the intensity of their behavioral responses to whether the intruder’s territory is bordering or not (neighbors or strangers) or to the intruder’s social status, even though dominant and subordinate males are thought to pose different threats and social status is encoded in scents. Thus, we did not find support for the dear enemy phenomenon and conclude instead that, in dominant Alpine marmots, no intruder should enter a foreign territory. Research taking a more holistic approach of the evolution and maintenance of territoriality is required to understand the flexibility of responses to intruders in group-living species.
