BACKGROUND Although the potential of big data analytics for health care is well recognized, evidence is lacking on its effects on public health. OBJECTIVE The aim of this study was to assess the impact of the use of big data analytics on people’s health based on the health indicators and core priorities in the World Health Organization (WHO) General Programme of Work 2019/2023 and the European Programme of Work (EPW), approved and adopted by its Member States, in addition to SARS-CoV-2–related studies. Furthermore, we sought to identify the most relevant challenges and opportunities of these tools with respect to people’s health. METHODS Six databases (MEDLINE, Embase, Cochrane Database of Systematic Reviews via Cochrane Library, Web of Science, Scopus, and Epistemonikos) were searched from the inception date to September 21, 2020. Systematic reviews assessing the effects of big data analytics on health indicators were included. Two authors independently performed screening, selection, data extraction, and quality assessment using the AMSTAR-2 (A Measurement Tool to Assess Systematic Reviews 2) checklist. RESULTS The literature search initially yielded 185 records, 35 of which met the inclusion criteria, involving more than 5,000,000 patients. Most of the included studies used patient data collected from electronic health records, hospital information systems, private patient databases, and imaging datasets, and involved the use of big data analytics for noncommunicable diseases. “Probability of dying from any of cardiovascular, cancer, diabetes or chronic renal disease” and “suicide mortality rate” were the most commonly assessed health indicators and core priorities within the WHO General Programme of Work 2019/2023 and the EPW 2020/2025. Big data analytics have shown moderate to high accuracy for the diagnosis and prediction of complications of diabetes mellitus as well as for the diagnosis and classification of mental disorders; prediction of suicide attempts and behaviors; and the diagnosis, treatment, and prediction of important clinical outcomes of several chronic diseases. Confidence in the results was rated as “critically low” for 25 reviews, as “low” for 7 reviews, and as “moderate” for 3 reviews. The most frequently identified challenges were establishment of a well-designed and structured data source, and a secure, transparent, and standardized database for patient data. CONCLUSIONS Although the overall quality of included studies was limited, big data analytics has shown moderate to high accuracy for the diagnosis of certain diseases, improvement in managing chronic diseases, and support for prompt and real-time analyses of large sets of varied input data to diagnose and predict disease outcomes. ObjectiveIn general, published studies analyse healthcare utilisation, rather than foregone care, among different population groups. The assessment of forgone care as an aspect of healthcare system performance is important because it indicates the gap between perceived need and actual utilisation of healthcare services. This study focused on a specific vulnerable group, middle-aged and elderly people with chronic diseases, and evaluated the prevalence of foregone care and associated factors among this population in China.MethodsData were obtained from a nationally representative household survey of middle-aged and elderly individuals (≥45 years), the China Health and Retirement Longitudinal Study, which was conducted by the National School of Development of Peking University in 2013. Descriptive statistics were used to analyse sample characteristics and the prevalence of foregone care. Andersen’s healthcare utilisation and binary logistic models were used to evaluate the determinants of foregone care among middle-aged and elderly individuals with chronic diseases.ResultsThe prevalence of foregone outpatient and inpatient care among middle-aged and elderly people was 10.21% and 6.84%, respectively, whereas the prevalence of foregone care for physical examinations was relatively high (57.88%). Predisposing factors, including age, marital status, employment, education and family size, significantly affected foregone care in this population. Regarding enabling factors, individuals in the highest income group reported less foregone inpatient care or physical examinations compared with those in the lowest income group. Social healthcare insurance could significantly reduce foregone care in outpatient and inpatient situations; however, these schemes (except for urban employee medical insurance) did not appear to have a significant impact on foregone care involving physical examinations.ConclusionIn China, policy-makers may need to further adjust healthcare policies, such as health insurance schemes, and improve the hierarchical medical system, to promote reduction in foregone care and effective utilisation of health services.

RESUMOObjetivo: compreender as concepções de pessoas idosas acerca das repercussões do adoecimento crônico na sua saúde mental. Método: trata-se de estudo qualitativo, descritivo, realizado com 13 pessoas idosas com diagnóstico de doenças crônicas cadastradas em uma Unidade de Saúde da Família. Coletaram-se as informações a partir da aplicação de um formulário de um roteiro de entrevista semiestruturada. Analisaram-se as entrevistas conforme a técnica de análise de conteúdo. Resultados: a partir da análise do conteúdo das descrições originárias das entrevistas, emergiram quatro categorias temáticas: << Sentimentos relacionados à doença crônica >>; << Mudanças nos hábitos de vida após a descoberta da doença crônica >>; << Dificuldade para aceitação da doença crônica >>; e << A busca pela espiritualidade para aceitação da doença crônica >>. Conclusão: percebeu-se, com o alcance do objetivo do estudo, a necessidade de qualificação dos profissionais atuantes da Atenção Básica para assistir o ser idoso, não apenas para a resolutividade de suas demandas no momento, mas também para saber ouvir e identificar as possíveis repercussões que esse adoecimento traz para a vida desses indivíduos e sua saúde mental. Method: qualitative, descriptive study with 13 elderly people diagnosed with chronic diseases registered in a Family Health Unit. The information was collected through the application of a form of a semi-structured interview script. The interviews were analyzed according to the content analysis technique. Results: the analysis of the content of the descriptions collected in the interviews resulted in four thematic categories: << Feelings related to the chronic disease >>; << Changes in life habits after the discovery of the chronic disease >>; << Difficulty to accept the chronic disease >> and << Search for spirituality for acceptance of the chronic disease >>. Conclusion: in order to reach the objective of the study, it was noticed the need to qualify professionals working in Primary Care to assist the elderly people, not only for the resolution of their demands at the moment, but also to know how to listen and identify possible repercussions that this disease brings to the lives of these individuals and to their mental health. Conclusión: con el alcance del objetivo del estudio, se percibió la necesidad de calificación de los profesionales actuantes de la Atención Básica para asistir el ser anciano, no apenas para la resolutividad de sus demandas en el momento, pero también para saber oir e identificar las posibles repercusiones que esa enfermedad trae para la vida de estos individuos y su salud mental.
