Background: Atopic dermatitis is a chronic inflammatory dermatosis for which racial specificities have been reported. Objective: To describe the clinical aspects of atopic dermatitis on children. Methods: Descriptive cross-sectional study in one public hospital and one private hospital in Brazzaville, Congo, for three years. Children under the age of 16, consulted for atopic dermatitis were included. Demographic and clinical data were collected. Atopic dermatitis was defined according to the criteria of the United Kingdom Working Party and its severity appreciated by SCORAD. The data was processed by EpiInfo 7® software. Results: Seventy-five children were included. They were 49 girls (65.33%) and 26 boys (34.67%), mean age 4.3 years [9 months-12 years]. Personal atopic history was found in 46.67% of cases and family atopic history in 61.33% of cases. Initial symptoms of atopic dermatitis had occurred before 2 years of age for 65 children (86.67%). The average consultation time was 33.4 months. In infancy, erythematous vesicular or oozing plaques were observed in 68.18% of cases and scaly erythematous plaques on folds in 50% of cases. In childhood, scaly erythematous plaques were observed in 84.90% of cases and lichenified eczema in 10.67% of cases. Xerosis was noted in 89.33% of all cases. Secondary infection was seen in 33.33% of cases. Twenty-five cases (33.33%) were severe. Conclusion: This study reveals a delayed diagnosis of atopic dermatitis, unusual clinical features and the importance of secondary infection. Information for families and clinicians should be considered to improve the earliness of the diagnosis.

Background: Atopic dermatitis (AD) is a chronic and recurrent inflammatory dermatitis often associated with other atopic manifestations, such as asthma and allergic rhinitis. This study aimed to determine the epidemiological and clinical aspects of AD and to assess the quality of life (QoL) of patients suffering from AD in our setting. Materials and Methods: This was a cross-sectional study conducted from February through April 2017 in seven hospitals in Cameroon. The study included patients above 18 who presented themselves to a dermatology consultation, were diagnosed with AD, and gave their consent. To assess the severity of AD and evaluate the QoL of the patients, standardized scales, such as SCORAD and QoLIAD, were employed. Results: The study enrolled 46 patients between 18 and 69 years of age with a mean age of 31 ± 12 years and the prevalence of AD at 1.5%. Most of the participants were females, with a sex ratio of 0.4:1, living in urban areas (93.5%). Food (34.8%) and cosmetic products (21.7%) were found as the main risk factors in the occurrence of AD. Upon physical examination, the upper and lower limbs were found to be the most affected in 84.8% and 54.3% of cases, respectively; in addition to cutaneous xerosis (45.7%), lichenification (43.5%), and excoriations (37%). Of the 46 patients, 9 (20%) had severe AD, 32 (70%) had moderate AD, and 5 (10%) had mild AD. QoL was impaired in 43 of the 46 patients (93.5%). Conclusion: Atopic dermatitis is a pathology that impacts the QoL of adults. A QoL assessment is, therefore, an important step in the management of AD.

Background About 60% of individuals with atopic dermatitis (AD) develop their first manifestation during infancy. Cow’s milk (CM) exposure is considered to be a risk factor for AD.Objective To evaluate for an association between cow’s milk exposure and atopic dermatitis in infants > 6 months of age.  Methods This cross-sectional study consisted of subjects from a previous study and new subjects recruited in order to meet the minimum required number of subjects. Our study population comprised 120 infants, born between 1 February and 30 November, 2012 in Sanglah Hospital, Denpasar. Subjects were divided into CM and non-CM groups and analyzed for their risk of AD. Subjects were included to CM group if they were fed with cow’s milk/formula  and included to non-CM group if they were breastfeed exclusively in the first six months of life. Other possible risk factors were assessed by multivariate analysis. Results One hundred twenty subjects were enrolled and analyzed (59 in the CM and 61 in the non-CM groups). The prevalence of AD was 30%. Multiple logistic regression analysis revealed a significant association between CM exposure and AD, with odds ratio (OR) 2.37 (95%CI 1.036 to 5.420; P=0.04). In addition, maternal diet including eggs and/or cow’s milk during the breastfeeding period was significantly associated with AD in infants (OR 3.18; 95%CI 1.073 to 9.427; P=0.04).Conclusion Cow’s milk exposure is significantly associated with atopic dermatitis in infants  > six months of age.

Introduction: Herpes zoster (HZ) is characterized by an extremely painful vesicular rash, which may be complicated by secondary infection and post-herpetic neuralgia. To date, multiple risk factors associated with HZ have been established, including endocrine diseases, immunosuppressive conditions, cancers, and other chronic medical conditions. Objectives: To assess the profile of herpes zoster associated with co-morbid condition. Methods and Materials: This cross sectional study was conducted among purposively selected 130 HZ patients in the department of Dermatology and Venereology, Combined Military Hospital (CMH) Dhaka from January 2017 to December 2018. Data were collected through face to face interview using pretested semi-structured questionnaire. Results: Out of 130 patients, majority of the patients were male (69.2%) and mean age was 53.5±9.8 years and majority gave the history of chicken pox 60% and common site of involvement were chest (right and left) 33.1% and 48.5%, upper back (right and left) 30.8% and 43.1% and upper right arm 33.1%. About 66.9% patients gave the history of having co-morbid condition like Diabetes 30.8%, Stroke 9.2%, Hypertension 2.3%, Myocardial infarction 3.8%, Peptic ulcer disease 10.0%, Malignancy 2.3%, Tuberculosis 2.3% and Irritable bowel syndrome 2.3%. Conclusion: Based on our study finding, we can conclude that herpes zoster is a disease that is associated with other co-morbid conditions. If herpes zoster is an early manifestation of undiagnosed co-morbid condition, patients should undergo testing for undiagnosed disease when they present with herpes zoster. JAFMC Bangladesh. Abstract Background Antibiotics are the most prescribed drugs especially in pediatrics and for Upper Respiratory tract Infections (URI). Physicians are facing many challenges in their practice regarding antibiotics prescription. The aim of this study, the first of its kind in Lebanon, is to evaluate attitudes and practices of Lebanese pediatricians towards antibiotics prescription with a special focus on URI in order to identify challenges facing physicians. Methods This is a cross-sectional study conducted in 2018 among pediatricians registered in the Orders of Physicians. A 63-item questionnaire in English was sent by e-mail or by message to the mobile phone of all pediatricians with subsequent reminders. Data was collected from May 1st through July 2nd 2018. Statistical analysis was done using SPSS 21.0. The analysis involved descriptive quantitative statistics (means and standard deviations, frequencies and percentages). Results From a total of 1213 physicians approached, 117 agreed to participate in the study giving a response rate of ~ 10%. A high proportion of Lebanese pediatricians replied correctly on avoiding antibiotics in cases of cough (86.3%), URI (74.4%), tympanic membrane dysfunction (64.1%) and prevention of secondary infection (63%). Eighty percent of physicians prescribed antibiotics for pharyngitis without requesting a throat culture. The majority of physicians admitted that antibiotic use is considerable in their community and that antibiotic resistance is a threat on the national and international level. Forty percent of pediatricians agreed and strongly agreed that giving advice to parents reduces their antibiotic demand. The same percentage disapproved that they might prescribe antibiotics in order to gain parents’ trust. Conclusion National campaigns are required to increase parents’ education and promote judicious antimicrobials use. Implementing national guidelines, ensuring continuing medical education for doctors and regulating over the counter sale of antibiotics are highly recommended.

AimsThe disease burden of cognitive impairment is significant and increasing. The aetiology of cognitive impairment can be structural, such as in mild cognitive impairment (MCI) due to early Alzheimer's disease (AD), or in functional cognitive disorder (FCD), where there is no structural pathology. Many people with FCD receive a delayed diagnosis following invasive or costly investigations. Accurate, timely diagnosis improves outcomes across all patients with cognitive impairment. Research suggests that analysis of linguistic features of speech may provide a non-invasive diagnostic tool. This study aimed to investigate the linguistic differences in conversations between people with early signs of cognitive impairment with and without structural pathology, with a view to developing a screening tool using linguistic analysis of conversations.MethodIn this explorative, cross-sectional study, we recruited 25 people with MCI considered likely due to AD, (diagnosed according to Petersen's criteria and referred to as PwMCI), 25 healthy controls (HCs) and 15 people with FCD (PwFCD). Participants’ responses to a standard questionnaire asked by an interactional virtual agent (Digital Doctor) were quantified using previously identified parameters. This paper presents statistical analyses of the responses and a discussion of the results.ResultPwMCI produced fewer words than PwFCD and HCs. The ratio of pauses to speech was generally lower for PwMCI and PwFCD than for HCs. PwMCI showed a greater pause to speech ratio for recent questions (such as ‘what did you do at the weekend?’) compared with the HCs. Those with FCD showed the greatest pause to speech ratio in remote memory questions (such as ‘what was your first job?’). The average age of acquisition of answers for verbal fluency questions was lower in the MCI group than HCs.ConclusionThe results and qualitative observations support the relative preservation of remote memory compared to recent memory in MCI due to AD and decreased spontaneous elaboration in MCI compared with healthy controls and patients with FCD. Word count, age of acquisition and pause to speech ratio could form part of a diagnostic toolkit in identifying those with structural and functional causes of cognitive impairment. Further investigation is required using a large sample.
