Treatment results of 30 patients with diaphyseal fractures of shin bones are presented. All patients were treated by transosseus osteosynthesis tehnique with rod apparatus developed by author. Good outcome of treatment was received in 28 (93,3%) patients, satisfactory - in 2 (6,7%) in one - three years after operation. This method of treatment permits to perform a closed accurate reposition of bone fragments, to achieve the stable fixation for the whole period of treatment and to mobilize patients on second day after operation.

Objective The aim of this study was to evaluate epoxy-pin external skeletal fixation technique for the treatment of open fractures in calves and foals. Study Design Twenty-eight calves and four foals (weighing 45–105 kg) with fractures distal to the stifle or elbow made the subject for the retrospective study. The pins (2.0–3.0-mm Kirschner wires, crossed at 60–90°) were fixed at least at two locations in both proximal and distal bone fragments as per the case situation. The pins in the same plane were bent (∼2 cm from the skin) towards the fracture site or joint and were joined using an adhesive tape (additional pins used when required) to make a temporary scaffold of connecting bars or rings. Thoroughly mixed epoxy putty was applied along the pin scaffold (the epoxy columns were 20–25 mm diameter) and allowed to set for 45 to 60 minutes. All animals were evaluated based on various clinical and radiographic observations made at regular intervals. Results The epoxy-pin fixation was easy to apply and provided stable fixation of bone as indicated by early weight bearing, and fracture healing within 45 to 60 days (17/32 cases). The functional recovery was good to very good in 14 animals and satisfactory in nine cases by 12 months after removal of the fixator. Conclusions The multiplanar epoxy-pin external skeletal fixation provides stable fixation of unstable open fractures distal to the stifle or elbow joint; hence, it can be used to treat a variety of fractures in calves and foals weighing up to approximately 100 kg, especially open infected fractures of lower limb, which are difficult to treat by conventional techniques.

Summary. The main condition for preventing complications of intramedullary blocking osteosynthesis of long bones is following the tactical and technical principles of osteosynthesis and conscious rejection of such surgical techniques and manipulations that can lead to disturbances of the course of reparative processes of bone tissue. Objective. On the basis of the study and summary of the identified complications of intramedullary blocking osteosynthesis of diaphyseal fractures of bones of limbs, to determine ways to prevent complications of the reparation. Materials and Methods. The results of treatment of 403 patients who underwent intramedullary blocking osteosynthesis of the femur, tibia and humerus types A1-A3, B1–B3 and C2 by AO/ASIF were studied and analyzed. Results. Analyzing the technological complications that we have divided, depending on the consequences they led to, early (up to 2 weeks after surgery), late (2 weeks after surgery) and reparative, we found that the most common technological mistakes were incorrect preoperative planning and disruption of technology and surgery, which in 21.6% of cases led to instability of bone fragments in the fracture area, and the absence or disruption of recovery and rehabilitation of patients, which occurred in 26.3% of cases. Disorders of osteosynthesis technology, namely, significant intra-operative traumatization of bone fragments and soft tissues, and non-eliminated soft tissue interposition resulted in inappropriate consolidation of bone fragments in 39% and 13%, respectively. The main cause of reparative complications was bone marrow drilling, which was performed for 56% of the patients with delayed consolidation of bone fragments, 56.3% of the patients with false joints, and 48% of the patients with bone fractures. Conclusions. Strict following the tactical and technical principles of intramedullary blocking osteosynthesis and rejection of bone marrow drilling without indications will reduce the incidence of bone dysregeneration. The study of the effect of bone marrow drilling on reparative bone regeneration in intramedullary blocking osteosynthesis is actual and needs further deep analysis.

Treatment results of 76 patients with combined femur and shin fractures are presented. All patients were treated with transosseous osteosynthesis technique by Ilizarov. It has been shown that the technique enables to perform closed precise reposition of bone fragments, achieve the stable fixation for the whole period of treatment and mobilize patients on 2-3 day after operation. For proper formation of bone regenerate and rigid fragment consolidation it is neces- sary to disassemble the device gradually during the treatment process.

SummaryEfficacy of the rod-through-plate fixation for fracture repair was evaluated in six clinical cases of canine long bone fractures. This fixation incorporates principles of intramedullary and extramedullary osteosynthesis for transverse and short oblique fractures of long tubular bones. The plate-through-rod device is comprised of one pair of curved rods, a connecting plate and two bone screws. The connecting plate has both a hole and a channel on each end. Curved rods have a long curved part and a straight part with a hole in it for screw fixation. All components are made up of medical stainless steel. Clinical and radiographical examinations were performed from week seven to18 months after the operation. Functional abnormalities were not observed at this period in five patients and all fractures were healed. In one patient screw stripping in the proximal fragment of the femur was detected radiologically and minimal implant dislocation was noted at seven weeks after the initial repair. The rod-through-plate fixation method gives strong fixation of bone fragments with minimal traumatization of soft tissue during the operation. The design of the rod-through-plate is intended to reduce pressure of the plate on the cortex in the area of the fracture, whereas the dynamic fixation provided by the intramedullary rods may allow micromotion, thus stimulating callus formation and avoiding implant- induced osteoporosis. Clinical relevance: The rod-through-plate fixator has a simple construction and its use expands the treatment possibilities for diaphyseal fractures of long bones.

A special device assembled from standard details of Ilizarov apparatus is described. The device enables not only to perform the reduction of navicular bone fragments but provides their stable fixation as well. Conditions for walking on the first days after transosseous osteosynthesis with gradually increasing loads upon the operated extremity are created. Clinical case is presented.

Annotation: The aim of the work was to analyze the impact of the deterioration in the quality of nutrition of the population during the economic crisis that occurred after 1990 on the treatment of bone fractures. The technique: In the period from 1970 to 2008, the impact of the deterioration in the nutrition quality of the population in particular of the Kurgan Region of Russia, expressed in preserving the caloric component due to an increase in the consumption of vegetables and bread and a decrease in the consumption of meat products by more than 35%, was analyzed. bone fragments in Ilizarov treatment in 1923 patients with closed diaphyseal fractures of the bones of the leg. Results: The increase in the periods of fixation of bone fragments began in 1976 among a group of socially most vulnerable patients of retirement age, then among people over 50, among students, and after 1986 it became apparent in groups of patients of working age and reached by 2000 38% with helical and 111% (p≤0,001) for comminuted fractures. A special place in the methods of surgical treatment of bone fractures is occupied by transosseous osteosynthesis using external fixation devices of various designs.Objective: to analyze the lower jaw exoskeleton in finite element programs.Materials and research methods. During the research, 36 human lower jaws were involved from the museum of the Department of Oper-ative Surgery and Topographic Anatomy of the Volgograd State Medical University. They were scanned in 3D. All obtained images were loaded into a virtual scene reconstruction program. Fractures of the lower jaws in the corner area were modeled, an apparatus for external fixation of the lower jaw exoskeleton was installed, and the chewing load on the lower jaw was simulated. The place of application of the force was an area on a small fragment of a repositioned 3D model of the mandibular bone corresponding to the place of attachment of the masseter muscle. The evaluation of virtual studies was carried out according to the results of the stress-strain states of the bones and apparatus, the schedule of displacements of objects and the results of the analysis of the safety factor.Research results and discussion. In the course of evaluating the virtual placement of the mini-fixator wires of the apparatus for external fixation of the lower jaw exoskeleton, it was revealed that the main load is applied to the mini-fixator wires on a large fragment and the bone in the area of the wires. For the possibility of precise positioning of the osteofixers of the external fixation device, a device for the safe installation of the spokes of the lower jaw exoskeleton was also developed.Conclusion. Thus, on the basis of computer mathematical analysis, it can be argued that the developed design of the apparatus for external fixation of the lower jaw exoskeleton works under conditions of the maxillofacial region, performs reposition and fixation of bone fragments of the lower jaw under conditions of chewing functioning of the restored fracture of the lower jaw.

The aim: The aim of this research was to study causes of the development of adverse outcomes in isolated femоral diaphyseal fractures. Materials and methods: A retrospective analysis of the protocols of clinical and radiological examination of 21 patients was performed. Based on the initial expert assessment, these patients have not been established the severity of injuries due to the development of complications in the postoperative period. Conclusions: Patient-dependent, implant-dependent, and surgery-dependent causes that caused a violation of the stability of osteosynthesis (95.2%) were identified. This led to a secondary displacement of bone fragments (71.4%), delayed fracture consolidation (61.9%), the formation of pseudarthrosis (38.1%), the development of post-traumatic osteomyelitis (14,3), migration of screws from the osseous plate (47.6%), and the development of post-traumatic contracture of the knee joint (81.0%).

Treatment results of 59 patients with femoral bone defects and diaphyseal pseudarthroses were analyzed. To substitute bone defects of more than 6 cm permeable porous endoprostheses were used. In patients with normotrophic and hypertrophic pseudarthrosis stable osteosynthesis with interfragmental compression of bone fragments by shape-memory-effect tightening bows was performed. In patients with atrophic pseudarthrosis bone plasty with autograft was applied. In all patients the weight-bearing ability of the affected limb was restored. Total restoration of function was achieved in 75.6% of patients.

<p class="abstract"><strong>Background:</strong> Elastic stable intramedullary nailing for the treatment of paediatric femur and tibial diaphyseal fractures was introduced by Prevot and colleagues in 1979. It follows three-point fixation principle that provides internal support in presence of cortical contact and an intact soft-tissue envelope. This technique has many advantages, including better reduction, dynamic axial stabilization, shorter hospitalization with early rehabilitation and low complication rate.</p><p class="abstract"><strong>Methods:</strong> This is a prospective observational study done at Kauvery Medical Centre, Trichy between May 2017 to May 2018 consisting of 39 children between age 5 to 16 years with diaphyseal fractures of femur and tibia. The fractures were treated by closed reduction and internal fixation with titanium elastic intramedullary nailing. The patients were evaluated clinically and radiologically and followed for an average of 6 months. Outcome was assessed using transcutaneous electrical nerve stimulation (TENS) scoring system used by Flynn et al.<strong></strong></p><p class="abstract"><strong>Results:</strong> Our series consisted of 39 patients (22 cases with fracture shaft of femur and 17 cases with fracture shaft of tibia), 33 males and only 6 females. Average time for radiological union was 9.89 weeks. All patients had full range of hip and ankle motion and 2 (5.1%) patients had mild restriction in knee flexion at 12 weeks.</p><p class="abstract"><strong>Conclusions:</strong> Elastic stable intramedullary nailing is an ideal method for treatment of paediatric femoral and tibial diaphyseal fractures due to lower complication rate and good functional outcome in comparison to other methods of treatment.</p>
