The article presents current data on the high epidemiological and social significance of meningococcal infection caused by Neisseria meningitidis W in the Russian Federation and abroad. The article contains a review of the literature on the clinical features of the course of generalized forms of meningococcal infection caused by serogroup W. The clinical observation of a rare variant of generalized meningococcal infection with the development of acute epiphyseal osteomyelitis of the distal epiphysis of the right tibia, arthritis of the right ankle joint, right wrist joint, joints of 1-st right toe in an early childhood is described.

The article contains a description of the clinical observation of a patient with Fahr’s syndrome. A review of the literature on this rare neurodegenerative disease, Fahr’s disease, and Fahr’s syndrome is presented. An observation of a 54-year-old patient Sh. with Fahr’s syndrome is presented. The authors of the article set themselves the goal of drawing attention to Fahr’s syndrome, which is the sphere of interests of doctors of various specialties. Raising the awareness of doctors about this disease will contribute to its timely diagnosis and treatment.

Background: Palmar pits are intriguing and are found in association with diverse dermatologic conditions. Some are innocuous, whereas others have sinister implications. Objective: Six cases of asymptomatic palmar pits are described, and the literature is reviewed in brief. Method and Results: Of the six patients, five were male and one was female, with a mean age of 63.5 years. In all six patients, the palmar pits were noticed incidentally during a cutaneous examination for some other unrelated dermatoses, except in one in whom the pits were the reason for visiting a dermatologist. The number of pits varied from one to nine. Dupuytren contracture was observed in five patients. Sole involvement was seen in only one patient. None had overlying hyperkeratosis or associated symptoms relating to palmar pits. Conclusions: Palmar pits may have sinister implications in conditions such as nevoid basal cell carcinoma syndrome or Cowden syndrome. The association of palmar pits and Dupuytren contracture seems to be common but is underreported. Palmar pits may be the clinical cutaneous sign of systemic diseases and may sometimes provide a diagnostic clue to other dermatoses. This report emphasizes the significance of the art of clinical observation in dermatology.

The article presents a brief review of the literature and clinical observation of glutaric aciduria type 1 (GA1) in a child, manifested by encephalitis-like crisis and subdural hematomas, which were initially regarded as the consequences of traumatic brain injury. Based on the analysis of clinical and neuroimaging data, the diagnosis of GA1 was assumed, which was later confirmed by molecular genetic research. After therapy with the inclusion of a specialized diet with protein restriction, some improvement in motor activity was noted. The presence of bilateral subdural hematomas dictates the need for differential diagnosis with GA1. A highly informative method of diagnosing GA1 is brain MRI.

Alzheimer's disease affects an estimated 2 million elderly in the U.S. and challenges primary care physicians to assist caregivers in dealing with the daily management of these patients. To support the clinical observation of weight loss in Alzheimer patients despite adequate food intake, we reviewed the existing literature. To date, eight international studies have focused on nutrition in Alzheimer's disease and all have found weight loss. It is not clear whether this weight loss is a component of or a consequence of the disease.These findings suggest systemic, metabolic alterations in Alzheimer's disease. They require further investigation as to their nature and as to their appropriate recognition and management to retard the deteriorating effects of chronic weight loss and malnutrition. Finally, some reports lead to speculation that nutritional strategies may improve cognitive function.
