The practice of implementing the national project “Healthcare” at the regional level indicates a lag in the set rates, which causes the need to adjust the organizational structure of the healthcare system of the constituent entities of the Russian Federation. At the same time, the implementation of the recommendations of the Ministry of Health of the Russian Federation regarding the formation of a network of medical organizations of primary health care in combination with the recommendations of the Ministry of Health on the creation of centers for organizing primary health care in the regions creates the preconditions for building regional clusters in the field of healthcare in the constituent entities of the Russian Federation.The purpose of this study is to find ways to eliminate the lag of the regions of the Central Federal District of the Russian Federation in terms of achieving the goals set by the President of the country in the implementation of the national project “Healthcare”, focusing on the possibility of building a cluster policy in the process of developing a strategy for the development of modern Russian regions.The research methods are based on a systematic approach to the construction of regional clusters and the analysis of documents that make it possible to perform both statistical and content analysis, as well as methods that have found application in the development of strategically significant decisions, which include the classical analysis of external and internal environment factors, and the construction method. morphological matrices, and the Foresight method when focusing on the national development goals of the country by 2030, as well as design and programming methods typical for the strategic management of socio-economic systems.The results of the study are to develop an approach to resolve the accumulated problems that hinder the achievement of the set national development goals of the country until 2024 and until 2030, based on the formation of the cluster policy of the region in the health sector, implemented by the methods of strategic management of socio-economic systems, with the subsequent adjustment of the regional development strategy and the subsequent formation of the cluster policy of the region as a whole.The conclusions of the study are that the implementation of a sequence of procedures aimed at building a cluster focused on the implementation of the national project “Health” will allow the regions to preserve the population, as well as the health and well-being of people.

The article is devoted to the features of the organization and accounting of payment for medical care provided by medical institutions. The article highlights the trends in reforming the system of payment for medical care, which include the use of per capita payment based on the principles of Fund maintenance. The review of the current regulatory framework in the field of financial support of medical care was conducted. Based on the analysis of models of per capita financing of primary health care in the subjects of the Russian Federation, their insufficient focus on achieving final results is noted. The article deals with the organizational mechanism and accounting mechanism of payment for medical care in health care institutions.

Researchers in many countries are looking for effective ways to organize payment for primary health care and conclude that the method of per capita financing of clinics with fund-holding has both positive and negative aspects, which necessitates their obligatory consideration when deciding on the use of this payment method. The article, based on an analysis of the experience of foreign countries, shows that equal access to primary health care is ensured through the expansion of coverage of the population by services and regulation of the distribution of funds for health services. To improve the efficiency of the health system certain mechanisms of fine-tuning the existing methods of planning, organization and payment of health care, aimed at the end results of activities, are needed. Exploring the historical and international aspects of primary health care will result in more efficient implementation of the tasks of modernizing the primary health care level of the Russian Federation, approved by the Russian Government’s Resolution as of 09.10.2019 No. 1304, as well as of the activities within of the federal project “Development of the Primary Health Care System” being a part of the national health project “Health” developed in compliance with the Decree of the President of the Russian Federation of May 7, 2018 No. 204 “On the national goals and strategic objectives of the development of the Russian Federation until 2024”. The purpose of the study is to analyze the ways to pay for medical care in foreign countries and compare it with the Russian experience of per capita payment on the principles of fund-holding. Noncommunicable diseases, mainly cardiovascular diseases and cancers, and external causes account for more than 80% of mortality in Russia. The leading causes of death and disability are directly associated with behavioural risk factors, physical inactivity being one of them. Until a few years ago, a clear standard recommendation on physical activity (PA) counselling for general practitioners did not exist in Russia. In 2010, the guidelines on physical activity counselling for primary health care providers were developed. On the basis of these guidelines the national recommendations on PA were developed in 2011. They were included in the national recommendations on Cardiovascular Prevention of the Society of Cardiology of the Russian Federation. In 2012 the guidelines were adopted by the Ministry of Health of the Russian Federation and recommended to primary health care providers of all Russian regions. One of the reasons why primary health care providers are not involved enough in counselling their patient regarding a healthy lifestyle – PA as well as smoking, healthy nutrition or other habits – is that these consultations are not covered by health insurance companies. Other barriers are the absence of the topic of a healthy life-style in pre-graduate and post-graduate curricula of medical universities as well as limited counselling skills in physicians. In 2011 an educational training course for primary health providers based on the guidelines described above was established by the National Research Centre for Preventive Medicine. The further implementation of the PA recommendations depends on the one hand on their inclusion in the curricula of medical universities and on the other hand on the creation of ways for involving and motivating primary health care providers to counsel patients in this area.

Significance. The existing personnel imbalance in health care, including the number of medical staff and occupied positions in the central district hospitals (CDH) of the constituent entities of the Russian Federation and structural units of these hospitals, makes it difficult to improve availability and quality of primary health care (PHC). Development and implementation of the standard CDH models with the standard staffing table, on the basis of which it is possible to form the optimal structure of the central district hospital with the optimal staffing table based on the justified need for human resources, will contribute to improving quality and availability of primary health care at the municipal level. Purpose of the study: to significantly substantiate the regular number of medical personnel of the central district hospital with differently sized catchment areas. Material and methods: Forms of the federal statistical observation (FSN) No. 30 in the pilot territories in three constituent entities of the Russian Federation in the three different federal districts, procedures for care delivery by specialty. Analytical and statistical methods. Results. Actually, a doctor position should be included in the CRH staffing table if the standards allow for the full-time position. Certain health specialties allow for part-time positions if the outpatient and inpatient positions of the specialists can be combined into one full-time position, or part-time positions in the related specialties in the polyclinic with due regard to the population demand. Results. The structure of the CDH polyclinic and the list of full-time medical positions considerably differ depending on the size of the attached population. The adequate number of specialty doctors is ensured by the CDH work organization, distribution of specialists across separate subdivisions that are part of the CDH structure, and the use of telehealth technologies.

The article presents main properties of the federal projects of the National project «Health care», as well as the main activities of those projects, in which Roszdravnadzor has a particular interest. The article depicts the control and monitoring system applied by the Department of state control of implementation of state healthcare programs of Rosdravnadzor, which allows to detect in real time the subjects of the Russian Federation that have the highest risk of falling short of the targeted indicator of the regional projects and to take this information into account during the development of the plan for the control and surveillance activities.
