Objective of the study was to show the mean values of correction for near vision and to discuss the presbyopic correction in pseudophakic patients. Setting was the Eye department where authors work. Inclusion criteria for 55 patients were native or corrected distant vision of 0.8-1.0 on Snellen's chart; 0,6 on Jagger's chart for near vision; round pupil and good position of the implant. Biometry of the anterior chamber depth with Alcon biophysics during distant and near vision was performed in our study. ?Hi square" test was carried out and it was concluded that patients younger than 59 years (41 eyes) had median correction of +2.0 dsph, while patients older than 60 years (36 eyes) had correction of+3.0 dsph, but it was not statistically significant. There was no statistically significant difference of the correction between pseudophakic (41) and phakic (19) eyes in patients younger than 59 years. The anterior movement of the IOL was 0.18 mm in the younger group and 0.15 mm in the older group. With good IOL movement and new materials which could have changeable refractive power, the problem of pseudophakic correction for near vision might be solved.

Abstract Background To evaluate changes in anterior segment morphology on anterior segment optical coherence tomography (AS-OCT) following pars plana vitrectomy (PPV) without tamponade. Methods Patients who underwent PPV without tamponade for epiretinal membrane were evaluated. Eligible patients underwent intraocular pressure (IOP) measurement and AS-OCT preoperatively as well as 1 month and 6 months post-operatively. Anterior chamber width (ACW), anterior chamber depth (ACD), trabecular iris angle (TIA), angle opening distance at 500 and 750 µm (AOD), and trabecular iris space area at 500 and 750 µm (TISA) at four quadrants were recorded and analyzed. Additionally, the mean values of TIA (MTIA), AOD (MAOD), and TISA (MTISA) for each eye (mean of four quadrants) were analyzed. Results 23 patients completed the study. The mean age of participants was 56.4 ± 3.6 years of age and 13/23 (56%) were female. Mean IOP of patients was 18.1 ± 1.1, 18.3 ± 1.1, and 18.1 ± 1.2 preoperatively,1 month post-operatively, and 6 months post-operatively, respectively. (p = 0.83). No difference was detected post-operatively in measurements of ACW, ACD, MTIA, MAOD500, MAOD750, MTISA500, and MTISA750. Conclusion Pars plana vitrectomy without tamponade was not associated with changes in anterior chamber morphology.

Abstract Background: To create an anterior chamber depth (ACD) regression model for adult cataract surgery candidates from China, and to evaluate the distribution of their ocular biometric parameters.Methods: The ocular biometric records of 28,709 right eyes of cataract surgery candidates who were treated at Aier Eye Hospitals in nine cities from 2018 to 2019 were retrospectively analyzed. All measurements were taken with IOLMaster 700. We included patients who were at least 40 years old and were diagnosed with cataract.Results: The mean age of the patients was 68.6 ± 11.0 years. The mean values recorded were as follows: axial length (AL), 24.17 ± 2.47 mm; mean keratometry (Km) value, 44.09 ± 3.25 D; corneal astigmatism (CA), 1.06 ± 0.98 D; ACD, 3.02 ± 0.45 mm; lens thickness (LT), 4.52 ± 0.45 mm; central corneal thickness (CCT), 0.534 ± 0.04 mm; and white to white (WTW) corneal diameter, 11.64 ± 0.46 mm. The proportion of patients with long axial length (AL >25 mm) decreased with age. ACD, LT, AL, Km, WTW, and age were correlated. In the multivariate regression analysis of ACD, which included LT, AL, WTW, sex, Km, CCT, and age, there was a reasonable prediction with adjusted R2 = 0.629.Conclusions: The results show that high myopes are inclined to schedule cataract surgery at a younger age. LT and AL were found to be important factors that affect ACD. This study provides reference data for cataract patients from China.

Abstract Purpose: To evaluate the anatomical effects of implantable phakic contact lens (IPCL) (Care Group, India) on anterior segment and its visual outcomes .Patients and methods: In a prospective interventional case series study, 60 highly myopic eyes of 32 patients were subjected to IPCL implantation in the Ophthalmology Department of Minia University Hospital, Egypt from January 2019 to June 2021. All patients had complete ophthalmic examination and were followed up for 1 year. Pentacam was used for preoperative and postoperative estimation of anterior chamber depth (ACD), anterior chamber angle (ACA), anterior chamber volume (ACV) and IPCL vault in the 1st, 3rd, and 12th months. Assessment of corneal endothelium was done using specular microscope preoperatively and after 12 months. Preoperative and postoperative refraction and visual acuity were measured. Results: There was a statistically significant decrease in ACD, ACA, and ACV. There was no significant difference between preoperative and postoperative mean intraocular pressure (IOP) by the 12th month (P=0.163). The mean preoperative endothelial cell count (ECD) was significantly reduced from 2929.3±248 cells/mm2 to 2737.9±303 cells/mm2 at the 12th month (P<0.001). with a statistically highly significant improvement of mean Log Mar uncorrected visual acuity (UCVA) from 1.48±0.19 preoperatively to 0.46±0.11 by the end of follow up (P<0.001) with insignificant difference between preoperative best corrected visual acuity (BCVA) and postoperative UCVA (P=0.209). In the 12th month, the mean vault was 240±540 μm. No sight threatening complications occurred.Conclusion: Although IPCL induced anatomical changes, it was safe and effective for correction of high myopia.

Purpose:  To determine the Anterior Chamber depth and Intraocular pressure change following uncomplicated phacoemulsification. Study Design:  Quasi experimental study. Place and Duration of Study:  Layyton Rehmatullah Benevolent Trust Free Eye and Cancer Hospital, Lahore from 16/12/2017 to 15/06/2018. Methods:  One hundred and thirty nine patients who came to Layyton Rehmatullah Benevolent Trust Free Eye for phacoemulsification and IOL implantation were included in the study. Anterior Chamber depth was measured with IOL Master and intraocular pressure was determined using Goldmann Applanation Tonometer one day before and one month after surgery. Data was recorded on a self-designed proforma. Comparison between pre- and post-operative data with respect to change was analyzed by chi-square test. P-value ≤ 0.05 was considered significant. Results:  The mean intra-ocular pressure dropped from 14.36 ± 4.19 mmHg to 12.14 ± 4.26 mmHg 1 month after surgery with a mean drop of 2.21 ± 0.65 mmHg while the mean anterior chamber depth increased from 2.31 ± 0.08 mm to 3.59 ± 0.37 mm with a mean increase of 1.29 ± 0.36 mm. There was no statistically significant difference in the mean change of intra-ocular pressure and anterior chamber depth across various subgroups based on patient’s age, gender and duration of cataract. Conclusion:  Phacoemulsification and IOL implantation not only improves the visual acuity by removing the cataract but also deepens the anterior chamber and decreases intra-ocular pressure, which can have a beneficial effect in glaucoma patients. Purpose: The aims of this study were to compare the biometric parameters and axial lengths of eyes with phacomorphic glaucoma and mature cataract and to identify differences that might predispose to development of phacomorphic glaucoma. Methods: Three hundred forty-two patients were enrolled in this retrospective study. The eyes were divided into four groups—Group (G)1: phacomorphic glaucoma ( n = 29), G2: mature cataract ( n = 313), G3: contralateral phacomorphic glaucoma ( n = 29), and G4: contralateral mature cataract ( n = 313). Central corneal thickness and anterior chamber depth were assessed by optical low-coherence reflectometry (Lenstar LS 900®; Haag-Streit AG, Switzerland), while axial length was determined by A-scan ultrasound biometry. Results: The mean central corneal thickness of G1 was significantly higher than in other groups ( p < 0.001) and the mean anterior chamber depth of G1 was the lowest among the groups ( p < 0.001). Also, G2 had lower mean anterior chamber depth than G4 ( p < 0.001) and G3 had lower mean anterior chamber depth than G4 ( p = 0.007). Anterior chamber depth less than 3.27 mm had the higher odds ratio for distinguishing G3 versus G4 (odds ratio = 10.79, p < 0.001). Furthermore, patients aged ⩾68.9 years had the higher odds ratio for distinguishing G1 versus G2 (odds ratio = 2.82, p = 0.019). There was no significant difference in the presence of pseudoexfoliation material between G1 and G2 ( p = 0.057). There were no significant differences in axial length values among the four groups ( p = 0.097). Conclusion: Advanced age and shallow anterior chamber depth were found to be risk factors for developing phacomorphic glaucoma, but the presence of pseudoexfoliation material was not found to play a role as a risk factor in phacomorphic glaucoma development.

Purpose.To compare between a new optical biometer (AL-Scan, Nidek Co., Aichi, Japan) and an anterior segment optical coherence tomographer (Visante AS-OCT, Carl Zeiss Meditec, Dublin, USA) for measuring central corneal thickness (CCT), anterior chamber depth (ACD), and aqueous depth (AD).Methods.Sixty-three eyes of 63 normal subjects were examined with AL-Scan and Visante AS-OCT in this prospective study. One eye per subject was measured three times with both devices to record their CCT, ACD, and AD. All procedures were performed by the same operator. Agreement between the two devices was assessed using pairedt-tests, Bland-Altman plots, and 95% limits of agreement (LoA).Results.The mean CCT, ACD, and AD measured by AL-Scan were538.59±27.37 μm,3.70±0.30 mm, and3.16±0.30 mm, respectively. The mean values obtained by the Visante OCT were536.14±26.61 μm for CCT,3.71±0.29 mm for ACD, and3.17±0.29 mm for AD. The mean CCT by the AL-Scan was higher than that obtained by the Visante AS-OCT (difference =2.45±6.07 μm,P<0.05). The differences in ACD and AD measurements were not statistically significant. The 95% LoA of CCT, ACD, and AD were between −9.44 and 14.35 μm, −0.15 and 0.12 mm, and −0.15 and 0.12 mm, respectively.Conclusions.Since these two devices were comparable for measuring CCT, ACD, and AD, their results can be interchangeably used in the clinic.

Aim: To study the various ocular anatomical and physiological parameters in presbyopia. Materials and methods: We studied the various ocular anatomical and physiological parameters like corneal curvature (keratometry readings: K1 and K2), central corneal thickness (CCT), anterior chamber depth (ACD), lens thickness (LT) and axial length (AL) in 100 presbyopic patients between 35 - 55 years of age. The patients were divided into two age groups: I (35 – 44 years) and II (45-55 yrs). ACD, AL and LT were measured using an Ascan. CCT was measured with ultrasonic pachymetry. Results: The CCT decreased (BE), LT increased and ACD decreased (RE) significantly with increasing age (p < 0.05). There was no significant difference in males and females. Nearly 3/4th of the total increase in lens thickness was responsible for the decrease in the anterior chamber depth and the rest, 1/4th , goes posteriorly. Corneal curvature and AL showed no significant change with age. Conclusions: The mean of CCT decreased significantly with advancing age. As age increased, the mean value of lens thickness increased and anterior chamber depth decreased. Nearly 3/ 4th of total increase in LT was anteriorly, decreasing the ACD. Corneal curvature and AL has no relation with age. Abstract This study sought to compare the biometric values and intraocular lens (IOL) power obtained by standard ultrasound and optical biometry. We examined 29 eyes in preparation for cataract surgery. None of the patients had refractive surgery or corneal anomaly. In all patients, the horizontal and vertical refractive power of the cornea was determined using a keratometer (Bausch&Lomb). The axial length of the eye was determined via A-scan ultrasound (BVI-compact-V-plus) using Hollady’s formula. The IOL power and complete biometric measurements were obtained via an IOL Master-500-Zeiss using the Hollady-2 formula. All obtained values were compared and analysed using the statistical program SPSS 20. The average age of treated patients was 71.21±1.68 years. In 16 patients with dense cataracts (55.17%), it was not possible to determine the IOL power by optical biometry. Optical biometry obtained significantly increased axial length values of 24.04±0.29 mm compared with those obtained with ultrasound biometry (23.89±0.28 mm, p=0.003). The mean refractive cornea power values of the horizontal meridian measured using a keratometer (42.50±0.47 D) and an IOL Master (42.69±0.49 D) were not statistically different (p=0.187). The mean values of the refractive cornea power of the vertical meridian obtained using a keratometer (42.62±0.48D) and an IOL Master (43.36±0.51 D) exhibited a statistically significant difference (p=0.000). The keratometer obtained statistically significant lower mean values of corneal refractive power (42.73±0.32 D) compared with those obtained with optical biometry (43.22±0.35 D, p=0.000). Ultrasound biometry obtained significantly increased the mean values of IOL power (20.19±0.48D) compared with those obtained with optical biometry (19.71±0.48 D, p=0.018). The large number of patients who receive an operation for dense cataracts indicate the need for representation of both biometric methods in our clinical practice.

Introduction: The success rate of trabeculectomy is limited by postoperative scarring. Objective: To evaluate the effectiveness of intra-operative and post-operative use of 5- Fluorouracil (5-FU) in trabeculectomy Materials and methods: Thirty consecutive eyes undergoing trabeculectomy were randomized systematically into 3 groups of ten patients each. Group A served as a control; in Group B intraoperative 5-FU was used, whereas Group C received post-operative sub-conjunctival 5-FU. The variables studied were pre- and post-operative intraocular pressure, post-operative bleb characteristics and complications. Statistics: Data were evaluated using the SPSS ver 10.0 program. ANOVA, paired t test and chi2 tests were performed. Results: The means of age in years of patients in group A, B and C were 49 ± 9.23, 56.50 ± 8.39 and 52.10 ± 8.96 respectively (p = 0.222). The means of pre-operative IOP in groups A, B and C were 37.80 ±10 mmHg, 42.00 ± 11.22 mmHg and 29.40 ± 12.82 mmHg respectively. The medians of pre-operative anterior chamber depth (ACD) in groups B and C were 3 and 2 in group A according to van Herrick's grading. The mean values of final IOP were 11.90± 3.50, 11.70 ± 4.24, 11.00 ± 2.83 mmHg (p=0.841). The median post-operative anterior chamber depth in all the groups was similar compared to the pre-operative anterior chamber depth (p= 0.510). The final bleb scores in all the groups were similar (p=0.873). Conclusion: The intra-operative and post-operative use of 5-FU in trabeculectomy is almost equally effective in terms of IOP control and bleb characteristics. ABSTRACT A comparison has been made between (A), a relatively simple method for the measurement of aldosterone secretion rate, based on paper chromatography and direct densitometry of the aldosterone spot and (B) a more elaborate isotope derivative method. The mean secretion rate in 9 normal subjects was 112 ± 26 μg per 24 hours (method A) and 135 ± 35 μg per 24 hours (method B). The »secretion rate« in one adrenalectomized subject after the intravenous injection of 250 μg of aldosterone was 230 μg per 24 hours (method A) and 294 μg per 24 hours (method B). There was no significant difference in the mean values, and correlation between the two methods was good (r = 0.80). It is concluded that the densitometric method is suitable for clinical purposes as well as research, being more rapid and less expensive than the isotope derivative method. Method A also measures the urinary excretion of the aldosterone 3-oxo-conjugate, which is of interest in many pathological conditions. The densitometric method is obviously the less sensitive and a prerequisite for its use is an aldosterone secretion of 20—30 μg per 24 hours. Lower values are, however, rare in adults.
