ABSTRACT Loss of tooth structure occurs due to caries, endodontic treatment, or fracture caused by trauma; restoring these teeth is difficult task for a clinician. Post and core is an option for teeth when excessive tooth structure is lost and teeth need to be restored with crown and bridge procedure. They can be prefabricated post with composite built up or a one-piece custom-made post. The custom-made cast metal post and core have disadvantage due to their color that imparts grayish hue to overlying ceramic restorations. Masking the metal core can enhance the esthetic of all ceramic restorations that are placed over the cast post and core. Endodontic retreatment with direct crown access is the main choice in cases of endodontic treatment failure, even if the practitioner`s effort and duration is significantly greater compared with other procedures. The retreatment with crown access repeats root canal treatment through direct access at the level of tooth crown. The objective is to clean up the canal from irritant agents, such as microorganisms, surviving from the previous therapy or passed through afterwards. Thus, a treatment restart is in accordance with the logics of root canal therapy and is preferred at any time if possible. The aim of this study was to observe, over a 4-year span, the efficiency of endodontic retreatment with direct coronary approach. From 2012 to 2016, 148 patients, with ages ranging from 18 to 65 years, were retreated endodontically. All patients were included in a regular monitoring plan of 3 months and 1-year post treatment. X-rays were performed during these observations, assessed with Orstavik criteria (1991), each radiography having assigned a periapical index (PeriApical Index – PAI). In conclusion, this study confirms that first choice of retreatment in case of endodontic treatment failure is restarting of the retreatment with direct crown access (91% success rate), failure of which leads to a surgical approach using endodontic microsurgery techniques.

ABSTRACT An endodontically treated anterior tooth requires extracoronal restoration when the tooth structure is weakened or lost due to caries, endodontic treatment, placement of previous restorations and/or is discolored. The reduced tooth structure makes retention of extracoronal restoration difficult. The purpose of post is to provide retention for the core restoration, which replaces lost coronal structure. This article describes restoration of mutilated maxillary central incisors by using cast post and core, followed with porcelain fused to metal restorations. This randomized clinical trial evaluated the influence of the type of intraradicular posts (glass fiber post or cast metal post) in the success of endodontic treatment in teeth that received single metal-ceramic restoration. The sample consisted of adult patients, who presented for the first time seeking routine dental care at the dental school over a period of 5 years. Seventy-six out of ninety-six teeth that received endodontic treatment and intraradicular post were analyzed, randomly distributed in two groups – glass fiber post (n=38), and cast metal post (n=38). The quality of the endodontic treatment was radiographically evaluated, and the apical status was assessed using the periapical index (PAI). Statistical Analyses were performed to a significance level of 5%. Descriptive analyses were used to characterize the teeth and survival curves were created using the Kaplan–Meier method. The success rate of endodontic treatment was 90.8% (n=69) including glass fiber post and cast post. When appropriate endodontic treatment was performed is possible to conclude that the success of endodontic treated teeth that received posts was not related to the post type.

A comprehensive knowledge of the root canal morphology and its variations is a basic requisite for the success of the endodontic treatment. Mandibular molars may present a third or additional root, which if located lingually is called radix entomolaris or bucally is referred as radix paramolaris. This case report shows an endodontic retreatment of a radix entomolaris and details some variations in the approach to guarantee a successful terapy.

Background. Esthetic appearance of primary anterior teeth is one of the major demands in the dental field. Destructed anterior primary teeth due to caries is considered a major issue due to the difficulty in regaining the esthetic crowns and attaching them until the normal exfoliation time. There have been many attempts and tools used to attach the composite crowns to the treated canals of primary anterior teeth. The study evaluates endodontic treatment for destructed primary maxillary incisors with a glass fiber-reinforced post as a retentive tool to hold the esthetic composite crowns until the normal exfoliation time of primary incisors. Case Presentation. A four-year-old child attending a dental clinic complained of pain of maxillary incisors. Dental examination showed irreversible pulpitis of four maxillary incisors indicated for root canal treatment and crown placement. Endodontic treatment was carried out, and a glass fiber-reinforced post was used to get successful retention for the composite crowns. Follow-up was carried out for 3 years. The 3 crowns were retained successfully until replaced by permanent incisors. One crown fell during the treatment course. Conclusion. Retention of primary teeth is one of the challenges in pediatric dentistry. Restoration of primary decayed incisors is important for child medical, physical, and psychological conditions. A glass fiber-reinforced post and core is a strong retentive tool for composite crown retention for primary incisors. This procedure opens the door for a strong tool to retain composite crowns for a long time. The glass fiber-reinforced post and core is a strong retentive tool for composite crown retention for primary incisors.

Background: The failure of endodontic treatment caused by failure of the restoration and bacterial invation. Alongside proper chemical and mechanical preparation and hermetic obturation of the root canal system, another essential factor is ensuring a permanent and hermetically-sealed restoration following endodontic treatment. Endodontic retreatment encompasses the efforts to salvage a failing previously endodontically treated tooth. As a result of advances in endodontic materials and adhesive systems, restorations in the form of fiber post and adhesively cemented crown are becoming a choice in cases of teeth that have extensive damage. Case: A case is presented a 45-year-old female was referred to Prof. Soedomo Dental Hospital with a complaint of pain in mandibular second premolar which was earlier endodontically treated. Clinical and radiographic examinations showed damaged restorations, gutta percha filling that are not hermetic and radiolucent on periapical. A successful endodontic retreatment premolar using rotary retreatment file and preparation with crown down technique using rotary progressive multiple tapering file, with comprehensive endo-prosthetic treatment using prefabricated fiber post and porcelain fused to metal crown. Conclusion: Clinical and radiographic evaluation after 6 months showed complete healing and good adaptation of the restoration. The outcomes of this case showed that failure of endodontic retreatment respond favorably to endodontic retreatment, with proper indication, prefabricated fiber post and porcelain fused to metal crown can provide an effective conservative and esthetic option for reinforcing endodontically treated teeth.
