Accidents and near-miss accidents in chemical industries are widespread. Most of the incidents occurred due to combinations of organizational and human factors. To identify the causes for an incident of an accident analysis is needed, because it reveals the possible causes behind the accidents. Accident analysis shows the human and organizational factors that support learning from the events. Literature review shows that human error plays an important role of accidents in process industries. The chapter discusses some case studies which are received very little media publicity and also no proper assessment. At first reports on the incidents were collected from newspapers and then the place was visited to conduct an interview with local people and present and past workers with the help of the PESO (M/S Petroleum and Explosive Safety Organization, Eastern Region, Govt. of India).

Managed lanes (MLs) have been implemented as a vital strategy for traffic management and traffic safety improvement. The majority of previous studies involving MLs have adopted a limited scope of examining the effect of MLs segments as a whole, without considering the safety and operational effects of the design of access to the MLs. In the study, several scenarios were tested using microscopic traffic simulation to determine the optimal access design while taking into consideration accessibility levels and weaving lengths. The studied accessibility levels varied from one to three along the studied network. Both safety (i.e., speed standard deviation, time-to-collision, and conflict rate) and operation (i.e., level of service, average speed, average delay) performance measures were included in the analysis. Tobit models were developed for investigating the factors that affect the safety measures. ANOVA and LOS calculations were used to evaluate traffic operation. The results of the safety and operational analysis suggested that one accessibility level is the optimal option in the nine-mile network. A weaving length between 1,000 feet and 1,400 feet per lane change was suggested based on the safety analysis. In addition, from the operation perspective, a weaving length between 1,000 feet and 2,000 feet per lane change was recommended. The results also showed that off-peak periods had better safety and operational performance (e.g., lower conflict rate, less delay) than peak periods. This study has major implications for improving MLs by recommending the optimal accessibility level and weaving length near access zones.

Traffic safety situation in Lithuania, despite the progress made in 2008, is still not good enough compared to the other European Union countries. Therefore, Lithuania implements the traffic safety improvement measures affecting all the elements of the traffic safety system “Road user‐road‐vehicle”. In 2008 Lithuania started implementing the road safety audit procedure which substantially contributes to the improvement of safety situation on Lithuanian roads. This article gives the analysis of road safety audits of special plans and technical designs for road construction and reconstruction in Lithuania, makes the analysis of the main deficiencies of plans and designs and gives recommendations for their elimination. Santrauka Eismo saugumo situacija Lietuvoje, nepaisant 2008 m. pasiektos pažangos, vis dar yra nepakankamai gera, palyginti su kitomis Europos Sąjungos šalimis. Atsižvelgiant į tai, Lietuvoje įgyvendinamos eismo saugumo gerinimo priemonės, darančios įtaką visiems eismo saugumo sistemos „Eismo dalyvis–kelias–transporto priemonė“ elementams. Lietuvoje 2008 m. buvo pradėta taikyti kelių saugumo audito procedūra, kuri prisideda prie eismo saugumo gerinimo Lietuvos automobilių keliuose. Straipsnyje pateikta Lietuvoje atliktų specialiųjų planų ir automobilių kelių tiesimo bei rekonstrukcijos techninių projektų kelių saugumo auditų analizė, išanalizuoti pagrindiniai planų ir projektų trūkumai, pateiktos rekomendacijos jiems šalinti.

Analysis of Factors Influencing the Density of Fatalities on National Roads in PolandIn Poland 20% of the total number of accidents occur on the national roads, which constitute 7% of the length of all roads. In the case of fatalities this share is significantly higher as it constitutes up to 36% of the total casualties. In accordance with the EU Directive (Journal of Laws EU L. 319/59) the level of road traffic safety should be raised by targeting investment on the most dangerous road sections. Finding the dependence between road and traffic factors and the number of accidents and casualties of road accidents may be helpful in predicting safety levels and selecting road traffic safety improvement measures. The paper presents the experience of other countries and preliminary tests results of the impact of the selected factors on the density and the number of road accident fatalities on the national roads in Poland.

Mining industry associated with many type of technical involvement i.e. from low to high and run by unskilled, semi-skilled, skilled and technically skilled manpower oriented. The efficiency of this industry is mainly depending on safety of work force. Productivity is proportionate to safety of workmen, machinery and working site etc. The safety is better in the mine then it gets best productivity. The accident analysis in mining industry reveals that prime cause is lack of personal safety. When the safety thoughts roaming in the minds of work force as well as in management will always make the industry way leading to safe so that personal safety also good. The personal safety of workmen is influenced by internal and external agencies of behavioral attitude stress that is at home and at work site. In this paper explained some of safety tips particularly on personal safety matters inside and outside of the work site and behavioral attitude required for work force towards achieving goal of industry motive towards reach of Zero Hazardous, Zero Incidents and Zero losses.
