In modern medical literature, discussions continue about neuropathies of the facial nerve in children,disease factors, clinical and neurological features, the severity of the course, the difficulties of diagnosis and prognosis of the disease. The work is devoted to the analysis of the neuropathy of the facial nerve in children and adolescents, depending on the clinical forms of the disease, using electroneurosympathetic indicators, the latency period, the blinking reflex and thenatureof the lesion of the facial nerve accompanied by pain syndrome

Relevance. Various factors can cause facial neuropathy. Iatrogenic facial nerve injury in childhood and adolescence is a complicated medical, psychological and legal problem. Risk assessment of the facial nerve injury during the elective maxillofacial surgeries is required depending on the localization of the procedure. The statistical assessment is very important for the evaluation of the possible iatrogenic facial neuropathy in children and adolescents during the elective maxillofacial surgeries.Materials and methods. 715 medical records for 2017 from the Department of Pediatric Maxillofacial Surgery of the MSUMD Clinical Center for Maxillofacial, Reconstructive and Plastic Surgery were analyzed.Results. There was a risk of injury to the trunk or branches of the facial nerve during surgery in 121 cases (16,9%) for the technical complexity of the surgical approach and the pathology location.Conclusions. There is a high risk of the facial nerve injury during the elective maxillofacial surgery in children and adolescents due to the complex anatomy of the area. Intraoperative neuromonitoring is recommended to prevent iatrogenic neuropathy of the facial nerve.

Aim. We defined the prevalence of back pain in children and adolescents aged 917 years with spinal deformities. Material and methods. The cross-sectional study included 230 students with different spinal deformities aged 917 years. The prevalence of back pain, intensity, location, and situations in which it occurred were assessed via questionnaire. Results. Among 230 respondents, 186 (80.9%) admitted that they had experienced back pain (mainly in the lumbar spine) at various frequencies within the year preceding the study. Mild pain was prevalent (71% of respondents). Girls experienced back pain significantly more frequently than boys. Conclusions. Back pain in children and adolescents requires clinical and instrumental examination, including X-ray. Back pain is a frequent phenomenon in children with different spinal deformities. Тhe incidence of pain in children and adolescents with spinal deformities in our study is statistically higher than that of healthy individuals of the same age group.

In recent years medical practitioners more often pay their attention on role of pathology of different organs and systems of human’s body which are associated with connective tissue dysplasia. The importance of this problem is caused by the great prevalence of connective tissue dysplasia, systemacity of damage, high probability of different diseases’ formation. Connective tissue dysplasia is the violation of the connective tissue structure during embryonic and postnatal periods because of genetically modified fibrillogenesis of extracellular matrix, leading to homeostasis disorder on tissue and organ levels with the progressive course. There was held the clinical neurological examination with 120 patients, who had neurological features of vertebral syndrome of lumbosacral spine. Analysis of the clinical examination results demonstrated that patients with vertebral syndrome of lumbosacral spine (p<0,05) more common can occur pathology of joints and varicose veins of the lower extremities. Significantly (p<0,05) (scoliosis, kyphosis, kyphoscoliosis, tendency to dislocation, stretching the ligaments) increase the duration of treatment and the expression of a pain syndrome. The results of examination and monitoring of patients in the dynamics of the treatment showed that patients with vertebral syndrome of lumbosacral spine (p<0,05) more often have anatomical changes in the lumbosacral spine than patients without evidence of DST.

AbstractMuch controversy remains on the current management of Chiari I deformity (CID) in children, with many clinical, surgical and ethic-legal implications. The Brazilian Society of Pediatric Neurosurgery (SBNPed, in the Portuguese acronym) has put together a panel of experts to analyze updated published data on the medical literature about this matter and come up with several recommendations for pediatric neurosurgeons and allied health professionals when dealing with CID. Their conclusions are reported herein, along with the respective scientific background.

Wegener’s granulomatosis or granulomatosis with polyangiitis is a necrotizing vasculitis affecting both arterioles and venules. The disease is characterized by the classical triad involving acute inflammation of the upper and lower respiratory tracts with renal involvement. However, the disease pathology can affect any organ system. This case presents Wegener’s granulomatosis presenting with facial nerve palsy as the first manifestation of the disease, which is rarely reported in medical literature.
