Objective: To evaluate the functional outcome and quality of life of patients with rheumatoid arthritis (RA) who underwent resection arthroplasty of the first to fifth metatarsal heads. Methods: Prospective case series study in which 8 feet of 5 patients underwent resection arthroplasty of the five metatarsal heads via a plantarapproach to the lateral rays and via a medial approach to the first metatarsal. Dressing and orthotics were used for 3 months to correct the position of the toes. Patients were evaluated in the preoperative period and at least one year after the procedure using two functional evaluation scales (AOFAS lesser toes and AOFAS hallux) and one quality of life scale (EQ-5D). All results were subjected to statistical validation using theMann-Whitney U test. Results: The subjects had a mean age of 71 years, with a prevalence of the female gender. The mean AOFAS lesser toe and AOFAS hallux scores increased from 38.6 and 24.5 preoperatively to 85.7 and 83.1 postoperatively, respectively (p<0.001). The quality of life scores measured through the EQ-5D improved from 8.0 to 5.6, on average. Conclusion: Resection arthroplasty of the five metatarsal heads improves the function and quality of life in the short term of patients with rheumatoid foot.Level of Evidence IV; Therapeutic Studies; Case Series.

RESUMOObjetivo: avaliar a adesão aos tuberculostáticos e a qualidade de vida de pacientes com tuberculose pulmonar. Método: trata-se de um estudo quantitativo, do tipo série de casos, com 18 pacientes, utilizando-se um questionário, a escala de qualidade de vida SF – 36 e o teste de Morisky e Green. Armazenaram-se os dados em planilhas do Excel 2013 e os analisaram com recurso de estatística descritiva e inferencial, utilizando o SPSS 24.0. Apresentaram-se os resultados em de tabelas. Resultados: pontua-se que 12 pacientes (66,7%) tinham alta adesão e seis (33,3%) tinham média adesão aos tuberculostáticos. Avalia-se que, acerca da qualidade de vida, os domínios aspectos sociais (68,75 ± 25,28) e saúde mental (80,00 ± 16,09) alcançaram as médias mais elevadas. Conclusão: conclui-se que a tuberculose compromete a qualidade de vida das pessoas infectadas, pois, além da implicação física, há o comprometimento emocional e psíquico. Torna-se relevante capacitar os profissionais de saúde para o diagnóstico precoce da tuberculose, visando à melhoria da qualidade do atendimento, com ganhos para a qualidade de vida dos usuários e a maior adesão ao tratamento. Se torna relevante capacitar a los profesionales de salud para el diagnóstico precoz de la tuberculosis, buscando la mejora de la calidad de la atención, con ganancias para la calidad de vida de los usuarios y la mayor adhesión al tratamiento. The purpose of this study was to determine the effect of functional limitations and fatigue on the quality of life (QoL) in people with multiple sclerosis (MS). A descriptive case series study at Lagoa Hospital - Rio de Janeiro, Brazil was carried. The main outcome measurements were demographic variables, QoL (SF-36 v.1), disability (EDSS), motor function of the upper limb (Box & Blocks test), Tone (Modified Ashworth Scale), gait (Hauser ambulatory index) and fatigue (Fatigue Severity Scale). Sixty one patients fulfilled the study criteria. The mean age of patients was 39 years and 74% of patients were female. The most of cases presented mild disability (EDSS<3.5). A decrease was found in all domains of QoL. It was found association between physical functioning and the variables of EDSS, fatigue, lower limb tone and gait. Gait, disability, hypertonia of the lower limbs and fatigue negatively affected QoL in people with MS.

Objective: To assess the functionality of the affected upper limb in children diagnosed with hemiplegia aged between 4 and 8 years after applying low-intensity modified constraint-induced movement therapy(mCIMT). Methods: Prospective case series study. A mCIMT protocol was applied for five weeks, with two hours of containment per day. The study variables were: quality of movement of the upper limb, spontaneous use, participation of the affected upper limb in activities of daily living, dynamic joint position, grasp-release action, grasp strength, supination and extension elbow movements. Four measurements were performed, using the QUEST scale, the SHUEE Evaluation, a hand dynamometer and a goniometer. Results: The sample was composed of 8 children with moderate manual ability. Statistically significant differences were detected in all the studied variables (p&amp;lt;0.05). The greatest increase occurred in spontaneous use from assessment 1-4 (p = 0.01), reaching 88.87% active participation in bimanual tasks. The quality of movement of the upper limb obtained a significant value due to the increase in dissociated movements and grasp (p = 0.01). Conclusion: A low dose (50 hours) of mCIMT increased the functionality of children diagnosed with congenital hemiplegia between 4 and 8 years of age with moderate manual ability.

<p><strong>Objetivo</strong></p><p>El objetivo de nuestro trabajo es estudiar las complicaciones de la cirugía en las escoliosis neuromusculares operadas en nuestro centro, así como la funcionalidad y calidad de vida de los pacientes luego del tratamiento quirúrgico.</p><p><strong>Métodos</strong></p><p>Se realizó un estudio retrospectivo, tipo serie de casos. Se revisó la historia clínica y se entrevistó a paciente y cuidador principal. Se valoró los resultados luego de la cirugía en cuanto a las complicaciones, funcionabilidad, calidad de vida y grado de satisfacción de los cuidadores.</p><p><strong>Resultados</strong></p><p>Obtuvimos un 26,7% de complicaciones, debidas a infección y alteraciones hemodinámicas. En cuanto a la funcionabilidad y calidad de vida de estos pacientes obtuvimos resultados positivos, siendo más significativos en cuanto a la capacidad de estar sentado y las actividades de la vida diaria.</p><p><strong>Conclusiones</strong></p><p>La cirugía de los pacientes con  escoliosis neuromuscular mejora la capacidad y el equilibrio de estar sentado, favoreciendo con esto la capacidad digestiva, respiratoria y de vida social del paciente. Si bien se trata de cirugías con altos indices de complicaciones muestra indices elevados de satisfacción por parte de los cuidadores.</p><p> </p><p> </p><p><strong>Abstract</strong></p><p><strong></strong><br /><strong>Introduction</strong>: The objective of this study was to evaluate surgical complications in patients with scoliosis operated on in our Center, as well as their functionality and quality of life after surgical treatment.</p><p><br /><strong>Methods</strong>: A retrospective case series study was carried out. Medical history was reviewed and the patient and primary caregiver were interviewed. Results after surgery were assessed for complications, function, quality of life and caregivers´ degree of satisfaction.</p><p><br /><strong>Results</strong>: Complication rate was 26.7%, due to infection and hemodynamic alterations. Positive results were obtained regarding function and quality of life of these patients, being more significant in terms of the ability to be seated and activities of daily living.</p><p><br /><strong>Conclusions</strong>: Surgery of patients with neuromuscular scoliosis improves the capacity and balance of sitting, favoring digestive, respiratory and social life capacity. Although rate of complications is high, satisfaction rates on the part of the caregivers are elevated.</p>

Objectives The objective of this study was to evaluate the long-term quality of life in cats following perineal urethrostomy. Methods This study comprised a retrospective case series of 74 cats with urinary obstructions that received perineal urethrostomies at the American Society for the Prevention of Cruelty to Animals (ASPCA) Animal Hospital between September 2015 and July 2017. Medical records were reviewed for information on the number of obstructions prior to surgery, urinary tract problems at the time of surgery and other potential factors influencing long-term welfare. Owners were surveyed by telephone and text 5–29 months after their cat’s surgery. Responses were compiled and analyzed to determine owner perception of welfare postsurgery, and to identify patterns in medical history and welfare outcomes. Results In total, 105 cat owners were eligible for the survey; 74 responded. Altogether, 100% of responding individuals reported at least the same quality of life postsurgery compared with the cat’s quality of life before demonstrating urinary problems (48% reported better). On a quality-of-life scale of 1–10 (10 = excellent), 100% of responding cat owners reported at least 7; 75% reported 10. Conclusions and relevance Quality-of-life outcomes for cats 5–29 months after receiving perineal urethrostomy surgery were reported to be very good. Given this finding, and that perineal urethrostomy surgery should mitigate future blockages, we suggest considering perineal urethrostomy surgery as a standard tool to manage urinary obstructions in cats. The needs of the specific client and patient, including risks, costs and welfare of the cats, should drive the options for management of urinary obstructions in male cats.

Background: Epilepsy is one of the most common neurological disorder and characterized by recurrent seizures caused by abnormal electrical activity in the brain. Epilepsy is both a medical diagnosis and a social label that can negatively impact quality of life (QoL).Aims and Objective: This study aimed to evaluate the correlation between characteristics of seizure and QoL among epilepsy patients.Materials and Methods: This is a case series study conducted at Bethesda Hospital, Yogyakarta. Subjects answer 8 questions in the SF-8 questionnaire by choosing one from several alternative answers. Sub-scale assessed in this questionnaire including general health perception (GH), physical functioning (PF), physical role functioning (RP), bodily pain (BP), vitality (VT), social role function (SF), mental health (MH), and emotional role functioning (RE). Each answer will be score and processed in a program from Optum™. The final results will be summarized into physical component (PCS) and mental component (MCS).Results: Total of the subjects were 27. Subjects were dominated by male, age< 60 years. The highest score is on the bodily pain subscale (mean: 50.00 ± 8.0739), wherease the lowest score is on global health perception (mean: 43.95 ± 7.1970). Overall, subjects in this study have a better mental status than physical status (47.14 ± 10.2093 vs 46.90 ± 7.9418). Type of seizure (PCS p: 0.794; MCS p:0.093), duration of epilepsy(PCS p: 0.832; MCS p: 0.856), history of epileptic status (PCS p: 0.141; MCS p: 0.951), the presence of neurological deficit (PCS p: 0.140; MCS p: 0.283), frequency of seizure before treatment (PCS p: 0.648; MCS p: 0.213), frequency of seizure after treatment (PCS p: 0.249; MCS p: 0.407), and frequency of seizure within the last 1 year were not significant to influence the QoL (PCS p:0.978; MCS p: 0.513).Conclusion: There is no significant characteristics of seizure that influence the QoL.Asian Journal of Medical Sciences Vol.9(6) 2018 1-6

Background The subscapularis tendon is commonly released during shoulder arthroplasty, and its integrity and repair postoperatively have been shown important to help maximize patient function. However, diagnosing subscapular tendon failure can be difficult with magnetic resonance imaging secondary to metal artifact as well as very costly. Purpose The purpose of this study was to assess the utility of ultrasound imaging in evaluating subscapularis integrity at specific time points following shoulder arthroplasty, in a blinded fashion. Secondarily, we report on the correlation between the condition of the subscapularis and quality-of-life outcome measures. Study Design Prospective case series. Methods Ultrasounds were completed preoperatively and postoperatively at 1 week as well as at 1, 3, and 6 months. Each was read by a single musculoskeletal radiologist and categorized as “intact,” “torn,” or “unclear.” Clinical outcome was evaluated using the Western Ontario Osteoarthritis Shoulder (WOOS) index at these same time points. Results The final study group consisted of 35 procedures in 33 patients (19 females and 14 males, mean age 66 ± 9 years). Three patients had postoperative subscapularis failures that were confirmed in the operating room at the time of repair. Of 24 sonographs categorized as “unclear” in the postoperative period, the majority (n = 12, 50%) were taken at 1 week. Compared to preoperative scores, patients had lower WOOS scores at 1, 3, and 6 months postoperatively ( P < .001). Correlation analysis did not reveal an association between the ultrasound readings and the WOOS scores postoperatively. Conclusion The utility of ultrasound examination of the subscapularis tendon following shoulder arthroplasty is limited by timing and may be most useful when used by the physician within clinical context. Significant improvement was noted in disease-specific quality-of-life scores regardless of the status of the subscapularis tendon as read on ultrasound.

Abstract Background Inferior turbinoplasty (IT) and adenoidectomy (Ad) are frequently resorted to in children with chronic rhinitis (CR) refractory to medical therapy. The aim of this study is to document the long-term improvement in quality of life (QOL) in children with CR following endoscopic IT with or without Ad. Methods A retrospective case series study was conducted. We searched a prospectively kept surgical database for children ≤18 years old who had CR who underwent endoscopic IT with or without Ad between 2009 and 2016 at a tertiary care children’s center. Patients with sinonasal pathologies other than CR, had craniofacial syndromes or dysmorphism and had other sinonasal procedures or trauma were excluded. Collected data included demographics, secondary diagnoses, duration of follow-up, and complications of procedures. The Glasgow Children’s Benefit Inventory (GCBI) was administered by phone to assess QOL improvement. Results One hundred sixty-five eligible subjects were identified. Eighty-nine subjects met the inclusion criteria. Data was collected for the 60 subjects that were reached. Forty-two patients had IT only while 18 had IT and Ad. The mean age was 10.7 ± 2.7 years, with 31 males and 29 females. The median duration of follow-up (25th, 75th percentile) was 38.1 months (24.6, 55.8). The median GCBI score (25th, 75th percentile) was 22.9 (6.3, 39.6) revealing an overall positive benefit in all domains. There was only one complication. Conclusions This study validates prior findings regarding improvement of QOL and safety of IT with or without Ad for children with CR and indicates it is maintained in the long term.
