Background: Many studies have shown the benefits of diathermy over scalpel for making skin incisions in terms of post operative pain and post operative analgesics requirement. The objective of the study is to compare the pain following incision by scalpel and diathermy for skin in ENT surgery.Methods: We conducted a prospective, randomized study and compared early post operative pain and analgesics requirement in patients undergoing ENT and Head and Neck surgery in Department of ENT at Kathmandu Medical College from September 2016 to August 2017. The statistical analysis was done using MS Excel and SPSS software.Results: Out of 65 participants, 31 were allocated in scalpel group and 30 were allocated in diathermy group. The mean VAS score was significantly greater in scalpel group as compared to diathermy group in post operative 12, 24 and 48 hours (P<0.05). The mean ketorolac requirement was significantly more in scalpel group than in diathermy group in post operative 24 hours. Conclusions: The early post operative pain is less in ENT-Head and Neck surgery patients with skin incision by diathermy as compared to the patients with skin incision by scalpel.

Abstract Objective Noise in operating rooms (OR) can have negative effects on both patients and surgical care workers. Noise can also impact surgical performance, team communication, and patient outcomes. Such implications of noise have been studied in orthopedics, neurosurgery, and urology. High noise levels have also been demonstrated in Otolaryngology-Head and Neck Surgery (OHNS) procedures. Despite this, no previous study has amalgamated the data on noise across all OHNS ORs to determine how much noise is present during OHNS surgeries. This study aims to review all the literature on noise associated with OHNS ORs and procedures. Methods Ovid Medline, EMBASE Classic, Pubmed, SCOPUS and Cochrane databases were searched following PRISMA guidelines. Data was collected on noise measurement location and surgery type. Descriptive results and statistical analysis were completed using Stata. Results This search identified 2914 articles. Final inclusion consisted of 22 studies. The majority of articles analyzed noise level exposures during mastoid surgery (18/22, 82%). The maximum noise level across all OHNS ORs and OHNS cadaver studies were 95.5 a-weighted decibels (dBA) and 106.6 c-weighted decibels (dBC), respectively (P = 0.2068). The mean noise level across all studies was significantly higher in OHNS cadaver labs (96.9 dBA) compared to OHNS ORs (70.1 dBA) (P = 0.0038). When analyzed together, the mean noise levels were 84.9 dBA. Conclusions This systematic review demonstrates that noise exposure in OHNS surgery exceeds safety thresholds. Further research is needed to understand how noise may affect team communication, surgical performance and patient outcomes in OHNS ORs. Graphical abstract

AbstractThe authors present a technique using electrocautery diathermy to make surgical tattoos. This method has been used in over 300 patients who underwent head and neck surgery at Aberdeen Royal Infirmary and Albany Medical College, New York, over a period of five years. A wide variety of operative procedures such as total laryngectomies and neck dissections were performed. The electrocautery surgical tattoos have a major advantage of persisting until the end of the operative procedure by which time other types of tattoos have faded. The technique is widely available, inexpensive, and has to date been complication free.

Abstract Background Thyroid and parathyroid surgery often involves the use of heated instruments for dissection. Whilst these are beneficial, accidental thermal damage to the exposed skin edges can occur, resulting in an unsatisfactory cosmetic outcome. Tonsil swabs can be used in head and neck surgery intra-operatively to control bleeding. This paper describes an alternative use for them in protecting wound edges during the procedure. Method Damp tonsil swabs are sutured onto the wound edges after the initial skin incision. They remain present for the duration of the surgery and are removed at the time of skin closure. Results The tonsil swabs provide protection and help avoid accidental injury to the skin. No complications with this technique have been experienced. Conclusion This paper describes a simple, effective and practical technique for protecting the skin during neck procedures using resources readily available in a standard ENT operating theatre.

The Norfolk and Norwich retractor is a vital tool in head and neck surgery. It is of great aid in training junior surgeons and has become an integral part of the standard neck dissection instrument set in our unit. This retractor enables good exposure of the carotid sheath, its atraumatic blunt tip retracting the carotid sheath without damage. It makes a single skin incision for neck exposure possible, rather than a Y, T or wine glass incision, avoiding a three-point junction, especially in the post-irradiated neck. In thyroid surgery it reduces the need for manual retraction thereby relieving the assistant surgeon and enhancing the quality of the learning experience.

Background: There is a paucity of information about the dental sciences related articles published in an Otorhinolaryngology journal from India. This bibliometric study aimed to audit the dental sciences related articles published in Indian Journal of Otolaryngology and Head and Neck Surgery (IJOHNS) from 2014 to 2016 over a 3 year period.Materials and Method: Bibliometric analysis of issues of IJOHNS from 2014 to 2016 was performed using web-based search from Pubmed Central. The articles published were analyzed for topic of dental sciences, type of article, type of study, international collaborations, source of funding, number of authors and authorship trends.Results:Out of the total 60 published articles related to dental sciences, original articles (42), clinical reports (14),short communication(3) and review articles(1) contribute the major share. Regarding the relationship with dental sciences, the maximum number of published articles were related to oral and maxillofacial surgery (53) followed by oral pathology and microbiology (44), oral medicine and radiology (37), prosthodontics (17) and community dentistry (3).Among the articles published in IJOHNS, oral cancer (14) followed by diseases of salivary gland (13), oral mucosal lesions (6) and reconstructive surgery (5) form the major attraction of the contributors. The largest numbers of published articles related to dental sciences were received from Gandhi Medical College, Bhopal, Madhya Pradesh (5) and Basavatarakam Indo American Cancer Hospital and Research Centre, Hyderabad, Andhra Pradesh (3).Conclusion: This paper may be considered as a baseline study for the bibliometric information regarding dental sciences related articles published in an Otorhinolaryngology journal from India. IJOHNS have published dental sciences related articles based on the merit of the topic irrespective of the speciality of the author.

This paper focuses on the statistical analysis of mimetic muscle rehabilitation after head and neck surgery causing facial paresis in patients after head and neck surgery. Our work deals with a classificationan evaluation problem of mimetic muscle rehabilitation that is observed by a Kinect stereo-vision camera. After a specific brain surgery, patients are often affected by face palsy, and rehabilitation to renew mimetic muscle innervation takes several months. It is important to be able to observe the rehabilitation process in an objective way. The most commonly used House–Brackmann (HB) scale is based on the clinician’s subjective opinion. This paper compares different methods of supervised learning classification that should be independent of the clinician’s opinion. We compare a parametric model (based on logistic regression), non-parametric model (based on random forests), and neural networks. The classification problem that we have studied combines a limited dataset (it contains only 122 measurements of 93 patients) of complex observations (each measurement consists of a collection of time curves) with an ordinal response variable (four HB grades are considered). To balance the frequencies of the considered classes in our data set, we reclassified the samples from HB4 to HB3 and HB5 to HB6—it means that only four HB grades are used for classification algorithm. The parametric statistical model was found to be the most suitable thanks to its stability, tractability, and reasonable performance in terms of both accuracy and precision.

Objectives: To evaluate 113 cases of nasal foreign body removal with regard to type of foreign body, location, complications, techniques for removal, age , genderand to present the results of theevaluation.Methods: Between April 2009 to May 2011, a total of 113 nasal foreign bodies were attended inOutpatient Department ofENT, Head and Neck Surgery in Sir Salimullah Medical College, MitfordHospital,Dhaka. A retrospective reviewwas undertaken to evaluate the parameters related to the nasal foreign body and their removal and the data wereanalyzed.Results:113 cases of nasal foreign bodies were attended in Outpatient department of ENT, Head and Neck Surgery in Sir Salimullah Medical College, Mitford Hospital, Dhaka. Of these 113 cases, 101cases were removed in outpatient department and 12 cases were admitted and were removed under general anaesthesia. We found higher incidence in patients between ages 2 and 5 years. The distribution of nasal foreign body was 44.25% in male and 55.75% in female.Conclusion:Nasal foreign bodies are encountered daily in our routine clinical practice in the pediatric age group. General anaesthesia is required in uncooperative agitated patients or impacted foreign body to avoid complications.Button batteries and penetrating foreign body must be treated immediately.Bangladesh J Otorhinolaryngol; October 2015; 21(2): 102-109

Introduction: The nose is the central most part of the face, so any nasal deformities may cause a high level of psychological distress on one’s quality of life. Septorhinoplasty is a surgical procedure used to correct both functional as well as aesthetic problems of the nose. It enhances facial harmony and increases self-confidence. The objective of this study is to find out the prevalence of out-patients who underwent septorhinoplasty in a tertiary care centre in Nepal. Methods: A descriptive cross-sectional study among 5,760 patients who visited the outpatient department of Otolaryngology-Head and Neck Surgery of Kathmandu Medical College from April 2019 to September 2020. Ethical approval was taken from the Institutional Committee of Kathmandu Medical College. A convenient sampling technique was used. The preoperative and postoperative assessment was done with the help of a rhinoplasty outcome evaluation questionnaire. Statistical Package for the Social Sciences is used for analysis. Point estimate at 95% Confidence Interval was calculated along with frequency and proportion for binary data. Results: The prevalence of septorhinoplasty was 117 (2.03%) during the study period (95% Confidence Interval= 1.66-2.40). Among 117 patients, 67 (57.3%) were males and 50 (42.7%) were females. Among 117 patients, 85 (72.6%) patients underwent an open approach, while 32 (27.4%) patients underwent closed approach rhinoplasty for the correction of both aesthetic and functional problems. Conclusions: This study concludes the prevalence of septorhinoplasty is low which may be due to the COVID-19 pandemic during the study period.
