Abstract Understanding individual factors (e.g., personality) associated with self-reported memory problems is important to refine identification of individuals at a higher risk of developing Alzheimer’s disease (AD). Using multilevel modeling, we examined the association of family history of AD and personality traits with self-reported memory problems in older adults (n = 421; 72.21% White; 62.95% female; Mage = 76.69). Results showed that individuals with a family history of AD reported more frequent memory problems and greater one-year memory decline. Similar findings were reported for individuals with higher extraversion scores. Further, older adults with higher neuroticism scores reported greater one- and ten-year memory decline. Neuroticism was positively related to frequency of memory problems, but only among participants with a family history of AD. Findings suggest that higher neuroticism and lower extraversion may increase older adults’ reports of memory problems. Family history of AD may further exacerbate this tendency.

People with a first-degree family history of Alzheimer’s disease are at an increased risk of developing dementia. Subjective memory impairment among individuals with no measurable cognitive deficits may also indicate elevated dementia risk. It remains unclear whether nondemented people with a positive family history of Alzheimer’s disease are more likely to experience cognitive deficits and whether such an association reflects underlying neuropathology. We therefore investigated subjective memory impairment and hippocampal cortical thickness in 40 healthy older adults and 35 patients with amnestic mild cognitive impairment. We found greater subjective memory impairment and left hemispheric hippocampal cortical thinning associated with a first-degree family history of Alzheimer’s disease in healthy older adults. This suggests that subjective memory impairment could reflect preclinical stage neurodegeneration among individuals with the family history risk factor.

Organic psychiatric disorders result from brain dys­function caused by organic pathology inside or outside the brain. Dementia is the most common condition, with Alzheimer’s disease alone affecting 1 per cent of the population at 60 years, rising to 40 per cent over 80 years. Many of the rarer organic psychiatric dis­orders tend to affect a wider age range, but present in similar ways. Given the changing demographics of most developed countries, disorders producing cognitive im­pairment in older adults are becoming increasingly important for provision of healthcare services and in daily clinical practice. This chapter will cover the more common causes of cognitive impairment, and there is additional information in Chapters 18 and 20 on psych­iatry of older adults in psychiatry and medicine. There are three common clinical presentations of or­ganic psychiatric disorders: … 1 Delirium— an acute generalized impairment of brain function, in which the most important feature is impairment of consciousness. The disturbance of brain function is generalized, and the primary cause is often outside the brain; for example, sepsis due to a urinary tract infection. 2 Dementia— chronic generalized impairment, in which the main clinical feature is global intellectual impairment. There are also changes in mood and behaviour. The brain dysfunction is generalized, and the primary cause is within the brain; for example, a degenerative condition such as Alzheimer’s disease. 3 Specific syndromes— which include disorders with a predominant impairment of isolated areas; for example, memory (amnesic syndrome), thought, mood, or personality change. These include neurological disorders that frequently result in organic psychological complications; for example, epilepsy…. Table 26.1 lists the main categories of psychiatric disorder associated with organic brain disease. The following sections describe these syndromes and the psychiatric consequences of a number of neurological conditions. Organic causes of other core psychiatric conditions (e.g. anxiety and psychosis) are covered in the relevant specific chapters. Delirium is characterized by an acute impairment of consciousness producing a generalized cognitive impairment. The word delirium is derived from the Latin, ‘lira’, which means to wander from the furrow. Delirium is a common condition, affecting up to 30 per cent of patients in general medical or surgical wards, with the primary cause often being a sys­temic illness. The term ‘acute confusional state’ is a synonym for delirium.

Abstract:A potential relationship between Alzheimer's Disease (AD) and insulin gene expression was suggested by the observation that patients with AD have altered levels of fasting blood sugar and insulin. Since polymorphisms in the region 5' to the insulin gene have been associated with blood glucose levels, we have studied this polymorphism in AD patients. Subjects were 19 nondiabetic AD patients with symptoms of aphasia and apraxia and a family history of AD; and 20 age and sex-matched nondiabetic controls without family history of AD. The 5' polymorphic region of the insulin gene was analyzed by restriction enzyme digestion of DNA extracted from whole venous blood. We did not observe a correlation between the size of the 5' polymorphic region and AD.
