A 51 year old woman while treated on methotrexate (MTX) and leflunomide (LEF) for rheumatoid arthritis (RA), developed exertional breathlessness and undue tiredness followed by fever and cough, at her 11 years of suffering. Her respiratory symptoms developed around 12 weeks after leflunomide intake. She improved with steroid treatment. The Leflunomide (LEF) induced interstitial pneumonitis is an uncommon condition in absence of prior lung disease but potentially fatal. Septic arthritis of the sternoclavicular joint is an uncommon condition, and the diagnosis can be missed until a complication occurs. The sternoclavicular joint is more often involved in ankylosing spondylitis, degenerative arthritic conditions (i.e., rheumatoid arthritis and osteoarthritis), and primary and secondary metastatic conditions. The patient described in this case report came to the otolaryngology department on two occasions for treatment of a unilateral cutaneous neck abscess. The correct diagnosis was not made until the second visit. The author reviews the clinical course, diagnosis, and treatment of this uncommon disease.

1. Our experience with a six-year-old boy who had diarrhea, urethritis, conjunctivitis and arthritis is presented as a classical case of Reiter's syndrome. The patient recovered fully in a period of five months and has remained well during the past five years. 2. The cause of Reiter's syndrome remains unknown but in children may be related to Flexner bacillus dysentery. 3. The disease in children is usually less severe than in adults and all 11 children reported have recovered. The recent literature is reviewed. 4. Treatment with antibiotics is of no value and steroid therapy has not prevented exacerbations nor shortened the course of the disease. 5. The nongonococcal triad of urethritis, conjunctivitis, and arthritis is a distinct clinicopathologic entity which differs from known infections and juvenile rheumatoid arthritis.

Visceral Leishmaniasis is a very common but neglected disorder in Bangladesh. It can remain subclinical or become symptomatic with an acute, sub acute or chronic course. Atypical presentations can be equally challenging to the clinician. Visceral leishmaniasis is associated with various autoimmune phenomena. Interstitial lung disease is rare diseases in children. Respiratory symptoms appear progressively, are often subtle and diagnosis is often delayed by many months after onset. The histological diagnosis is confirmatory but high-resolution chest computed tomography is the most sensitive imaging technique for demonstrating and identifying interstitial lung disease. Interstitial lung disease in children mostly is idiopathic though it may be caused by several conditions. TAJ 2018; 31(2): 73-79
