Aim: This study was done to compare the effectiveness of drotaverine hydrochloride and valethamate bromide in shortening duration of labour. Methods: It is a hospital based prospective study, carried out in the department of Obstetrics & Gynaecology at Kathmandu Medical College Teaching Hospital (KMCTH), Sinamangal from 1st June to 31st August 2005. 99 patients, who were in active labour, were taken into the study. Out of which 50 were in drotaverine group (Group 1) who had received 40 mg of drotaverine hydrochloride intramuscularly at 3 to 5 cm dilatation of the cervix. Rest of 49 patients were in valethamate bromide group (Group 2). They received 3 doses of 8 mg valethamate bromide and 20 mg hyoscin butyl bromide intravenously half an hour interval. Result: The injection-delivery interval was studied in both Groups. The duration of injection-delivery interval was found to be almost equal in primigravidae of both Groups (p=0.72). But in multigravidae interval was significantly shorter in drotaverine Group (p=0.03). Conclusion: Both drotaverine and valethamate bromide are equally effective in shortening duration labour in primigravidae, where as drotaverine is more effective in multigravidae in accelerating labour. Background: To compare the efcacy of Drotaverine Hydrochloride and Valethamate Bromide in shortening of the rst stage of Labor. Drotaverine is more effective in regards of shorten the 1st stage of labor, rate of cervical dilatation with less side effects in compare to Valethamate Bromide. Methods:Two Hundred demographically similes woman with full term pregnancy in active labour were included in the study and divided into two groups viz. First Group: 100 women were given injection Drotaverine Hydrochloride (Drotin) 40 mg intramuscularly at 3-4 cm dilation at two hour interval. Second Group: 100 women were given injection Valethamate Bromide Intramuscularly at 3-4 cm dilation at one hour interval. Maximum three injections were given in both the groups. Comparative analysis was carried out as regards to duration of various stages of labor, rate of cervical dilatation, mode of delivery, side effects and feto-maternal outcome. Results: In Group-I Average duration of active phase of rst stage of labor was 149.78 minutes. In Primigravida it was 170.22 minutes and 129.35 minutes in Multigravida. In Group II Average duration of active phase of rst stage of labor was 294.62 minutes. In Primigravida it was 321.71 minutes and 267.54 minutes in Multigravida. Average rate of cervical dilatation in Group-I was 2.81cm/hour. In Primigravida it was 2.50 cm/hour while in Multi gravid it was 3.33cm/hour. In Group-II average rate was 1.42 cm/hour, in Primigravida it was 1.30 cm/hour and in Multigravida1.57 cm/hour. Conclusions: Drotaverine signicantly shorten the duration of rst stage of labor and found to be safe with no adverse effect on the mother and fetus.

BACKGROUND: Contractility of the myometrium is usually diminished during pregnancy to accommodate and protect the growing products of conception; drugs like Hyoscine butylbromide and Drotaverine are used to shorten the duration of labor in many hospitals all over the world. OBJECTIVE: To compare the effect of intravenous Drotaverine and Hyoscine Butylbromide rectal suppository on duration of labour and rate of cervical dilatation. METHODS: Three hundred pregnant women with term pregnancy in active labour were selected by simple randomization and were divided into three groups: Group A, where injection Drotaverine intravenous was given; Group B, where Hyoscine Butylbromide rectal suppository was instilled; and in Group C, where no drug was given. RESULTS: Mean duration of first stage of labour was 179.1 min, 159.3 min and 299.0 min in groups A, B and C, respectively. Rate of cervical dilatation in groups A, B and C was 2.6, 2.9 and 2.0 cm/hr, respectively. Most of the subjects delivered vaginally. No serious side effects were noted in both the drug groups. CONCLUSION: Hyoscine Butylbromide is more effective than Drotaverine in reducing the duration of active phase of labour. There is a significant improvement in the rate of cervical dilatation with Hyoscine Butylbromide when compared to Drotaverine. JMS 2012;15(1):39-43.

INTRODUCTION: Prolonged labour contributes to increased perinatal and maternal morbidity. Various drugs have been tried to hasten the cervical dilatation. The ultimate aim of the obstetrician is to accomplish the delivery in the shortest possible time without compromising maternal and fetal safety. This study aims to compare the efficacy of Hyoscine Butyl Bromide with Drotaverine Hydrochloride for augmentation of labor. MATERIAL AND METHODS: This was a prospective study conducted on 120 women with term gestation; in active labor. The patients were chosen by simple randomization and were divided into 3 groups- A, B and C respectively of 40 patients each. Group A received intramuscular injection drotaverine hydrochloride one ampule(40 mg), group B received intramuscular injection hyoscine butylbromide (20 mg) and group C which was control group, received no drug. RESULTS: The mean rate of cervical dilatation with buscopan was 2.23cm/hr while it was 2.03cm/hr and 2.08cm/hr in drotaverine group and control group respectively. Thus the drug delivery interval was less in buscopan group. Mean duration of active phase of first stage of labor was 156.13mins in buscopan group against 181.25mins in drotaverine group though buscopan was found to have less effect on duration of second stage of labor. CONCLUSION: Buscopan was found to be an effective and safe drug in shortening the duration of first stage of labor without any significant detrimental effects on the mother and newborn thereby minimizing maternal and fetal morbidity.

ABSTRACT Objective To evaluate and compare the effects of drotaverine and valethamate on cervical dilatation. Materials and methods Total of 100 patients (aged 20 to 30 years) including both primigravidae and multigravidae in first stage of labor, were divided randomly into two groups with 50 patients in each. The drotaverine (D) and valethamate (V) groups were given intravenously, 40 mg drotaverine hydrochloride to the former with every 2 hours for a maximum of 3 doses and 8 mg valethamate bromide to the latter with maximum of 6 doses half an hour apart. Results In primigravidae and multigravidae the average duration of active phase is shortened by 3 hours with 1.92 cm/ hour cervical dilatation in drotaverine group and 1 hour 45 minutes with 1.44 cm/hour in valethamate group (p-value < 0.05). There was no significant difference in the duration of second and third stages in both groups. No obstetrical complications or major side effects observed in both groups. Conclusion Drotaverine accelerates labor better than of valethamate. The reduction of pain during labor is better with drotaverine when compared with valethamate. Background: Labor is the most perilous journey a woman has to undertake. Painless and short labor is desired by every woman and is a constant aim for obstetrician. Objective of this study was to compare the efficacy of drotaverine hydrochloride with valethamate bromide for cervical dilatation in labor in primigravida.Methods: The present study was conducted in the department of obstetrics and gynecology, S. P. Medical College and Associated Group of Hospitals, Bikaner, Rajasthan from 1st August 2018 to 31st July 2019 over a period of 1 year. Total of 300 patients were included in the study with 150 patients in each of the two groups. In this study the effects of Drotaverine hydrochloride (Group A) and Valethamate bromide (Group B) on cervical dilatation were compared.Results: In our study, the mean drug delivery interval was 101.58±78.06 mins in Group A and 134.24±94.12 mins in Group B. The mean number of doses of drug required was 1.27±0.55 in Group A and 4.17±1.62 in Group B.  The difference was statistically highly significant (p <0.001).Conclusions: Drotaverine hydrochloride is found to be better than valethamate bromide in shortening the duration of active phase of labor in primigravidae. Drotaverine was found to be a better drug, hence, can be used to reduce the agony of the laboring woman without any significant side effects on the mother or the fetus.
