Management of ileocolic intussusception in the Czech Republic

Introduction: Ileocolic intussusception is one of the most frequent causes of acute abdomen in infants and toddlers. The purpose of this study was to evaluate the treatment of ileocolic intussusception in the Czech Republic in 2019. Methods: Fourteen departments of paediatric surgery participated in this multicentre retrospective study. A SurveyMonkey Inc. questionnaire was used for anonymous data collection of patients with ileocolic intussusception. Results: In 2019, ileocolic intussusception was diagnosed in 162 patients; median age was 30 months (3 months to 9.5 years). Contrast enema was indicated in 133 patients, primary surgery in 14 patients and spontaneous reduction was found in 15 patients. All departments using contrast enema had a success rate of 89.5%; 12 departments approached the contrast enema procedure under general anaesthesia (including myorelaxation at 7 departments) and 2 departments under sedation. Contrast enema under general anaesthesia with/without myorelaxation was significantly more successful than contrast enema under sedation (108/113, 95.6% vs 11/20, 55%; p<0.0001). In total, 28 patients were treated surgically, in 14 primary surgery was performed. Both surgical treatment and bowel resection were statistically significantly more frequent in departments where contrast enema was performed under sedation in comparison to departments performing contrast enema under general anaesthesia (9/20, 45% vs 19/142, 13.4%; p=0.0018 and 7/9, 77.8% vs 5/19, 26.3% p=0.0166). Conclusion: The success rate of contrast enema in the Czech Republic was 89.5%. The highest success rate is achieved by performing contrast enema under general anaesthesia.

AbstractThe paper explores the legislative unity of government and opposition blocs in the Czech Chamber of Deputies over a period of 20 years. As voting unity is usually rather low in the Czech Republic, temporarily high concentrations of votes by these blocs are linked to higher rates of conflict between the government and opposition. I use the Rice and UNITY indices to compare average unity scores of individual cabinets and also explorative time series of unity vectors in order to analyse bloc concentration, success rate, and increased conflict. The outcomes are relevant both as comprising a case study and methodological observations: (1) Broad differences in the logic of interaction are confirmed (e.g. caretaker cabinets show less conflict than standard cabinets). Although no universal trend (e.g. no transition from consensual to conflictual practice) is found, the Czech opposition became more concentrated and resorted to serial blocking tactics in the second decade; thus, a major change of behavior occurred after all. (2) The Rice and UNITY indices correlate considerably; UNITY’s discrimination capacity is not distorted significantly despite the nature of equilibria in the Chamber. Moreover; the UNITY index is able to easily distinguish contested votes not detectable by the Rice index alone.

Ultrasound-guided hydrostatic reduction of acute intussusception is a nonoperative technique, simple, performed under ultrasound guidance without risk of irradiation. Purpose: Our study aimed to evaluate the efficiency and utility of Ultrasound-guided hydrostatic reduction in the management of infant intussusception. Material and Methods: We conducted a retrospective study of cases of intussusception in infants (1-24 months) treated in the pediatric surgical emergency service of Rabat children’s Hospital during the period 2016-2020. Of the 135 cases collected during the study, 109 patients received ultrasound-guided hydrostatic reduction while 26 infants inderwent primary surgery. Results: This condition was most common in the age group (3-9 months) with 78% of cases. The sex ratio M/F was 2.29. The ileocolic type was the most encountered (56%). The complete ultrasound hydrostatic reduction was achieved for 88 infants with a success rate of 81% and an average hospitalization time of 1.4 days. 21 cases (19%) of which 16 failed hydrostatic reduction and 05 recurrences within 24 hours, were taken up by surgery with manual reduction and bowel resection for respectively 19 and 02 cases. Conclusion: Ultrasound guided hydrostatic reduction is a safe, quick and efficient method with a high success rate, a decrease in the occurrence of recurrences and complications and should be recommended first.

This study aimed to assess the numbers of juvenile European hedgehogs admitted to rescue centers in the Czech Republic from the viewpoint of their weight on admission, the reason for their admission, and the success rate of their release back into the wild. The results of our study show varying levels of success in the rearing of hedgehogs admitted at different ages (weights) and a varying period required for their rehabilitation. The greatest chance of release was seen in hedgehogs with a weight on the admission of 500–599 g (64.22% released) and 400–499 g (63.31% released). In contrast, the smallest number of young hedgehogs successfully rehabilitated and released was seen in hoglets weighing 200–299 g (35.24% released) on admission, which corresponds to the weight of hedgehogs at the time of weaning. Time spent at a rescue center may pose an undesirable threat to the lives of animals in some categories. Hedgehogs weighing up to 99 g on admission spent the longest period time at rescue centers (a median of 48 days), while hedgehogs weighing 500–599 g on admission spent the shortest time (a median of 7 days). The majority of hedgehogs in the lowest weight categories were admitted due to their inability to survive on their own. A large percentage of hedgehogs of greater weight, in contrast, were juvenile hedgehogs brought to rescue centers needlessly. The percentage of released animals did not exceed 65%, however, even for entirely independent categories of older juveniles. From this perspective, the fact that hedgehogs are often brought to rescue centers in the belief that they are not self-sufficient young, though they are actually juvenile or even adult individuals that do not require human care, can be considered a significant finding.
