PurposePrevious studies have revealed increased frontal brain activation during speech comprehension in background noise. Few, however, used tonal languages. The normal pattern of brain activation during a challenging speech-in-nose task using a tonal language remains unclear. The Mandarin Hearing-in-Noise Test (HINT) is a well-established test for assessing the ability to interpret speech in background noise. The current study used Mandarin HINT (MHINT) sentences and functional magnetic resonance imaging (fMRI) to assess brain activation with MHINT sentences.MethodsThirty native Mandarin-speaking subjects with normal peripheral hearing were recruited. Functional MRI was performed while subjects were presented with either HINT “clear” sentences with low-level background noise [signal-to-noise ratio (SNR) = +3 dB] or “noisy” sentences with high-level background noise (SNR = −5 dB). Subjects were instructed to answer with a button press whether a visually presented target word was included in the sentence. Brain activation between noisy and clear sentences was compared. Activation in each condition was also compared to a resting, no sentence presentation, condition.ResultsNoisy sentence comprehension showed increased activity in areas associated with tone processing and working memory, including the right superior and middle frontal gyri [Brodmann Areas (BAs) 46, 10]. Reduced activity with noisy sentences was seen in auditory, language, memory and somatosensory areas, including the bilateral superior and middle temporal gyri, left Heschl’s gyrus (BAs 21, 22), right temporal pole (BA 38), bilateral amygdala-hippocampus junction, and parahippocampal gyrus (BAs 28, 35), left inferior parietal lobule extending to left postcentral gyrus (BAs 2, 40), and left putamen.ConclusionIncreased frontal activation in the right hemisphere occurred when comprehending noisy spoken sentences in Mandarin. Compared to studies using non-tonal languages, this activation was strongly right-sided and involved subregions not previously reported. These findings may reflect additional effort in lexical tone perception in this tonal language. Additionally, this continuous fMRI protocol may offer a time-efficient way to assess group differences in brain activation with a challenging speech-in-noise task.

OBJETIVO: Estudar comparativamente a habilidade de reconhecimento de fala no silêncio e na presença de ruído competitivo em crianças usuárias de implante coclear utilizando dois diferentes processadores de fala. MÉTODOS: Foram avaliadas 26 crianças usuárias do implante coclear Nucleus 24M/24K, da Cochlear Corporation®, divididas em dois grupos de acordo com o processador de fala utilizado. O Grupo 1 foi composto por 16 crianças que faziam uso do processador de fala Sprint e o Grupo 2 foi composto por 10 crianças que faziam uso do processador de fala Freedom. Foi aplicado o Hearing in Noise Test - versão em Português/Brasil, em campo livre, na condição de silêncio e na presença de ruído competitivo. RESULTADOS: O desempenho do Grupo 2 (Freedom) foi superior ao desempenho do Grupo 1 (Sprint) em todas as condições de avaliação, sendo evidenciada diferença entre eles na condição de silêncio. CONCLUSÃO: O processador de fala apresentou influência significativa na percepção de fala de crianças usuárias dos implantes cocleares Nucleus 24K e 24M. As características de pré-processamento do som presentes no processador de fala Freedom podem ter contribuído para o melhor desempenho do Grupo 2 nos testes de percepção de fala realizados. Novos estudos são necessários para complementação destes achados.

Speech in noise tests that measure the perception of speech in presence of noise are now an important part of audiologic tests battery and hearing research as well. There are various tests available to estimate the perception of speech in presence of noise, for example, connected sentence test, hearing in noise test, words in noise, quick speech-in-noise test, bamford-kowal-bench speech-in-noise test, and listening in spatialized noise-sentences. All these tests are different in terms of target age, measure, procedure, speech material, noise, normative, etc. Because of the variety of tests available to estimate speech-in-noise abilities, audiologists often select tests based on their availability, ease to administer the test, time required in running the test, age of the patient, hearing status, type of hearing disorder and type of amplification device if using. A critical appraisal of these speech-in-noise tests is required for the evidence based selection and to be used in audiology clinics. In this article speech-in-noise tests were critically appraised for their conceptual model, measurement model, normatives, reliability, validity, responsiveness, item/instrument bias, respondent burden and administrative burden. Selection of a standard speech-in-noise test based on this critical appraisal will also allow an easy comparison of speech-in-noise ability of any hearing impaired individual or group across audiology clinics and research centers. This article also describes the survey which was done to grade the speech in noise tests on the various appraisal characteristics.

People are quicker to detect examples of real-world object categories in natural scenes than is predicted by classic attention theories. One explanation for this puzzle suggests that experience renders the visual system sensitive to midlevel features diagnosing target presence. These are detected without the need for spatial attention, much as occurs for targets defined by low-level features like color or orientation. The alternative is that naturalistic search relies on spatial attention but is highly efficient because global scene information can be used to quickly reject nontarget objects and locations. Here, we use ERPs to differentiate between these possibilities. Results show that hallmark evidence of ultrafast target detection in frontal brain activity is preceded by an index of spatially specific distractor suppression in visual cortex. Naturalistic search for heterogenous targets therefore appears to rely on spatial operations that act on neural object representations, as predicted by classic attention theory. People appear able to rapidly reject nontarget objects and locations, consistent with the idea that global scene information is used to constrain naturalistic search and increase search efficiency.

The present study investigated the effects of expansion time constants on the objective performance of 20 hearing instrument users fitted binaurally with digital in-the-ear products. Objective performance was evaluated in quiet using the Connected Speech Test and in noise using the Hearing in Noise Test. Results indicated that objective performance in quiet and in noise decreased as the expansion time constant increased. Furthermore, expansion time constants affected the objective performance of listeners with varying degrees of hearing loss in a similar manner.
