Infrared thermography has been proven to be an effective non-invasive method in diabetic foot ulcer prevention, yet current image processing algorithms are inaccurate and impractical for clinical work. The aim of this study was to investigate the accuracy of our automated algorithm for feet outline detection and localization of potential inflammation regions in thermal images. Optical and thermal images were captured by a Flir OnePro camera connected with an Apple iPad Air tablet. Both thermal and optical images were merged into an edge image and used for the estimation of foot template transformations during the localization process. According to the feet template transformations, temperature maps were calculated and compared with each other to detect a set of regions exceeding the defined temperature threshold. Finally, a set of potential inflammation regions were filtered according to the blobs features to obtain the final list of inflammation regions. In this study, 168 thermal images were analyzed. The developed algorithm yielded 95.83% accuracy for foot outline detection and 94.28% accuracy for detection of the inflammation regions. The presented automated algorithm with enhanced detection accuracy can be used for developing a mobile thermal imaging system. Further studies with patients who have diabetes and are at risk of foot ulceration are needed to test the significance of our developed algorithm.

Purpose: The number of patients who are suffering from diabetes nowadays is increasing significantly. In some countries, the percentage of population who suffer from this disease can reach up to 20%. Diabetic patients have to deal with their medical conditions and any further complications that this disease may cause. One of the most common conditions is the Diabetic Foot Ulcer (DFU). The early detection of these ulcers can help and may save the life of diabetic patients. Methods: This work proposes a mobile application for the detection of possible ulcers using a smart phone along with a mobile thermal camera (FLIR ONE). The proposed system captures thermal images of the feet from the thermal camera. The app that identifies ulcers was built using Android studio. The images were acquired to the Samsung S6 smart phone using the FLIR ONE SDK. Image processing techniques were deployed based on Open CV Library. The procedure of detecting possible ulcers was implemented based on analyzing the thermal distribution on the two feet. The developed application compares the difference between the temperature distribution on the two feet and checks if there is a Mean Temperature Difference (MTD) greater than 2.2oC (the value which indicates a possible ulcer development). Results: The system was tested under simulated conditions by heating different locations of the subjects’ feet to different temperature ranges; one image with temperature less than 2.2oC and another three images with temperature greater than 2.2oC. The system has successfully identified possible ulcer regions along with an image showing the location of the possible ulcers. Conclusions: This work is a very first step in developing a complete mobile thermal imaging system that can be validated clinically in the future.

AbstractChronic non-healing wounds represent an increasing problem. In order to enable physicians and nurses to make evidence based decisions on wound treatment, the professional societies call for supporting tools to be offered to physicians. Oxygen supply, bacteria colonization and other parameters influence the healing process. So far, these parameters cannot be monitored in an objective and routinely manner. Existing methods like the microbiological analysis of wound swabs, mean a great deal of effort and partly a long delay. In this paper 42 fluorescence images from 42 patients with diabetic foot ulcer, recorded with a hyperspectral imaging system (TIVITA®), converted for fluorescence imaging, were analysed. Beside the fluorescence images, information about the bacterial colonization is available from microbiological analysis of wound swabs. After preprocessing, principal component analysis, PCA, is used for data analysis with a 405 nm excitation wavelength, the emission wavelength range 510 - 745 nm is used for analysis. After dividing the data into a training and a test dataset it could be shown, that bacteria are detectable in the wound area. A quantification in bacterial colonization counts (BCC) was not in the focus of the research in this study stage.

Introduction:  Diabetes is one of the most prevalent metabolic chronic diseases due to the imbalance production of insulin. One of the studies reported that in 2010 worldwide 285 million adults had diabetes and this figure may be increase to 439 million by the year 2030. Globally Diabetic foot ulcers (DFUs) constitute major health problem in people that significantly contribute to morbidity and mortality in diabetes patients. Approximate 1.0% to 4.1% of the annual population-based incidences of a diabetic foot ulcer (DFU) were reported. Due to this the lifetime may be as high as 25%. In Asian countries diabetic foot ulcer are major problems which are different from European countries or developing countries.  From many studies reported diabetic foot problems in India are infectious and neuropathic in nature as compared to developed countries. According to World Health Organization (WHO) diabetic foot is defined as lower limb of a diabetic patient characterized by infection, potential risk of pathologic consequences ulceration or destruction of deep tissues associated with neurological abnormalities, various changes in peripheral neuropathy vasculopathy and superimposed infection that are mainly responsible foot ulceration. Ulcers are one kind of abscess which is difficult to treat because of poor wound healing that result from a combination of neuropathy, ischemia and hyperglycemia.  Aim: The main objective was to study the outcome of treatment modalities and it’s relating factors to complication in diabetic foot ulcer.  Material and method:  Total 60 diabetic foot ulcer patients with the age range from 20 to 70 years were included.  From all the patients’ detailed past and present history were recorded.  For all the patients, general, physical and local and systemic examinations were also done. Detail   laboratory examination like Fasting and Post Prandial Blood sugar levels, blood count, ECG, ESR, complete urine examination for the presence of ketone bodies and sugar, x-ray as well as culture and sensitivity of the discharge from ulcer were also done. Patients were treated with various treatment methods like conservative treatment, split skin grafting and amputation. Result: In this study male patients were more in proportion as compared to female. This study showed that maximum with the age group 14 -50 (43.3%) years old followed by 18.3% in 31-40 years old, 16.7% in 61-70 years old.  6.7% showed the least age group as 20 -30 years old.  Out of total 60 patients, 38.3% of the patients showed diabetic ulcer foot which was more whereas 15% showed diabetic gangrene foot which was least. 25% showed diabetic cellulites foot and 21.7% showed as diabetic abscess foot.  Conclusion: Globally as diabetes mellitus cases are increasing and it became rapidly the public health problem. This may be due to burden on economy, health system and on society to manage the diabetic foot problems. Diabetic foot management guidelines must be made into our practice protocols which may preventing limb loss, and decrease mortality and increase the quality of life of the patient. Hence for this it is only possible with the help of foot care education and health care workers.  Hence, foot infection is to put first and care for it like hands.
