Background: The current study investigated the moderating role of the school context on the effects of a Dutch health promoting school initiative on children’s health and health behaviors. Methods: The study used a mixed-methods design. The school context (n = 4) was assessed by the characteristics of the school population, teacher’s health-promoting (HP) practices, implementers’ perceived barriers, school’s HP elements, and dominating organizational issues. Outcomes included objectively assessed BMI z-scores and physical activity (PA), and parent and child-reported dietary intake. Analyses included linear mixed models (four intervention schools versus four control schools), and qualitative comparisons between intervention schools with similar HP changes. Results: Effects on outcomes varied considerably across schools (e.g., range in effect size on light PA of 0.01–0.26). Potentially moderating contextual aspects were the child’s socioeconomic background and baseline health behaviors; practices and perceived barriers of employees; and organizational issues at a school level. Conclusions: Similar HP changes lead to different outcomes across schools due to differences in the school context. The adoption of a complex adaptive systems perspective contributes to a better understanding of the variation in effects and it can provide insight on which contextual aspects to focus on or intervene in to optimize the effects of HP initiatives.

Background. Upper limb (UL) performance, or use, in daily life is complex and likely influenced by many factors. While the recovery trajectory of UL impairment poststroke is well documented, little is known about the recovery trajectory of sensor-measured UL performance in daily life early after stroke and the potential moderating role of psychosocial factors. Objective. To examine the recovery trajectory of UL performance within the first 12 weeks poststroke and characterize the potential moderating role of belief, confidence, and motivation on UL performance. Methods. This was a longitudinal, prospective cohort study quantifying UL performance and related psychosocial factors early after stroke. UL performance was quantified via bilateral, wrist-worn accelerometers over 5 assessment sessions for 24 hours. Belief, confidence, and motivation to use the paretic UL, and self-perceived barriers to UL recovery were quantified via survey. Change in 4 accelerometer variables and the moderating role of psychosocial factors was tested using hierarchical linear modeling. The relationship between self-perceived barriers and UL performance was tested via Spearman rank-order correlation analysis. Results. UL performance improved over the first 12 weeks after stroke. Belief, confidence, and motivation did not moderate UL performance over time. There was a negative relationship between UL performance and self-perceived barriers to UL recovery at week 2, which declined over time. Conclusions. Sensor-measured UL performance can improve early after stroke. Early after stroke, rehabilitation interventions may not need to directly target belief, confidence, and motivation but may instead focus on reducing self-perceived barriers to UL recovery.

Objective We examined health-related communication between same-sex and other-sex friends and how communication was related to health-related behavior. Participants Data from 243 emerging adults attending college ( Mage = 18.96, SD = 1.43; 55.6% male) were analyzed. Methods Participants completed measures assessing the frequency in which they talked about and made plans to engage in exercise and nutrition-related behaviors with friends, as well as how often they engaged in exercise and nutrition-related behaviors. Results In general, participants reported more health-related communication with same-sex friends. Health-related communication with same-sex friends was positively related to health behaviors for men and women. However, the pattern of results differed for men and women depending on the topic of communication and the behavior being examined. Conclusion Our study extends the literature by examining the role of sex of friends in health communication and planning and how interactions with friends relate to health-promoting behavior.

Mediating role of coping styles in the relationship between anxiety and health behaviors of obese adolescents Obesity is one of the major health problems in adolescents. Health-detrimental lifestyle (i.e. lack of physical activity, inappropriate nutrition) as well as maladaptive styles of coping with stress are regarded as belonging among determinants of obesity. The aim of the study was to establish factors mediating between anxiety and diet-related health behaviors. Participants in the study were 113 adolescents with obesity whose body weight was over 97th centile. They were examined using a set of self-report questionnaires to measure anxiety, coping styles and health behaviors. Emotion-focused coping and seeking social contacts (social diversion) were found to act as mediators between adolescents' trait anxiety and their health behaviors. The findings suggest that to enhance obese adolescents' health-promoting behaviors appropriate conditions should be ensured that would not only enable them to express their emotions, but also promote their socializing with peers.

Drawing upon ideas stemming from social disorganisation theory, this study explores how structural and social aspects of the school context affect youth substance-use behaviours in terms of smoking, alcohol and/or drug use. A key focus is to investigate the joint effect of school collective efficacy and schools’ substance-use norms on students’ substance use. Analyses are based on combined information from two independent data collections conducted in 2014 among ninth grade students ( n = 5122) and teachers ( n = 1105) in 81 senior-level schools in Stockholm. Results from multilevel analyses confirm previous research by suggesting that the proneness to engage in substance use varies depending on the socioeconomic profile of the school. Youth in socioeconomically advantaged schools were more prone to engage in substance use than youth in disadvantaged school settings. Furthermore, collective incentives for exerting social control against substance use seem to be weaker in schools where conventional values towards substance use (anti-substance-use norms) are suppressed.

We analyzed data from the National School-based Health Survey (PeNSE) carried out in Brazil in 2015 (n = 102,072 adolescents) to estimate how much of the individual variance in the prevalence of health behaviors is attributable to the school level. Multilevel logistic regression models were calculated to estimate the variance partitional coefficient (VPC) of the use of drugs, intake of unhealthy food, leisure physical activity and weight-related behaviors. The between-schools variance was significant in all tested models. The highest VPCs were observed when the use of drugs was analyzed (15%-20% of the total variance of smoking and use of illegal drugs). Lower, but still significant, values were observed in the other outcomes. The school context plays an important role in the adolescents’ health and should be considered in the design of public policies and actions in public health.
