Acetabular cup liner and prosthetic head exchange to increase the head diameter for management of recurrent luxation of a prosthetic hip in two dogs

SummaryComponent malalignment and impingement are possible causes of recurrent luxation following total hip replacement in the dog. In the two cases presented in this report, luxation that was probably due to impingement was managed by exchanging the standard 17 mm prosthetic head for a 24 mm prosthetic head. This required removal of the original acetabular cup liner and placement of a new polyethylene liner that would accept the 24 mm head into the stable acetabular shell. In the first case, a 50 kg Malamute dog, recurrent luxation was initially managed by component alignment revision, iliofemoral suture, triple pelvic osteotomy and a novel lasso technique, without long-term success. After exchanging the head and cup liner, luxation did not recur over a 12-month period. In the second case, a 65 kg Newfoundland dog, impingement was suspected after a second luxation event. Luxation did not recur during the nine months after exchange of the head and cup liner. The larger prosthetic head used in these two cases increased the impingement-free range-of-motion of the joint and increased the translation distance required for luxation (jump distance).

Abstract Objective The aim of this study was to report the successful revision of a loose perforated press-fit cup by exchange with a porous-coated press-fit cup while maintaining the original locking stem and head–neck unit. Methods Cup revision was performed in a dog with a loose acetabular cup that had undergone total hip replacement with a perforated press-fit cup and locking stem 29 months previously. The original locking stem was well integrated. A single session cup exchange was performed by implantation of a 28-mm porous-coated press-fit cup with 16 mm internal diameter to allow the original locking stem and head–neck unit to be preserved. Results Revision total hip arthroplasty by cup exchange resulted in excellent functional outcome with long-term follow-up 18 months postoperatively. There were no intraoperative or postoperative complications. Conclusion A loose perforated press-fit cup was successfully revised with a porous press-fit cup while allowing preservation of the initial locking stem and head–neck unit.

Ceramics have been increasingly used in orthopaedics during the last 30 years. Their biological inertness, high hardness and good mechanical strength make them excellent candidates for components such as femoral heads and acetabular cup in Total Hip Replacement prostheses. Currently used bio-inert ceramics – alumina and zirconia – give good clinical results, especially compared to metal – polymer couplings. However, they are subjected to severe biological, tribological and mechanical solicitations during more than fifteen years for the most successful prostheses. They answer these solicitations by presenting specific degradation mechanisms. We will thus examine the phenomena that can account for the long-term behaviour of zirconia components (heads and cups) in THR prostheses.

Fifteen patients (16 hips) with an ankylosed hip joint for a mean of 36 years (3.5 to 65), had their hips converted to a cementless Total Hip Replacement (THR). They were followed-up for a median of 10.75 years (5.0 to 19) with none lost to follow-up. All patients showed improved mobility and function post-operatively. The Harris Hip Score improved from 70 (SEM 3.4) to 83 post-operatively (SEM 4.4), which was statistically significant (p < 0.05). There was one acetabular cup revision at 5 years post implantation for aseptic loosening. All other femoral and acetabular components remained clinically and radiographically well fixed. We conclude that a previously ankylosed hip can be effectively converted to a cementless total hip replacement with good long term results.

Aseptic loosening of the acetabular component is the principal long-term complication in total hip replacement. Radiographic evidence suggests a higher incidence of loosening with medialization of the axis of rotation of the acetabular cup. A simplified two-dimensional photoelastic investigation of the load transfer in the pelvis demonstrates that the stress distribution is substantially changed when this axis of rotation is medialized. Strain gauge measurements on three dry hemi-pelvic bones confirm this effect. It is postulated that the redistribution of load resulting from medialization may result in inadequate skeletal support for the acetabular component predisposing to loosening.

SummaryA retrospective case series study was done to determine the long-term outcome of operations upon dogs treated for canine hip dysplasia by means of a triple pelvic osteotomy (TPO). Twentyfour dogs with bilateral hip dysplasia, that received a unilateral TPO between January 1988 and June 1995, were re-examined at the Ontario Veterinary College. The assessment included physical, orthopedic and lameness examinations, standard blood work, pelvic radiographs and force plate gait analysis. They were compared to bilaterally dysplastic dogs that had not been treated, and also to normal dogs. Force plate data analysis demonstrated a significant increase in peak vertical force (PVF) and mean vertical force over stance (MVF) in the limb that underwent surgical correction by means of a TPO, when compared to the unoperated hip. It was determined that performing a unilateral TPO on a young dysplastic dog resulted in greater forces and weight bearing being projected through the TPO corrected limb when compared to the unoperated limb.Dogs with bilateral hip dysplasia treated with a unilateral triple pelvic osteotomy (TPO) were assessed by force plate gait analysis, radiographs and orthopedic examination. There was a significant increase in hip Norberg angles over time, although degenerative changes did progress. Limbs that had been operated upon had significantly greater peak and mean ground reaction forces than limbs that had not received an operation.

Thailand has been affected by COVID-19, like other countries in the Asian region at an early stage, and the first case was reported as early as mid-January 2020. Thailand’s response to the COVID-19 pandemic has been guided by the “Integrated Plan for Multilateral Cooperation for Safety and Mitigation of COVID-19”. This paper analyses the health resources in the country and focuses on the response through community-level public health system and legislative measures. The paper draws some lessons on future preparedness, especially with respect to the four priorities of Sendai Framework for Disaster Risk Reduction. At the end, the paper puts some key learning for future preparedness. While Thailand’s response to COVID-19 has been effective in limiting the spread of the disease, it falls short at being able to address the multiple dimensions of the crisis such as the economic and social impacts. The socioeconomic sectors have been hardest hit, with significant impact on tourism sectors. Sociopolitical system also plays an important role in governance and decision-making for pandemic responses. The analysis suggests that one opportunity for enhancing resilience in Thailand is to strive for more multilevel governance that engages with various stakeholders and to support grassroots and community-level networks. The COVID-19 pandemic recovery is a chance to recover better while leaving no one behind. An inclusive long-term recovery plan for the various impacted countries needs to take a holistic approach to address existing gaps and work towards a sustainable society. Furthering the Health Emergency Disaster Risk Management (HEDRM) Framework may support a coordinated response across various linked sectors rather than straining one particular sector.

Introduction: The Chiari pelvic osteotomy (CPO) has been recommended as a salvage procedure to improve head coverage in case of hip joint incongruence in paediatric hip disease. In this study, we aimed to assess the long-term results of CPO for severe Legg-Calvé-Perthes disease (LCPD). Methods: A total of 39 patients who underwent a CPO at our department between 1995 and 2010 were prospectively followed both radiologically (Stulberg classification) and clinically (Harris Hip Score [HHS], conversion into total hip arthroplasty). In this study, we retrospectively reviewed the cases of 12 hips (12 patients, 3 girls, 8 left hips) treated by CPO for severe LCPD (Catterall grade 3 or 4) with hip joint incongruence. Mean follow-up was 14.0 (range 7.6–21.3) years. Results: Mean age at surgery was 10.2 (range 8.2–17.8) years. Additional femoral osteotomy was performed in 8 patients. A good radiological result (Stulberg I or II) was achieved in 2 patients, a fair result (Stulberg III) in 4 patients, and a poor outcome (Stulberg IV or V) in 6 patients. Mean postoperative HHS averaged 93 (range 65–100) points. An excellent functional outcome (HHS 90–100 points) was achieved in 9 patients. No patient underwent total hip arthroplasty during follow-up. Postoperative limb-length discrepancy was found in 3 patients. Conclusions: CPO for severe LCPD with hip joint incongruence resulted in good long-term clinical outcome in about ⅔ of our patients after a mean of 14 years. Our results suggest that CPO can still be considered as a salvage joint-conserving procedure in this selected group of younger patients.

Hypocalcemia is a rare, but reversible, cause of dilated cardiomyopathy. Although cardiomyopathy may cause severe heart failure, calcium supplementation can reverse heart failure. We report here a patient with uremia and secondary hyperparathyroidism, who was complicated by persistent hypocalcemia and refractory heart failure. The cardiac failure was refractory to treatment with digitalis and diuretics, but dramatically responded to calcium therapy and restoration of normocalcemia. As a result, the patient was eventually diagnosed with hypocalcemic cardiomyopathy. To the best of our knowledge, this is the first case of this disease to be reported in a patient with uremia. Findings from our case may help clinicians to better understand hypocalcemic cardiomyopathy. Our case might also provide new insight into long-term cardiac complications and prognoses of patients undergoing parathyroidectomy due to secondary hyperparathyroidism.
