The present report aimed to explore the case of an 8-year-old patient with chief complaint of the lack of eruption of the maxillary right permanent central incisor, referring to the Department of Pediatric Dentistry, Faculty of Dentistry, Hamadan University of Medical Sciences. The corresponding tooth on the contralateral side had fully erupted. The patient’s history revealed that the predecessor deciduous tooth had sustained a trauma, resulting in the partial intrusion of the tooth into the alveolar bone, that is, the relative intrusion of the deciduous central incisor. CBCT examinations were ordered for further evaluation, which showed the upward displacement of the permanent tooth bud in the alveolar bone as a result of the trauma, adhering to the floor of the nasal cavity. Therefore, root formation was halted, making the tooth embedded.

ABSTRACT Objectives: To describe qualitatively and quantitatively the directions and magnitudes of rotations of permanent maxillary central incisors and first molars in the mixed dentition in repaired complete unilateral cleft lip and palate (UCLP) and study their associations with absence of teeth in their vicinity. Materials and Methods: Dental casts and orthodontic records taken prior to orthodontic preparation for alveolar bone grafting of 74 children with repaired UCLP (53 male, 21 female; aged 8.9 ± 1.0 years) were studied. Directions and magnitudes of permanent maxillary central incisor and first molar rotations were recorded. Tooth absence was confirmed from longitudinal radiographic records. Incisor and molar rotations were analyzed in relation to the absence of teeth in their vicinity. Results: Distolabial rotation of the permanent maxillary central incisor was noted in 77.14% on the cleft side, while distopalatal rotation was noted in 82.19% on the noncleft side. Incisor rotation was greater when a permanent tooth was present distal to the cleft side central incisor, in the greater segment. The permanent maxillary first molar showed mesiopalatal rotation, which was greater on the cleft side and when there was absence of one or more teeth in the buccal segment. Conclusions: Presence and absence of teeth were associated with the severity of incisor and molar rotations in UCLP. Crowding of anterior teeth in the greater segment was associated with a greater magnitude of rotation of the cleft side permanent central incisor. Absence of one or more buccal segment teeth was associated with greater magnitude of rotation of the molar.

Introduction.Tooth avulsion in the young permanent dentition is a frequent finding, and its prognosis depends on the treatment of the avulsed tooth before replantation, the extra-alveolar time, the storage medium, and the patient’s general health. The present report describes management of an immature avulsed lower central incisor 90 minutes after the accident.Methods.A right lower central incisor of a 7-year-old girl was avulsed, and it was soaked in a glass of milk. 90 minutes after avulsion, replantation was performed, and the tooth was splinted; but after two weeks the replanted tooth’s pulp was necrotic. Thus, endodontic treatment was performed and root canal was filled using a calcium hydroxide and iodoform paste (Metapex). Three months later, the intracanal medication was washed out and the canal was sealed using an apical plug of calcium enriched mixture (CEM) cement.Results.20 months after replantation the tooth was completely asymptomatic, with physiologic mobility. Also, continued root formation including an apical segment beyond the artificial apical plug was observed.Conclusion.Creation of an appropriate apical barrier following the disinfection of root canal system promoted continued root-end growth in a replanted immature permanent tooth.

The present report follows the case of a young boy with solitary median maxillary central incisor (SMMCI) syndrome between the ages of 4 and 7 years. This condition is characterized by the presence of one single maxillary central incisor in the midline instead of two central incisors. No other developmental abnormalities involving growth or brain function were noted at, or subsequent, to birth. This report includes a discussion of the aetiology of SMMCI syndrome and its association with birth defects such as holoprosencephaly (HPE), CHARGE and VACTERL, as well as a discussion of the long-term prognosis and associated dental and medical issues for this particular patient

ABSTRACT Tooth rotation is a dental anomaly of position, in which there is a displacement of the tooth within the alveolar bone around its longitudinal axis. Although it is a common finding in the premolar—molar region, rotations of the maxillary centrals are extremely rare and such aberrations are multifactorial in their origin. It is important to have a thorough understanding of such anatomical variation, which can cause occlusal and esthetic problems in patients, to alert the dental surgeons, so that they are well prepared to carry out esthetic and functional rehabilitation of the teeth involved. An odontoma is usually found in the surrounding area of the unerupted permanent tooth bud and rarely adjacent to the primary central incisor. This report presents a case of an unerupted malformed primary central incisor, histologically diagnosed as a compound odontoma. A 5.5 year-old healthy boy presented with an unerupted maxillary left primary central incisor and mild gingival buccal swelling. No history of traumatic injury was recorded. Radiological examination revealed a tooth-like mass with a partially developed root and a malformed crown in the area of the left primary central incisor. Delayed development of the successor left permanent incisor in comparison to the right one was noted. The rational for early surgical intervention to enable normal development and eruption of permanent incisors is described. Clinical and radiographic diagnoses as well as several etiological factors and a differential diagnosis are considered and reviewed.

Introduction: The maxillary anterior region is becoming a major concern due to its aesthetic relevance. The buccal bone thickness is important for implant placement, orthodontic treatment and restorative treatment. Objective: To assess the thickness of alveolar bone in the maxillary central incisor using cone beam computed tomography (CBCT). Methods: A cross-sectional observational study was conducted at Department of Dental Surgery, Bir Hospital where CBCT of 53 samples from July 2019 till December 2019, the archived CBCT images was assessed retrospectively. The thickness of the labial bone in a direction perpendicular to the outer surface of the tooth root was measured at a distance of 2 mm from the cementoenamel junction (CEJ). The measurement was taken thrice and the mean measurement was considered. Results: The labial alveolar bone thickness in maxillary central incisor was found to be 0.55±0.27 mm at a distance of 2 mm from the CEJ. Only 2 (3.8%) of the samples had an alveolar thickness of >1 mm. No statistically significant difference was found with respect to gender and age. Conclusion: The average thickness of the labial alveolar bone in maxillary central incisor using cone beam computed tomography was found to be thin.

When dental implants are malpositioned in relation to the adjacent teeth and alveolar bone or in an excessive buccal or lingual position, the final prosthesis rehabilitation impairs the peri-implant health of the gingival tissues and the aesthetics of the patient. Thus, the purpose of this case was to report and discuss a multidisciplinary protocol for the treatment of a compromised maxillary tooth in a patient with an abscess in his right central incisor due to an excessive buccal implant position. The patient presented with an implant-supported provisional restoration on his right maxillary central incisor and a traumatic injury in his left central incisor. The treatment protocol consisted in (i) abutment substitution to compensate the incorrect angulation of the implant, (ii) clinical crown lengthening, (iii) atraumatic extraction of the left central incisor, and (iv) immediate implant placement. Finally, (v) a custom abutment was fabricated to obtain a harmonious gingival contour around the prosthetic crown. In conclusion, when implants are incorrectly positioned in relation to the adjacent teeth, associated with soft-tissue defects, the challenge to create a harmonious mucogingival contours may be achieved with an interdisciplinary approach and with the placement of an appropriate custom abutment.
