The objective was to determine whether trauma in primary dentition causes alterations in the development of permanent dentition. Searches were made in May 2020 using PubMed, MEDLINE, MEDES, Scopus, Lilacs, and Embase. Papers in English, German, and Spanish, without restrictions in the year of publication, were included. The quality of the studies was analyzed using the NOS Scale. The search retrieved 537 references, and seven studies were included for a qualitative analysis. The results showed that trauma to a deciduous tooth can damage the bud of the permanent tooth. Enamel discoloration and/or hypoplasia were the most common sequelae in the permanent teeth after trauma to the primary predecessor. The type and severity of sequelae in the permanent tooth are associated with the development phase of the bud. Children with trauma of their primary teeth should receive checkups until the eruption of the permanent teeth for the early diagnosis and treatment of possible sequelae. Intrusion of the primary tooth was the trauma that caused the most damage and enamel alterations the most frequent sequelae.

Introduction Global patterning and timing of permanent tooth emergence is influenced by ethnicity, with no known timings reported for ethnic Melanesian children living in the tropical archipelago of Vanuatu. Aim To determine timings of permanent tooth emergence and sequencing for children who reside in rural Vanuatu. Methods Children aged 4-17 years (n=1026), part of a larger oral health cross-sectional study, were examined recording all permanent teeth present, across four spatially separated islands. Binary logistic modelling established children’s median age of emergence of each permanent tooth for each study area. Results The median emergence of first permanent molars for girls is 4.9-years and 5.3 -years for boys. In all locations, children had all permanent teeth emerge by age 11 years (excluding 3rd molars). Clinically important differences exist for permanent tooth emergence by study area. Discussion Permanent teeth emerge earlier for Ni-Vanuatu children compared to both Melanesian children of Papua New Guinea as well as other ethnicities across Oceanic countries. These results can be used as a set standard for Vanuatu. Early tooth emergence suggests oral health education programmes should target pregnant women with clinical preventive strategies commencing for their children before 5-years of age.

The present report aimed to explore the case of an 8-year-old patient with chief complaint of the lack of eruption of the maxillary right permanent central incisor, referring to the Department of Pediatric Dentistry, Faculty of Dentistry, Hamadan University of Medical Sciences. The corresponding tooth on the contralateral side had fully erupted. The patient’s history revealed that the predecessor deciduous tooth had sustained a trauma, resulting in the partial intrusion of the tooth into the alveolar bone, that is, the relative intrusion of the deciduous central incisor. CBCT examinations were ordered for further evaluation, which showed the upward displacement of the permanent tooth bud in the alveolar bone as a result of the trauma, adhering to the floor of the nasal cavity. Therefore, root formation was halted, making the tooth embedded.

Background: Melanotic neuroectodermal tumor of infancy (MNTI) is a very rare tumor, and case reports of orthodontic treatment for patients with MNTI may be previously unreported. This article describes the orthodontic treatment for a 7-year-old girl with MNTI in the mandible. Case: Her chief complaint was anterior crossbite. Although she had an MNTI diagnosis at the age of 8 months, it remained subclinical. Therefore, she has been regularly followed-up by computed tomography (CT) and magnetic resonance (MR) imaging without aggressive treatment. We had worried about the stimulation of MNTI on the mandible by changing her occlusal position with orthodontic treatment. Therefore, we sufficiently explained to her and her family that orthodontic treatment was at risk for worsening MNTI. However, they desired treatment that consisted of maxillary protraction and slow expansion to correct anterior crossbite and encourage the permanent tooth eruption. After 19 months of active orthodontic treatment, the anterior crossbite was improved, and the eruption of permanent teeth made good progress. No evidence of progression and exacerbation of MNTI has been found by both CT and MR imaging. As the observation period is still short, we need a careful and long-term follow-up of her occlusion and MNTI. Furthermore, when we encounter rare cases without previous experience and reports, informed consent was of particular importance.

Congenital missing teeth (OMIM #106600) is the most common dental abnormality. The aim of the study was to evaluate the effects of tooth agenesis on the total mandibular length, length of the mandibular body and alveolar process, and the mandibular anteroposterior position. The material was obtained from the Department of Orthodontics, Medical University of Warsaw. The study group included 116 patients aged 9–18 years with a congenital absence of at least two permanent tooth buds in the maxilla and/or mandible (mean: 6.2 teeth missing/patient). All patients were Caucasians: 68 (59%) females and 48 (41%) males. The control group included 115 patients without tooth agenesis matched with the age and gender of the study group. A cephalometric analysis was performed, and it was focused on assessing anteroposterior mandibular measurements. This assessment was based on 17 measurements (12 linear and 5 angular). Statistical analysis of the cephalometric measurements between the study group and the control group showed significant changes regarding selected mandibular measurements. Tooth agenesis does not affect the total length of the mandible and the length of the mandibular body, but it might reduce the length of the mandibular arch length and result in a more retrusive mandibular position.

Background The permanent tooth formation process may be disrupted in preterm infants with potential discrepancies in size and subsequent occlusal disturbances. Objective To systematically analyse and quantitively synthesize the available evidence regarding the impact of preterm birth on permanent tooth crown dimensions. Search methods Unrestricted searches in 6 databases and manual searching of the reference lists in relevant studies were performed up to March 2021 (Medline via PubMed, CENTRAL, Cochrane Database of Systematic Reviews, Scopus, Web of Science, ProQuest Dissertations and Theses Global). Selection criteria Observational studies investigating permanent tooth crown dimensions in preterm and control full-term born individuals. Data collection and analysis Following study retrieval and selection, relevant data were extracted, and the Newcastle-Ottawa scale was used to assess the selection, comparability, and outcome domains. Exploratory synthesis and meta-regression were carried out using the random effects model. Results Three studies were located from the initially retrieved records and the assessments with the Newcastle-Ottawa scale identified issues regarding the selection and comparability domains. Overall, the mesiodistal and the buccolingual dimensions of the permanent teeth in both dental arches tended to be smaller in children born prematurely than full term children. Subgroup analyses showed statistically significant differences for the extremely preterm to control group comparisons for the incisors and the first molars. Meta-regression showed a modificatory effect of gestational age and racial background but not of birth weight and gender on tooth size. The quality of available evidence was rated at best as moderate. Conclusions Premature birth could potentially be associated with reduced tooth-crown dimensions in some permanent teeth especially in children born extremely preterm. Although the results from these observational studies should be approached with caution until more information becomes available, the possible clinical implications in terms of diagnosis and treatment planning should be considered. Purpose: Leeway space is clinically crucial in pediatric dentistry because it is utilized to resolve tooth crowding and allow the first molars to drift mesially to establish a Class I molar relationship in the later stages of mixed dentition. This study investigated leeway space in the mixed dentition of Taiwanese children of different sexes and ages. Materials and Methods: The digital panoramic dental films of 182 lower arches of 119 boys and 63 girls aged 5–10 years were analyzed in this retrospective study. The mesiodistal crown widths of the primary canines and first and second molars and the permanent canines and first and second premolars were measured using medical imaging software. Differences in leeway space were statistically analyzed. Results: The average leeway space was 1.29 ± 1.48 mm on each side of the lower arch. The leeway space of children aged 5–6 years was significantly greater than that of children aged 7–8 years. No gender difference in crown width was discovered, except with regard to the primary first molar. Although no gender difference in leeway space was observed, permanent teeth affected leeway space more for girls than for boys. Conclusion: In Taiwanese children, although leeway space does not differ by sex, age affects leeway space. However, permanent tooth size has an influence on the leeway space of girls.

Introduction: Bioceramic materials, gray and white mineral trioxide aggregate (GMTA, WMTA), have been shown to have high rates of success in various endodontic applications. A major drawback is their tendency to discolor teeth compared to Biodentine (BD), that has been claimed not to discolor teeth. The aim of this study was to compare tooth discoloration after applying different pulpotomy base materials (BD, GMTA and WMTA). Study design: Forty human incisors teeth were used in this study. Coronal access was achieved by a Tungsten Carbide drill, and the pulp chambers were accessed and chemo-mechanically debrided. Each material was placed in the pulp chamber, up to the cervical sectioning level. All specimens were incubated at 37°C and 100% humidity for three months and have been evaluated before the study and weekly. Color was assessed according to the CIE L*a*b* color space system. Results: ΔE of all experimental groups (GMTA, WMTA and BD) were significantly different from the control group at all time points (P<0.05). Color changes in the GMTA and WMTA groups, had no statistically significant differences, but showed higher discoloration compared to BD group in the cervical part of the crown, since week 1 (P<0.05). WMTA group showed significant discoloration in the cervical part as of week 1 (P<0.05), and gradually increased over time (Figure 2). BD group showed no significantly discoloration over time. GMTA group showed the significant discoloration at week 1 and week 14 (P<0.05). Conclusions: both GMTA and WMTA pulpotomy materials may discolor tooth structure over time in an extracted permanent anterior tooth model. When choosing bioceramic pulpotomy material, BD may be preferable in esthetic area.

Abstract The purpose of the study was to investigate the association between single nucleotide polymorphisms (SNPs) in genes encoding estrogen receptors (ESR1 and ESR2, respectively) and delayed tooth emergence (DTE). This cross-sectional study was composed of biological unrelated children of both sexes, age ranging from 11 to 13 years old. DTE was defined when the successor primary tooth was still present in the oral cavity after its exfoliation time or the absence of the permanent tooth emergence into the oral cavity. Children were diagnosed with DTE when they had at least one delayed permanent tooth, according to age of exfoliation of each tooth proposed by The American Dental Association. Genomic DNA from saliva was used to evaluate the SNPs in ESR1 (rs9340799 and rs2234693) and ESR2 (rs1256049 and rs4986938) using Real-Time PCR. Chi-square or Fisher exact tests and Logistic Regression adjusted by age and gender were performed. SNP-SNP interaction was accessed by multifactor dimensionality reduction (MDR) analysis also adjusted by gender and age. The established alpha of this study was 5%. Among 537 included children, 296 (55%) were in the “DTE” group and the 241 (45%) were in the “Control” group. Age and gender were not statistically different among the groups (p>0.05). Genotype distribution of the SNPs rs9340799, rs2234693, rs1256049 and rs4986938 were not associated with DTE (p> 0.05). The models elected by MDR were not statistically significant either. Conclusions: The studied SNPs in ESR1 and ESR2 were not associated with permanent DTE.

Background: Bone grafts can provide an optimal environment for permanent tooth to erupt and enhance the stability of the alveolar maxilla. Although autologous bone is an optimal source for osteogenesis, its inevitable donor site morbidity has led to active research on bone substitutes. This study was designed to evaluate the safety and feasibility of using biphasic calcium phosphate (BCP; Osteon) as a bone substitute in dogs. Methods: Bilateral third and fourth premolars of four 15-week-old mongrel dogs were used. All teeth were extracted except the third premolar of the right mandible, which was used as a control. After extraction of the premolars, each dog was administered BCP (Osteon), demineralized bone matrix (DBM; DBX), and no graft in the hollow sockets of the right fourth premolar, left fourth premolar, and left third premolar, respectively. Radiographs were taken at 2-week intervals to check for tooth eruption. After 8 weeks, each dog was sacrificed, and tooth and bone biopsies were performed to check for the presence of tooth and bone substitute particle remnants. Results: Four weeks after the operation, permanent tooth eruptions had started at all the extraction sites in each dog. Eight weeks after the operation, all teeth had normally erupted, and histological examination revealed BCP particles at the right fourth premolar. Conclusion: In all four dogs, no delay in the eruption of the teeth or shape disfigurement of permanent teeth was observed on gross inspection and radiologic evaluation. On histological examination, most of the BCP and DBM were replaced by new bone. Bone substitutes can be used as graft materials in patients with alveolar clefts.
