Penanganan bipolar sangat memerlukan intensitas dukungan optimal di waktu yang tepat, namun jarak dan biaya terkadang menjadi kendala. Self-monitoring and psychoeducation in bipolar patients with a smart-phone application (SIMPLe) merupakan aplikasi berbasis smartphone dengan pendekatan psikoedukasi untuk untuk manajemen diri pada penderita bipolar disorder yang mudah dan hemat biaya. Kajian ini bertujuan untuk meninjau dan menganalisis mengenai efektifitas dan kepuasaan penggunaan aplikasi SIMPLe dalam pengobatan bipolar disorder. Artikel ini dirancang dengan menggunakan desain literature review dengan sumber literatur yang didapatkan dari database science direct, proquest dan scopus. Penggunaan aplikasi SIMPLe selama tiga bulan atau lebih terbukti efektif dalam menekan angka kekambuhan. Pengguna aplikasi merasa puas dengan konten yang berada di dalam aplikasi. Penambahan konten aplikasi seperti  rencana krisis pribadi di kasus kekambuhan, menambah peristiwa stres pribadi dalam grafik suasana hati, variasi yang lebih besar dari pesan psikoedukasi dan label suasana hati interaktif menjadi harapan pengguna dalam pembaharuan aplikasi berikutnya.

ABSTRACTSerotonin (5-HT) has been implicated in the pathophysiology of bipolar disorders. Acute tryptophan depletion (ATD), which decreases serotonergic turnover, is an established paradigm to study serotonergic vulnerability in affective disorders. Literature on the application of ATD as a research tool in bipolar patients is limited to three studies, which revealed inconsistent results on mood modification. These inconsistencies may be attributed to differences in methodological procedures and / or characteristics of included patients. Patient selection, methodological aspects and procedures of these studies are critically considered and recommendations given. A research protocol to test the 5-HT vulnerability in bipolar disorder is proposed.

BACKGROUND Psychoeducation for bipolar disorder has significant impact on symptoms and treatment adherence. In Iran, as a low-resource setting, infrastructural barriers may hinder optimum delivery of this evidence-based intervention to patients, such as inadequate mental health professionals and difficulties in transportation and costs of care. OBJECTIVE This study seeks to explore the acceptability and feasibility of a web-based intervention for bipolar patients in Iran. METHODS A website has been developed as a platform for providing psychoeducational contents about bipolar disorder. Patients were chosen via a convenient sampling method in 2018-2019. The main component of the intervention included streaming 7 weekly video clips after attending a single in-person meeting, as well as a medication self-monitoring application. Information was collected about the feasibility and acceptability of the intervention. RESULTS 45 patients were invited from the day center and the outpatient clinic of Roozbeh psychiatric hospital, and some private clinics in Tehran. Twenty-three patients (51%) attended the first in-person session and provided the informed consent, and among this group, 14 patients dropped out during the study. Nine patients completed the course (attended 4 or more online sessions), while only 5 watched all the video sessions. The rate of adherence to the intervention and frequency of exposure to the website was much higher for those recruited from the private and outpatient clinics. CONCLUSIONS This web-based intervention can be feasible and acceptable only for a subgroup of patients with a specific educational status and socioeconomic level.

Abstract Background Despite an increasing number of available mental health apps in the bipolar disorder field, these tools remain scarcely implemented in everyday practice and are quickly discontinued by patients after downloading. The aim of this study is to explore adherence characteristics of bipolar disorder patients to dedicated smartphone interventions in research studies. Methods A systematic review following PRISMA guidelines was conducted. Three databases (EMBASE, PsychInfo and MEDLINE) were searched using the following keywords: "bipolar disorder" or "mood disorder" or “bipolar” combined with “digital” or “mobile” or “phone” or “smartphone” or “mHealth” or “ehealth” or "mobile health" or “app” or “mobile-health”. Results Thirteen articles remained in the review after exclusion criteria were applied. Of the 118 eligible studies, 39 did not provide adherence characteristics. Among the selected papers, study length, sample size and definition of measures of adherence were strongly heterogeneous. Activity rates ranged from 58 to 91.6%. Conclusion The adherence of bipolar patients to apps is understudied. Standardised measures of adherence should be defined and systematically evaluated in future studies dedicated to these tools.

Background: Patients with bipolar disorder demonstrate increased sensitivity to appetitive/rewarding stimuli even during euthymia. On presentation of arousing pictures, they show a peculiar response, suggesting heightened vigilance. While responding to looming arousing cues, studies show subjects with anxiety spectrum disorders exhibit increased reaction time (RT), explained by the “looming-vulnerability model.” This study aimed to investigate the responses to looming arousing cues in euthymic bipolar patients and their first-degree relatives, as compared to healthy controls. Method: A looming appetitive and aversive cue paradigm was designed for assessing the RT of patients to process appetitive and aversive cues. The behavioral inhibition/activation and sensitivity to reward/punishment amongst the groups were also assessed. Results: The bipolar group showed significantly longer RT to process appetitive cues irrespective of the looming condition. Aversive cues elicited significantly longer RT in both the bipolar group and in first-degree relatives, but only when presented with the looming condition. Significant looming bias was elicited in the bipolar group which suggested a particular cognitive style to looming cues. A composite measure of RT along with sensitivity to reward/punishment distinguishes the bipolar group and their first-degree relatives from the healthy controls. Conclusion: The looming vulnerability model may provide important insights for future exploration of cognitive endophenotypes in bipolar disorder.
