Abstract Background There are limited data which show how Emergency Department (ED)-specific knowledge and skills are acquired throughout the internal medicine fellowship, especially during 24-hour shifts. The aim of the study was to assess the differences in medical practice between fellows among different fellowship-years in ED settings. Methods We conducted a cross-sectional, retrospective study at a large university hospital using data on every ED examination done during 2016. All data were taken from the hospital information system. We compared 1st-year fellows with all older fellows, as well as 5th-year fellows with all younger fellows. Results In 2016, 55 fellows had ≥ 15 24-hour shifts with 3 attending fellows during one shift. A total of 19’916 patients were examined by attending fellows in the ED. When compared to 2nd-year fellows, 1st-year fellows examined more patients brought to ED by out-of-hospital emergency medical team and had significantly higher share of examined patients per 24-hour shift with lower length of stay. When compared to 5th-year fellows, 1st-year fellows examined older patients, but had lower share of examined patients during one 24-hour shift and participated in fewer CPR cases. When compared to younger fellows, 5th-year fellows participated in more CPR cases and had higher proportion of patients admitted to hospital. Conclusion To our knowledge, this is the first report on the ED medical practice differences among fellows of different fellowship-year. When compared to other fellows, 1st-year fellows participated less in CPR cases, but were faster in providing medical care in ED. 5th-year fellows had the highest proportion of patients admitted to hospital and more CPR cases.

<p class="abstract"><strong>Background:</strong> Otitis externa is common in medical practice. This study aims to identify the contributing factors of otitis externa in the city of Antsiranana, Madagascar.</p><p class="abstract"><strong>Methods:</strong> This is an analytical case-control type study, concerning patients with otitis externa, seen from January to July 2019, at the university hospital of place Kabary and at the grand Pavois medical practice, all in Antsiranana. The epidemiological, behavioural and clinical parameters were evaluated.  </p><p class="abstract"><strong>Results:</strong> We included 153 patients, among which 51were cases and 102 controls. The average age was 32.9 years (±19.41). The occurrence of otitis externa was significantly associated with the existence of a history of otitis externa, as well as ear cleaning 4 to 6 times per week (p=0.00; OR=46.17; CI=5.90-361.02). The diabetes exposure ratio was 18.79 in the ‘cases’ (p=0.00; CI=2.27-154.88), this ratio was 10.71 in the event of repeated swimming in pool (p=0.00; CI=2.22-51.70), 3.94 in case of ear cleaning with the cotton swab (p=00; CI=1.34-11, 60), and 2.23 when using headphones (p=0.02; CI=1.05-4.75).  The treatment of a previous otological pathology was a protective factor against otitis externa (OR=0.1500, p=0.0159, CI=0.0259-0.8689).</p><p class="abstract"><strong>Conclusions:</strong> The occurrence of otitis externa is related to well define daily behaviours and to other associated clinical factors. Managing these risk factors is the first step in treatment of otitis externa.</p>

Recently, the functional aspect as well as morphological aspect of the reserve cells in the cervix uteri drew much attention in view of the carcinogenesis in squamocolumunar junction. In this communication, the authors elucidate the ultrastructural features of the reserve cells in patients of various age groups visiting our university hospital and affiliated hospital.From conventional light microscopic point of view, the reserve cells tend to be pronounced in various pathological conditions, such as the persisting inflammation, proliferative disorders and irritation of hormones. The morphological patterns of the reserve cells from various stage and degree of irritation were observed.
