The successful implementation of the six sigma quality improvement program is demonstrated through the improvement of the capability process in producing products leading to zero defect. Therefore, the concept of calculating process capability is very important to understand in the implementation of the Six Sigma program. This research raises the problem of the low level of capability process in producing yarns at PT. Sutra Cotton Textile Industries, Surabaya. By wisely applying six sigma techniques can help reduce things that can affect overall costs. If a company has successfully implemented the concepts in six sigma, then it can be ascertained that the company is truly a reliable company in the world class. Another advantage gained from the implementation of the Six Sigma program is the achievement of targets, namely the increased capability process, of course, in terms of cost. From secondary data in May 2019, in the measure phase it is known that the DPMO value of 16,266 which can be interpreted that of one million production carried out there was a defect of 16,266 with a sigma value of 3.64. From these results it can be said that the production process in Winding has not been able to produce products that fit the desired specifications and is still lagging behind other subprocesses. method. The main priority in carrying out corrective actions based on FMEA is to clean and check the machine at the end of each production cycle. From the results of improvements, the sigma value and DPMO value obtained in September 2019 is 10,977 DPMO which means that of one million production carried out there is a defect of 10,977 with a sigma value of 3.80.

Purpose The purpose of this paper is to study the anodising process of a portable amplifier production process to identify and eliminate the sources of variations, in order to improve the process productivity. Design/methodology/approach The study employs the define-measure-analyse-improve-control (DMAIC) Six Sigma methodology. Within the DMAIC framework various tools of quality management such as SIPOC analysis, cause and effect diagram, current reality tree, etc., are used in different stages. Findings High rejection rate was found to be the main problem leading to lower productivity of the process. Four types of defects were identified as main cause of rejections in the baseline process. Pareto analysis resulted in detection of the top defects, which were then analysed in details to find the root cause of the problem. Further study resulted in finding improvement measures that were discussed with the management before implementation. The process is sampled again to check the improvements, and control measures were established. Practical implications The study provides a framework for implementation of DMAIC Six Sigma methodology for a manufacturing firm. The results presented are based on the data collected from the shop floor. Results and findings of the study were implemented for quality improvement of the process. Originality/value The study is based on an original research conducted with the objective of quality improvement in the anodising process of the production process. Besides presenting an approach to DMAIC Six Sigma methodology, an application of the current reality tree tool for root cause analysis is presented, a tool used limitedly in the Six Sigma studies. The tool finds its uniqueness in its ability to address problems relating multiple factors than isolated factors.

Traditional cast brass handicrafts Majapahit is one of the nation's cultural heritage which has a high historical value. The uniqueness and quality become the hallmark of this craft products. The results of handmade products such as sculptures, wall hangings, souvenirs, ornaments doors and so on. To increase the sales of traditional handicrafts of brass castings, required Majapahit applicative technical efforts in improving the quality of products which are ecofriendly. Quality improvement method such as Six sigma. Six sigma focuses on decreasing flawed rate, by reaching the standard 3, 4 flawed in a thousand possibility. Six sigma has 5 phases; define, measure, analyze, improvement, and control. In this research, six sigma is applied in producing cast brass handicraft starting from making the master mold, preparation of smelting raw materials, cooling process, releasing of the mold, and finishing. Purposefully to improve the quality by overcoming and decreasing the high of flawed possibility and also applying marketing mixture in improving the market segmentation of traditional cast brass of Majapahit market. The result is quality improvement is shown by decreasing happen in number of flawed product in total of mean for about 154, 8 decreased to 59,5. While the calculation of the six sigma, flawed product of cast brass handicraft is for about 1,7625 sigma value or being in level 2 and increased to 3,725 or being in level 4 with the flawed possibility in amount 235,000 for thousand times of production. The ability of production for the requirement which is wanted by the consumers and owning a good enough capability but still needed some correction to achieve world class home industry

Purpose This paper is one of seven in this volume that aims to elaborate different approaches to quality improvement in education. It delineates a methodology called Six Sigma. Design/methodology/approach The paper presents the origins, theoretical foundations, core principles and a case study demonstrating an application of Six Sigma in a school-community partnership in Milwaukee, Wisconsin. Findings The core principles underlying the approach are decreasing variability or unreliability in organizational work processes, eliminate waste or activity that does not add value to desired outcomes, identify defects and decrease their incidence, reduce the cost of work processes, and improve beneficiary/client satisfaction levels. The steps in this statistics-dependent method are design, measure, analyze, improve and control. Originality/value Few theoretical treatments and demonstration cases are currently available on commonly used models of quality improvement that might have potential value in improving education systems internationally. This paper fills this gap by elucidating one promising approach. The paper also derives value as it permits a comparison of the Six Sigma approach with other quality improvement approaches treated in this volume.

Postoperative pneumonia increases morbidity, length of stay, and hospital readmission rates. Current data support the use of perioperative chlorhexidine gluconate in elective cardiac surgery patients to prevent postoperative pneumonia. The objectives of this study were to implement a resident-driven quality improvement project and determine the efficacy of an oral care bundle in preventing postoperative pneumonia among noncardiac surgical patients. A retrospective review of postoperative pneumonia occurrences at our hospital captured by the NSQIP database from 2014 to 2016 was conducted. A pre- and postoperative pulmonary care bundle was implemented in all surgical patients undergoing general anesthesia and outcomes were tracked by NSQIP for up to 90 days postoperatively for calendar year 2017. The NSQIP-reported incidence of postoperative pneumonia at our hospital was reduced from 0.8 to 0 per cent (P = 0). The risk-adjusted smoothed rate fell from 1.17 (95% confidence interval 0.77–1.66) in 2014 to 0.33 (95% confidence interval 0.03–0.98) in 2017. We encountered multiple systematic issues while conducting this study, which led to an imbalanced compliance to the preoperative (90%) and postoperative (31%) bundle; however, there was no significant difference between these two groups. Successful implementation of a resident-driven quality project resulted in a decreased rate of postoperative pneumonia.

Abstract Background Quality Improvement Networks Quality Improvement Organizations (QIN-QIOs) developed community coalitions to align care coordination efforts for Medicare beneficiaries in order to reduce readmission rates within geographically defined communities. This CMS (Centers for Medicare & Medicaid Services) funded national quality improvement program worked with 380 coalitions from 2014-2019, facilitating a variety of interventions within each community. Baseline readmission rates among communities, calculated from claims data, varied from 17.7 to 112 readmissions/1000 beneficiaries. Program results ranged from +40.7% (high performance) to -35.8% (low performance) relative improvement.We applied an implementation framework (CFIR) to the QIN-QIO efforts to define common characteristics of interventions, implementation strategies, and contexts in which improvement efforts took place. We identify features associated with successful and unsuccessful intervention implementation, and with changes in readmission rates.Methods We selected 22 communities representing a range of relative improvement, geographic characteristics and baseline readmissions rates. We measured the QIN-QIO’s perception of influence of individual CFIR constructs on community readmission rates over time using a written assessment and elicited details and mechanisms through structured interviews. Two independent reviewers qualitatively coded transcribed interviews. Final ratings for the influence of each CFIR construct on community performance were assigned by consensus, ranging from -2 (strong negative influence) to +2 (strong positive influence).Results Some adaptation of the CFIR, such including codes in a coalition domain, and adding constructs to the outer setting domain, such as healthcare market characteristics, helped fit the framework to the QIN-QIO work. The characteristics of individuals domain was less applicable to this study. Several constructs were found to be associated with improvement, or lack of, in readmission rates in communities.Conclusions The CFIR is an appropriate taxonomy for understanding implementation of care coordination interventions in the QIN-QIO communities, with constructs from the Outer Setting and Process domains having the most influence on successful implementation. Communities effectively reducing readmissions had coalitions with favorable implementation climates, robust stakeholder engagement strategies, and interventions aligned with local concerns and capabilities. The CFIR can help guide, monitor and evaluate community-based improvement initiatives, although further development some constructs is needed.
