Changes in the systemic inflammatory response in women in the third trimester of pregnancy complicated by exacerbation of cytomegalovirus infection

Background: The mother’s diet has a direct impact on fetal development and pregnancy, and can also be important in the course of the body’s inflammatory response. An anti-inflammatory diet can be a promising way to counter an excessive inflammatory response in pregnancy. Objective: The aim of the study was to examine the association between the dietary inflammatory index (DII) and the pregnant women’s serum interleukin 6 (IL-6) and 10 (IL-10) and C-reactive protein (CRP) concentration in the course of normal and complicated pregnancy. Research Methods and Procedures: The study included 45 Polish pregnant women recruited to the study. The DII, a literature-based dietary index to assess the inflammatory properties of diet, was estimated based on a seven-day 24-h recall and an food frequency questionnaire (FFQ) in each trimester of pregnancy. At the same time as the nutritional interviews, blood samples were collected for the determination of IL-6, IL-10, and CRP concentrations. The studied group was divided into subgroups with normal and complicated pregnancy and depending on the DII median. Results: With the development of pregnancy, the DII score slightly decreased in subsequent trimesters: −1.78 in the first trimester, −2.43 in the second trimester, and −2.71 in the third trimester (p = 0.092). Independent of the trimester of pregnancy and the occurrence of pregnancy complications, the DII score did not affect the differences in the serum concentrations of IL-6, IL-10, and CRP, with the exception of CRP level in the second trimester in women with complicated pregnancy (subgroup with DII < median had a lower CRP level than subgroup with DII > median). In the first and third trimesters, there was a weak but significant positive correlation between the DII score and CRP concentration. During the second trimester, in the group with normal pregnancy and DII below the median, a significant negative correlation between the DII score and the serum IL-6 and IL-10 concentration was noted as well as in the third trimester for IL-6. Conclusion: The anti-inflammatory potential of a pregnant woman’s diet increases slightly with pregnancy development; however, its value has no permanent significant association with the level of CRP, IL-6, and IL-10.

The aim of this study was to investigate whether prepartal leukogram in cows with retained placenta could indicate the presence of subclinical systemic inflammatory response before the onset of disease. After calving, sixteen highly pregnant Holstein cows, aged 3 to 9 years, without clinical signs of the disease prior to calving were divided into two groups: the first group (n=9) were animals without retained placenta, or any visible inflammation after birth; the second group (n=7) were cows with retained placenta. Blood was sampled three times before parturition, at intervals of one week, and once 24 hours after birth. The number of total leukocytes, segmented and non segmented neutrophilic granulocytes (NG), lymphocytes and monocytes were determined by standard laboratory techniques. The results have shown that in the group of cows with retained placenta the number of mature neutrophils was slightly elevated in the third, second and last week before calving, and equal number of non segmented neutrophils in regard to the group with no retention. The results have also shown that, in both groups of cows, 24 hours after calving, the number of total leukocytes and the number of segmented neutrophils decreased, but the number of the non segmented neutrophils increased. Based on this, we can conclude that cows with retained placenta had no systemic inflammatory response during three weeks prepartal period, but 24 hours after calving, systemic inflammatory response was documented in all the cows. Moreover, the intensity of inflammatory response in cows with retained placenta was not more pronounced in comparison to cows without retained placenta.

The aim of the work is to study the activity of lipoperoxidation processes and antioxidant protection system in the umbilical cord blood of newborns from CMV-seropositive women with exacerbation of infection in the third trimester of pregnancy. Lipid peroxidation activity and antioxidant status in 30 newborns from CMV-seropositive women with exacerbation of infection in the third trimester of pregnancy and in 25 from CMV-seronegative women were studied. Erythrocytes were used to determine the products of oxidative modification of lipids; antioxidant status was evaluated in the blood plasma of the umbilical vein. The content of TBK-active products was determined by fluorometric method, lysophosphatidylcholine by thin-layer chromatography, arachidonic acid by gas-liquid chromatography. Antioxidant status was assessed by α-tocopherol content and superoxide dismutase activity. Registration of optical densities and fluorescence was carried out using a spectrofluorometer. The study found an increase in the content of red blood cells of the umbilical cord of newborns from CMV-seropositive women with an exacerbation of infection in the third trimester of pregnancy of TAC-active products in 2.1 times (p˂0.001), of lysophosphatidylcholine in 1.9 times (p˂0.001) and of arachidonic acid in 1.5 times (p˂0.001). The antioxidant status of umbilical cord blood was characterized by 1.5 times decrease in α-tocopherol (p<0.01) and 1.3 times decrease in superoxide dismutase activity (p˂0.001) compared with similar indices in the group of newborns from CMV-seronegative women. The results of the study show that the exacerbation of cytomegalovirus infection in the third trimester of pregnancy is accompanied by the development of oxidative stress and tension in the system of antioxidant protection in the blood of the umbilical cord of newborns due to increased consumption of α-tocopherol and suppression of superoxide dismutase activity.
