Development of a specialized module of a medical information system for recording relaparotomies and analyzing the effectiveness of the use of vacuum therapy

Aim research: to develop and implement a patient registration register after relaparotomies in operation on the basis of a clinical oncological dispensary. The premise of this study was the need to obtain statistical data to analyze the effectiveness of vacuum therapy during relaparotomies. By automating the process of recording relaparotomy data, it will be possible to analyze the impact of various factors on patient mortality. Designed the interface and database structure. Based on the described methods and tools, a specialized module of the medical information system for recording relaparotomies has been developed. Since the development took into account all the requirements of medical specialists in a specific field of knowledge, it was possible to develop a specialized software tool that can be used to obtain specific statistics. A patient registration register after relaparotomy was developed and implemented as a specialized module of the medical information system. The register collects, analyzes and interprets data on surgical methods used, including the use of vacuum therapy. To achieve the goal set in this work, the methods and tools presented in the article were used, which were applied in practice and can be similarly used in other medical institutions.

Developed medical information system (MIS) that enables you to automate and organize the processing of clinical database in medical institutions (MIS). In a study using IIAs analyzed data of dynamic observation of patients with various types of exudative edematous macular degeneration on the background of peripheral uveitis 376 people (406 eye). For professional computer analysis of the data used in the statistical package SPSS for Windows», which enabled to evaluate the effectiveness of a new method of treatment of patients with age-related macular degeneration.

The article presents the results of an empirical study of the value orientations of dental clinic employees. It is suggested that gaps in human values are factors that influence professional activities in terms of communication (in the production team) and ultimately the profitability of the individual in the teams. The initial data is obtained from the reports of the clinic’s medical information system and the Schwartz’s Value Survey (SVS) and Portrait Values Questionnaire (PVQ). In addition, we used a retrospective analysis and a questionnaire survey. Doctors were divided into two groups based on the impact of value gaps on the share of revenue plan fulfillment (type one and type two groups). Attention is drawn to the direction of relationships in the group of the first type, all correlations of average strength are direct, in the group of type 2 – reverse. In the structure of the profile of value orientations, there are differences between the groups on the second place in importance for the respondents in the group of the first type are the value orientation (VO) “Achievement” and secondly, “Self-Direction”, third place in the group of the first type is “Benevolence”, whereas in the group of the second type of “Benevolence” in 4th place. To increase economic efficiency, managers of medical institutions need to organize activities to create cultural artifacts about the normative ideals of those values that affect the workflow, take into account the data of the employee’s value profile for optimal selection of the team composition (work shift).

Purpose. The purpose of the article is to substantiate the need to modernize the information and communication support of medical institutions of Ukraine to improve the quality of medical care and medical services. Methodology of research. The theoretical and methodological basis of the study are the fundamental provisions of modern economic theory, scientific works of leading foreign and domestic scientists, international and domestic regulations on health care. General scientific and special research methods are used to achieve this goal, in particular: the method of system analysis – to study the information and communication support of medical institutions as a system; the method of structural analysis – to study the structure of the software of the medical information system “Doctor Eleks”; the method of comparison – to compare different methods of information processing in the studied medical information system. Findings. The need for modernization of information and communication support of medical institutions is determined. Prospects for the introduction of medical information systems in health care facilities, such as the “Doctor Elex” information system, are substantiated. Examples of introduction of the medical information system "Doctor Eleks" in foreign and domestic medical institutions are considered. The advantages of the researched medical information system “Doctor Eleks” for: management staff, medical staff and registry staff are formed. The software of the medical information system “Doctor Eleks” is analysed in detail in the context of the main modules: “registry”, “doctor”, “laboratory”, “administration”, “templates”. It is proved that the introduction of the medical information system “Doctor Eleks” in the activities of medical institutions will significantly increase the level of coordination of its staff and will positively affect the quality of medical care and medical services. Originality. The mechanism of introduction of modern information technologies in activity of medical institutions as the factor of increase of quality of medical service on the basis of use of the information system “Doctor Eleks”, which is developed for automation of work of medical institutions is offered. Practical value. The detailed analysis of the advantages of the medical information system “Doctor Eleks” can be useful for directors of domestic medical institutions in choosing the priorities of modernization of their information and communication support. A pilot medical information system is being implemented and currently is providing services for limited categories of patient data. In one year, physicians’ diagnoses for 500,000 office visits, 300,000 drug prescriptions for outpatients, one million clinical laboratory tests, and 60,000 multiphasic screening examinations are being stored in and retrieved from integrated, direct access, patient computer medical records.This medical information system is a part of a long-term research and development program. Its major objective is the development of a multifacility computer-based system which will support eventually the medical data requirements of a population of one million persons and one thousand physicians. The strategy employed provides for modular development. The central system, the computer-stored medical records which are therein maintained, and a satellite pilot medical data system in one medical facility are described.
