Developed medical information system (MIS) that enables you to automate and organize the processing of clinical database in medical institutions (MIS). In a study using IIAs analyzed data of dynamic observation of patients with various types of exudative edematous macular degeneration on the background of peripheral uveitis 376 people (406 eye). For professional computer analysis of the data used in the statistical package SPSS for Windows», which enabled to evaluate the effectiveness of a new method of treatment of patients with age-related macular degeneration.

Aim research: to develop and implement a patient registration register after relaparotomies in operation on the basis of a clinical oncological dispensary. The premise of this study was the need to obtain statistical data to analyze the effectiveness of vacuum therapy during relaparotomies. By automating the process of recording relaparotomy data, it will be possible to analyze the impact of various factors on patient mortality. Designed the interface and database structure. Based on the described methods and tools, a specialized module of the medical information system for recording relaparotomies has been developed. Since the development took into account all the requirements of medical specialists in a specific field of knowledge, it was possible to develop a specialized software tool that can be used to obtain specific statistics. A patient registration register after relaparotomy was developed and implemented as a specialized module of the medical information system. The register collects, analyzes and interprets data on surgical methods used, including the use of vacuum therapy. To achieve the goal set in this work, the methods and tools presented in the article were used, which were applied in practice and can be similarly used in other medical institutions.

Возрастная макулярная дегенерация считается одной из распространенных возраст ассоциированных офтальмологических патологий, приводящих к полной или частичной потере зрения. Вероятность развития возрастной макулярной дегенерации существенно повышается при наличии сопутствующей хронической сердечной недостаточности. Однако изучение динамики заболеваемости возрастной макулярной дегенерацией при сочетании хронической сердечной недостаточности осуществляется крайне редко. Цель настоящего исследования - анализ заболеваемости возрастной макулярной дегенерацией и прогнозирование ее распространенности в Тамбовской области у пациентов с хронической сердечной недостаточностью. Исследование проводилось ретроспективным методом за 2013-2018 годы по данным официальной статистики лечебно-профилактических учреждений общего и специализированного профиля. При сборе информации использовались данные, содержащиеся в «Медицинской когорте амбулаторного больного», «Медицинской карте амбулаторного больного». Для прогнозирования заболеваемости возрастной макулярной дегенерацией использовался регрессионный анализ и программа «Statistica 10.0». В ходе исследования установлено повышение уровня заболеваемости с 13,54 случаев на 100 000 населения в 2013 году в базовом регионе до 28,42 случаев на 100 000 населения в 2018 году с достоверной разницей. Разработанная прогностическая модель у=0,7124 ln(x1)+1,4586 показывает, что в 2019-2021 годах произойдет дальнейшее увеличение заболеваемости возрастной макулярной дегенерацией. При этом наибольшую распространенность среди выделяемых форм возрастной макулярной дегенерации в Тамбовской области за исследуемый временной период имеет неэкссудативная форма, варьирующая в диапазоне от 12,41 случаев до 25,98 случаев на 100 000 населения (р<0,001). Экссудативная возрастная макулярная дегенерация варьирует от 1,42 случаев до 2,30 случаев на 100 000 жителей области. Полученные данные представляют интерес для принятия упреждающих профилактических мероприятий по стабилизации заболеваемости возрастной макулярной дегенерацией Age-related macular degeneration is considered one of the most common age-associated ophthalmological pathologies leading to complete or partial loss of vision. The probability of developing age-related macular degeneration increases significantly in the presence of concomitant chronic heart failure. However, the study of the dynamics of the incidence of age-related macular degeneration in combination with chronic heart failure is extremely rare. The purpose of this study is to analyze the incidence of age-related macular degeneration and predict its prevalence in the Tambov region in patients with chronic heart failure. The study was conducted retrospectively for 2013-2018 according to official statistics of general and specialized medical institutions. When collecting information, the data contained in the "Medical cohort of an outpatient patient", "The medical record of an outpatient patient" were used. Regression analysis and the Statistica 10.0 program were used to predict the incidence of age-related macular degeneration. The study found an increase in the incidence rate from 13.54 cases per 100,000 population in 2013 in the base region to 28.42 cases per 100,000 population in 2018 with a significant difference. The developed prognostic model y1=0.7124 ln(x1)+1.4586 shows that in 2019-2021 there will be a further increase in the incidence of age-related macular degeneration. At the same time, the greatest prevalence among the isolated forms of age-related macular degeneration in the Tambov region over the studied time period has a non-exudative form, varying in the range from 12.41 cases to 25.98 cases per 100,000 population (p<0.001). Exudative age-related macular degeneration varies from 1.42 cases to 2.30 cases per 100,000 inhabitants of the region. The data obtained are of interest for taking proactive preventive measures to stabilize the incidence of age-related macular degeneration

Purpose Visual outcome in patients with neovascular age-related macular degeneration is variable. We aimed to evaluate for association between socioeconomic status visual acuity in neovascular age-related macular degeneration. Methods A retrospective single-center study of a consecutive group of neovascular age-related macular degeneration patients was performed. Socioeconomic status was determined for each patient based on the 2008 Israeli census. Medical information was extracted from medical records and included visual acuity and optical coherence tomography parameters. Associations between socioeconomic status and clinical outcomes were analyzed. Results A total of 233 patients were included in the analysis. A correlation was found between low baseline visual acuity of the first eye diagnosed with neovascular age-related macular degeneration and low socioeconomic status (r = −0.13, p = 0.049; n = 233). The difference between the visual acuity of the lowest and the highest socioeconomic status categories at baseline was approximately 3 ETDRS lines (p = 0.048). Socioeconomic status and baseline visual acuity of the second eye of the same individual with neovascular age-related macular degeneration were not correlated (r = −0.05, p = 0.95). Socioeconomic status was not associated with the number of anti-vascular endothelial growth factor injections of the first or second eye, or the visual acuity outcome of the first or second eye after 1 year of therapy (p = 0.421, p = 0.9, respectively). Central subfield thickness of the first eye at presentation as measured by spectral-domain optical coherence tomography was associated with socioeconomic status (r = −0.31 p = 0.001). Conclusion Individuals of lower socioeconomic status presented at more advanced stage of the disease when developing neovascular age-related macular degeneration in the first eye but not in the second eye. The research underscores the importance of improving referral patterns and awareness for the lowest socioeconomic status classes.

Purpose. The purpose of the article is to substantiate the need to modernize the information and communication support of medical institutions of Ukraine to improve the quality of medical care and medical services. Methodology of research. The theoretical and methodological basis of the study are the fundamental provisions of modern economic theory, scientific works of leading foreign and domestic scientists, international and domestic regulations on health care. General scientific and special research methods are used to achieve this goal, in particular: the method of system analysis – to study the information and communication support of medical institutions as a system; the method of structural analysis – to study the structure of the software of the medical information system “Doctor Eleks”; the method of comparison – to compare different methods of information processing in the studied medical information system. Findings. The need for modernization of information and communication support of medical institutions is determined. Prospects for the introduction of medical information systems in health care facilities, such as the “Doctor Elex” information system, are substantiated. Examples of introduction of the medical information system "Doctor Eleks" in foreign and domestic medical institutions are considered. The advantages of the researched medical information system “Doctor Eleks” for: management staff, medical staff and registry staff are formed. The software of the medical information system “Doctor Eleks” is analysed in detail in the context of the main modules: “registry”, “doctor”, “laboratory”, “administration”, “templates”. It is proved that the introduction of the medical information system “Doctor Eleks” in the activities of medical institutions will significantly increase the level of coordination of its staff and will positively affect the quality of medical care and medical services. Originality. The mechanism of introduction of modern information technologies in activity of medical institutions as the factor of increase of quality of medical service on the basis of use of the information system “Doctor Eleks”, which is developed for automation of work of medical institutions is offered. Practical value. The detailed analysis of the advantages of the medical information system “Doctor Eleks” can be useful for directors of domestic medical institutions in choosing the priorities of modernization of their information and communication support. Aim. To improve measures aimed at ensuring the continuity of the dynamic health monitoring of students on the basis of medical institutions of the University. Methods. On the basis of the technology «client-server», which is oriented on the management system of the Firebird databases, developed was an information-analytical system of dynamic observation of the health of students. Results. This system presumes a specific algorithm for the medical examination, first of all the screening methods: psychological testing, measurement of anatomical and physiological parameters, obtaining of the medical history, physical examination, cardiorhythmography and routine clinical and laboratory tests. With the help of original methods the medical information system provides a complex report on the state of health of the student and on the risks of the most common diseases in this age group. Conclusion. Health care providers of medical institutions of a University can use the developed system for dynamic monitoring of the health of the students.

Die altersbedingte Makuladegeneration (AMD) ist eine der häufigsten Ursachen für einen irreversiblen Visusverlust bei Patienten über 65 Jahre. Nahezu 30% der über 75-Jährigen sind von einer AMD betroffen. Trotz neuer Erkenntnisse in der Grundlagenforschung ist die Ätiologie, zu der auch genetische Faktoren gehören, noch nicht völlig geklärt. Aus diesem Grund sind die Behandlungsmöglichkeiten zum jetzigen Zeitpunkt noch limitiert, so dass man lediglich von Therapieansätzen sprechen kann. Die derzeit zur Verfügung stehenden Möglichkeiten wie medikamentöse, chirurgische und laser- und strahlentherapeutische Maßnahmen werden beschrieben.
