Avabahuka is a disease of Amsasandhi ( Shoulder joint ) . Acharya Sushruta have described Avabahuka as a one of the type of Vata Vyadhi. It is one of the commonest musculoskeletal disorder . In Avabahuka,Vata gets lodged at the root of shoulder,subsequently constricting the veins and producing the loss of movements of the shoulder . Avabahuka can be corelate with Frozen shoulder having same complaints. Acharya Sushruta have mentioned Agnikarma for the treatment of Avabahuka

This report describes the regular use of acupuncture treatments for a patient with hypertension who could not tolerate the side effects of the antihypertensive agents. The patient received 60 acupuncture treatments in the course of 12 weeks, during which time his overall wellbeing improved, his blood pressure reduced and the side effects of antihypertensive drugs were removed. Although acupuncture plus the drug appeared to have a substantial synergistic effect that was weakened when the drug was discontinued, acupuncture may still play a role in the management of hypertension, especially for patients who cannot tolerate the side effects of antihypertensive agents.

In Frozen Shoulder (adhesive capsulitis), there is stiffness, pain and limited range of movements. It may happen after an injury, overuse of joint, from a disease such as diabetes or a stroke. Diabetes mellitus is a group of metabolic disorder with involvement of musculoskeletal system. In which most common is frozen shoulder. In frozen shoulder raise sugar in blood stick to collagen and restrict the movements. In first stage of frozen shoulder, Inflammation of capsule is present with severe pain.           Various diseases like Shoulder Impingement, Frozen Shoulder, and Rotator cuff tendinitis have resembling symptoms under the umbrella of disease Avabahuka described in Ayurveda. In Avabahuka there is Vata-Kapha dushti still Leech application was carried out in first stage of Frozen Shoulder. Leech application is indicated in Pittaj Dushthi and Hirudine present in saliva of Leech helpful in platelet aggregation inhibitor. Hence increase in blood supply to shoulder region may arrest fibrosis of capsule. Aim- Study the efficacy of Leech application in Frozen Shoulder in Diabetes Mellitus. Objective- To study the efficacy of frequent Leech application in the management of pain and range of motion of shoulder joint in Diabetes Mellitus. Hypothesis- Leech application is effective in Frozen Shoulder caused by diabetes Mellitus.

Introduction: Frozen shoulder is a self-limiting condition that affects daily activities. Early diagnosis and treatment is essential for better prognosis. Accurate diagnosis largely depends on reasoning skills of physiotherapy practitioners. Methods: Case study. Intervention: In this case, pattern recognition was used as diagnostic reasoning and posterior to anterior and caudal mobilization techniques to decrease pain and improve range of motion. Beside, theraband was to strengthen muscle strength of shoulder as well. Results: After application of 12 sessions of treatment, patient showed improvement in pain at VAS by 5 cm, improvement in passive ROMs by 20°, 10° and 25° in consequently in abduction, medial rotation and lateral rotation, improvement in SPDI by 42% and muscle power improved by Grade I in abductors, medical rotators and lateral rotators. Conclusion: Reasoning process helps physiotherapist for correct diagnosis and application of evidence based physiotherapy intervention showed significant improvements in symptoms after frozen shoulder.

Ayurveda is the traditional medical science of India. Vata aggravated brings about contractions of the Siras (nerves) of the Arm and produce the disease Avabahuka (Frozen Shoulder) then occurs Pain & loss of movement of the arm. Avabahuka is a disease that causes problems in daily activities of an individual like dressing, eating, personal hygiene and work. Shoulder joint is the most mobile joint in the body. Clinical presentation and pathogenesis of Avabahuka It Can be correlated with Frozen Shoulder. In classics, Nasya is indicated for Avabahuka. The aim of the present study was to evaluate the effectiveness of Nasya karma on Avabahuka. Method: In our Panchakarma OPD 55 years old male patient came with chronic pain in both shoulders joint. Patient was unable to movement both hand & he was Unable to carry out daily activities. Patient was taken 15 days Panchakarma therapy, After Snehana Nadi Swedana Nasya karma was done by Anu Tailam. Result: After 15 days of treatment there was marked relief of the symptoms like Pain, Stiffness etc. Conclusion: The therapies were found significantly effective and clinically safe and no adverse reactions were reported during the treatment period.
