Risks, Security, and Privacy for HIV/AIDS Data

Big data has the potential to transform healthcare systems for the prevention and treatment of HIV/AIDS by providing analytic tools that are capable of handling huge and different types of data at very fast speeds. Big data's transformative potential is also introverted by privacy and security requirements for HIV/AIDS patients' sensitive data that restrict health information exchange. Electronic health records provide the opportunity for HIV/AIDS patients to receive improved coordinated care from healthcare providers and easier access to their health information. This chapter discusses the various legal frameworks governing health information, dispels misconceptions about privacy regulations, and highlights how these legal frameworks provide privacy, confidentiality, and security to this sensitive information, and shows how EHRs can maximize the utility of big data to improve HIV/AIDS prevention and treatment.

Florida began the journey to health information connectivity in 2004 under Governor Jeb Bush. Initially these efforts were funded by grants, but due to the downturn in the economy, the state was unable to support growth in 2008. The American Recovery and Reinvestment Act of 2009 provided funding to further expand health information exchange efforts across the country. As a result, Florida is now able to move forward and make progress in information sharing. Harris Corporation was contracted to provide some basic services to the health care industry in 2011. However, challenges remain as privacy and security regulations are put in place to protect patients’ information. With two seemingly opposing mandates, sharing the information versus protecting the information, challenges continue to impede progress.

Summary Objectives: To summarize the recent literature and research and present a selection of the best papers published in 2016 in the field of Health Information Management (HIM). Methods: A systematic review of the literature for the IMIA Yearbook HIM section was performed by the two section editors with the help of a medical librarian. We searched bibliographic databases for HIM-related papers were searched using both MeSH headings and key words in titles and abstracts. A shortlist of candidate best papers was first selected by section editors before being peer-reviewed by independent external reviewers. Results: The five papers selected as ‘Best Papers’ illustrate a variety of themes, include authors from a variety of countries, and were published in some of the best journals in the field. The themes of the five best papers include health information exchange, personal health records, patient engagement, data quality, and e-quality measures. Conclusions: The discipline of Health Information Management is increasingly becoming allied with the field of Biomedical Informatics in that both disciplines have interests in common. Traditional HIM areas of expertise (in the pre-electronic health record world), such as coding and privacy and security of health information, are necessary for the electronic exchange and secondary use of health information. With the changes in healthcare delivery brought by the use of electronic health records, addressing issues of information governance is essential. This synopsis discusses these key issues at the intersection of HIM and informatics, examines the potential challenges, and points the way for best practices, future research, and public policy considerations and directions.

Summary Objective: The field of clinical informatics has expanded substantially in the six decades since its inception. Early research focused on simple demonstrations that health information technology (HIT) such as electronic health records (EHRs), computerized provider order entry (CPOE), and clinical decision support (CDS) systems were feasible and potentially beneficial in clinical practice. Methods: In this review, we present recent evidence on clinical informatics in the United States covering three themes: 1) clinical informatics systems and interventions for providers, including EHRs, CPOE, CDS, and health information exchange; 2) consumer health informatics systems, including personal health records and web-based and mobile HIT; and 3) methods and governance for clinical informatics, including EHR usability; data mining, text mining, natural language processing, privacy, and security. Results: Substantial progress has been made in demonstrating that various clinical informatics methodologies and applications improve the structure, process, and outcomes of various facets of the healthcare system. Conclusion: Over the coming years, much more will be expected from the field. As we move past the “early adopters” in Rogers' diffusion of innovations' curve through the “early majority” and into the “late majority,” there will be a crucial need for new research methodologies and clinical applications that have been rigorously demonstrated to work (i.e., to improve health outcomes) in multiple settings with different types of patients and clinicians.

Summary Objectives: To summarize excellent current research in the field of Health Information Systems. Method: Creation of a synopsis of the articles selected for the 2014 edition of the IMIA Yearbook. Results: Four papers from international peer reviewed journals were selected and are summarized. Conclusions: Selected articles illustrate current research regarding the impact and the evaluation of health information technology and the latest developments in health information exchange.
