This paper proposes a secure home healthcare system based on 6LoWPAN WSN. In this system, the hierarchical architecture based on clusters is proposed, and the hierarchical address structure based on location information is presented. Based on the architecture and the address structure, the secure address configuration algorithm is proposed. According to the destination IPv6 address, this system can automatically perform the routing in the link layer without route discovery. This system achieves the secure communication. Through this system, at any time a physician can monitor the physical parameters of a home-based patient such as a senior or less-mobile person, and a home-based patient can also alert a physician to call for help. In the latter case, the physician can quickly obtain the location of the patient in order to perform effective and timely treatment. The performance parameters of this system are evaluated and compared, and the data results show that this system effectively improves the system performance.

The proactive development in electronic health (e-health) has introduced seemingly endless number of applications such as telemedicine, electronic records, healthcare score cards, healthcare monitoring etc. Yet, these applications confront the key challenges of network dependence and medical personnel necessity, which hinders the development of universality of e-health services. To mitigate such key challenges, this chapter presents a versatile wired and wireless distributed e-home healthcare system. By exploiting the benefit of body sensor network and information communication technology, the dedicated system model methodically integrates some of the comprehensive functions such as pervasive health monitoring, remote healthcare data access, point-of-care signal interpretation and diagnosis, disease-driven uplink update and synchronization (UUS) scheme and emergency management to design a complete and independent e-home healthcare system.

Chronic Heart Failure (CHF) is a multi-faceted syndrome associated with high mortality and morbidity, as well as high health care costs from both patient and healthcare system perspectives. Optimal CHF disease management involves a high degree of information management and processing, for patients and providers, as well as timely and appropriate information sharing between them. Nurses have long been important conductors of information between patients and the healthcare system, and can provide a valuable perspective on the design of interactive information technology (IIT) to support multidisciplinary sharing of health information. The complimentary perspectives of holistic nursing and human-centred engineering design are discussed in evaluating multidisciplinary information needs and information management needs regarding CHF disease management of home-based CHF patients in Iceland, the author's home country.

The COVID-19 pandemic has necessitated the rise of telehealth modalities to relieve the incredible stress the pandemic has placed on the healthcare system. This rise has seen the emergence of new software, applications, and hardware for home-based physiological monitoring, leading to the promise of innovative predictive and therapeutic practices. This article is a literature-based review of the most promising technologies and advances regarding home-based physiological monitoring of patients with COVID-19. We conclude that the applications currently on the market, while helping stem the flow of patients to the hospital during the pandemic, require additional evidence related to improvement in patient outcomes. However, new devices and technology are a promising and successful venture into home-based monitoring with clinical implications reaching far into the future.

Abstract The E-healthcare system has a complex architecture, diverse business types, and sensitive data security. To meet the secure communication and access control requirements in the user-medical server, user-patient, patient-medical server and other scenarios in the E-healthcare system, secure and efficient authenticated key agreement and access authorization scheme need to be studied. However, the existing multi-server solutions do not consider the authentication requirements of the Wireless Body Area Network(WBAN), and are not suitable for user-patient, patient-medical server scenarios; most of the existing WBAN authentication scheme are single-server type, which are difficult to meet the requirements of multi-server applications; the study of user-patient real-time scenarios has not received due attention. This work first reveals the structural flaws and security vulnerabilities of the existing typical schemes, and then proposes an authentication and access control architecture suitable for multiple scenarios of the E-healthcare system with separate management and business, and designs a novel ECC-based multi-factor remote authentication and access control scheme for E-healthcare using physically uncloneable function (PUF) and hash. Security analysis and efficiency analysis show that the new scheme has achieved improved functionality and higher security while maintaining low computational and communication overhead.

With the increasing prevalence of people in developing countries who suffer strokes, the long-term care of people who have had a stroke and who are living with disabilities has substantial consequences for caregivers and their respective families. As the caregiver plays a pivotal role in the rehabilitation of the people who have had a stroke, the objectives of this study constitute an investigation into the complexities of caregiving, including both perceptions and experiences of the healthcare system. Semi-structured interviews were utilised to elicit post-stroke experiences of six caregivers. The challenge the South African context adds to these experiences was probed. The data were analysed qualitatively by thematic and content analysis. Prominent themes associated with caregiving included: role changes and relationship disruptions within the family, occupational and social implications, fatigue, anxiety, depression, loneliness, frustration as well as financial problems. Caregiver experiences were exacerbated by the inadequate support structures available. The results are interpreted within a biopsychosocial approach, concluding with the concerns raised by caregivers on the support they require from the healthcare system in order to provide home-based care.
