Hospitals are highly populated facilities that function 24 hours a day. They generate a large amount of wastewater through various sources which are very complex and hazardous. Furthermore, the emission of healthcare wastewater into the cities sewerage, rivers and groundwater pollute the entire system. Therefore, disposal of healthcare wastewater without adequate treatment can cause short and long-term ill-effects to the public's health. An onsite wastewater treatment would give a dual benefit of preventing the release of hazardous materials into other sources and bring the wastewater into the consumption cycle. At present, most countries utilize the treated wastewater to reduce the consumption of potable water. However, the literature review and preliminary studies revealed that wastewater management is poorly practised in the Sri Lankan healthcare sector. Therefore, this paper aimed to identify the extent of wastewater treatment, utilization and management in Sri Lankan state sector hospitals. The aim of the paper was achieved through a literature review and questionnaire survey. The required data were collected from nine state hospitals in Colombo. Findings of the study revealed that a high quality of wastewater treatment and efficient reuse of reclaimed water is a big challenge for the healthcare sector due to the complex nature of the wastewater process, higher cost of treatment and poor management. Moreover, it was found that currently wastewater generated from the state hospitals are discharged directly into the city's sewerage without any treatment. Finally, this paper discusses the current level of wastewater treatment, utilization and management in the state hospitals and identifies the critical barriers for the lack of poor practice of wastewater handling.

The inferior performance in housekeeping services has contributed to distinguishable levels of patient dissatisfaction. Therefore, similar to primary processes like medical treatment in healthcare sector same attention should be paid on secondary processes such as housekeeping services by the management. Moreover, the knowledge on housekeeping services is critical to plan and design proper housekeeping mechanisms in healthcare facilities. Therefore, this paper aims to explore the nature of housekeeping services in order to design proper housekeeping processes for private healthcare facilities in Sri Lanka. This research follows a qualitative research approach to examine the current housekeeping practices by adopting case study strategy. Semi structured interviews were conducted with seven respondents of the selected three cases from the private sector and direct observation method was used to better understand the housekeeping procedures. The results revealed that even though the bed capacity belongs to three groups, the housekeeping practices remain almost similar and the housekeeping in hospitals is essentially based on infection control and hygiene rather than aesthetical appearance compared to other facilities. Furthermore, the key differences such as absence of key designations such as upholsterers, painters, wall washers and catering team in the hierarchy and the absence of floor plans and area responsibility plans in the housekeeping divisions in Sri Lankan context were determined by comparing the research findings with the literature sources.

Design thinking and human-centred design is often discussed and utilised by teams and organisations seeking to develop more optimal, effective or innovative solutions for better customer outcomes. In the healthcare sector the opportunity presented by the practice of human-centred design and design thinking in the pursuit of better patient outcomes is a natural alignment. However, healthcare challenges often involve complex problem sets, many stakeholders, large systems and actors that resist change. High-levels of investment and risk aversion results in the status quo of traditional technology-led processes and analytical decision-making dominating product and strategy development. In this case study we present the opportunities, challenges and benefits that including a design-led approach in developing complex healthcare technology can bring. Drawing on interviews with participants and reflections from the project team, we explore and articulate the key learning from using a design-led approach. In particular we discuss how design-led practices that place patients at the heart of technology development facilitated the project team in aligning key stakeholders, unearthing critical system considerations, and identifying product and sector-wide opportunities.
