Contributing factors to ineffective nurse-physician communication: Survey results

Abstract Background Patient awareness surveys on polypharmacy have been reported previously, but no previous study has examined the effects of sending feedback to health professionals on reducing medication use. Our study aimed to conduct a patient survey to examine factors contributing to polypharmacy, feedback the results to health professionals, and analyze the resulting changes in the number of polypharmacy patients and prescribed medications. Methods After conducting a questionnaire survey of patients in Study 1, we provided its results to the healthcare professionals, and then surveyed the number of polypharmacy patients and oral medications using a before-after comparative study design in Study 2. In Study 1, we examined polypharmacy and its contributing factors by performing logistic regression analysis. In Study 2, we performed a t-test and a chi-square test. Results In the questionnaire survey, significant differences were found in the following 3 items: age (odds ratio (OR) = 3.14; 95% confidence interval (CI) = 2.01–4.91), number of medical institutions (OR = 2.34; 95%CI = 1.50–3.64), and patients’ difficulty with asking their doctors to deprescribe their medications (OR = 2.21; 95%CI = 1.25–3.90). After the feedback, the number of polypharmacy patients decreased from 175 to 159 individuals and the mean number of prescribed medications per patient decreased from 8.2 to 7.7 (p < 0.001, respectively). Conclusions Providing feedback to health professionals on polypharmacy survey results may lead to a decrease in the number of polypharmacy patients. Factors contributing to polypharmacy included age (75 years or older), the number of medical institutions (2 or more institutions), and patients’ difficulty with asking their physicians to deprescribe their medications. Feedback to health professionals reduced the percentage of polypharmacy patients and the number of prescribed medications. Trial registration UMIN. This paper presents findings of an investigation into benefits of using Building Information Modelling (BIM) in construction projects. The research methodology includes a review of BIM development and achievement in construction industry, questionnaire survey, interview, and content analysis of secondary data. Data was collected and analyzed to identify the various benefits reflected from BIM implementation in addressing common problems experienced by the construction industry. The review and survey results indicate that a high frequency of occurrence of time and cost overruns occur in construction projects. Contributing factors were mainly rework, poor planning, documents delivery and approval, constructability issues, resources and weather condition. While some of the factors were unpreventable, BIM could be used as a tool to eliminate problems causing time and cost overruns. Various project benefits have been reflected through the content analysis of project case studies. It was found that time was the most positively influenced by the use of BIM followed by coordination, procurement, communication, cost and change of scope of the project, and lastly, risk.

Abstract Background The Ulcerative Colitis (UC) Narrative global surveys examined patient and physician perspectives on living with UC and tried to identify gaps in optimal care. Questions explored patient-physician interactions, UC management goals, and resources for improving communication. Methods Questionnaires were conducted across 10 countries, covering aspects of UC including diagnosis, treatment, and impact on patient quality of life, in addition to standard demographic information. Descriptive statistics were calculated. Results Globally, 2100 patients and 1254 physicians were surveyed (from August 2017 to February 2018). Results showed 85% of patients were satisfied with the communication they had with their physician, including discussions relating to symptoms (86%) and medication options (81%). However, 72% of patients wished for more information and support at initial diagnosis, and 48% did not feel comfortable talking to their physician about emotional concerns. Most patients (71%) set UC management goals with their physician. Both patients (63%) and physicians (79%) wished for longer appointments. Although 84% of physicians believed patient advocacy organizations to be important in UC management, more than half (54%) never discussed them with patients. Conclusions These survey results highlight overall patient satisfaction with patient-physician communication but emphasize areas for improvement, such as patient desire to have more information earlier in their disease course. There is an unmet need for better information, materials, and support. Physicians need to consider which of the available tools and resources can help patients talk more openly, and accurately, because informed patients are more likely to engage with physicians in a shared decision-making process.

The present study’s primary objective is to identify contributing factors in selecting and evaluating a seafood supplier within the Taiwanese hospitality industry. It illustrates the application of a multicriteria decision-making process to supplier selection within a service setting where it is less common than in a manufacturing context. To implement the study, a survey instrument was created and submitted to Taiwanese hospitality firms, namely hotels and restaurants, to identify contributing factors in the selection of a seafood supplier within six initial areas concerning food hygiene, stafftraining, crisis management, information technology, competitive ability, and logistics and quality assurance. The analytic hierarchical process (AHP) was then applied to the survey results, and the first- and second-level hierarchical factors were rigorously identified and ranked. These can be regarded as useful benchmarks in identifying and ranking the selection and evaluation of a food supplier within the industry. The present study enhances the understanding of supplier selection in the hospitality industry and provides insights which hospitality firms can apply in managing their supply chains. The managerial and research implications of these findings are discussed.

Well-respected experts of higher education, including those at the National Survey of Student Engagement (NSSE) and founding director Dr. Alexander Astin of the Higher Education Research Institute at UCLA, have demonstrated strong correlations between the amount of time and effort students put into “educationally purposeful activities” outside the classroom (e.g., student organizations) and positive outcomes of student success, such as satisfaction, persistence, and academic achievement (Astin, 1984, 1993, 1999; Berger and Milem,1993; Pascarella and Terenzini, 2005). This study evaluates student involvement in on-campus student organizations. What characteristics are most highly correlated with participation? Based on the results of previous studies, we hypothesized that student engagement in on-campus student organizations is positively correlated with proximity to campus and that the closer a student lives to campus, the more apt they are to be involved with on-campus student organizations. Not only will students living closer to campus be more active with on-campus student organizations, but they are also more likely to be involved with on-campus student organizations directly related to their major. We also hypothesized that female students are more apt to be involved in on-campus student organizations related to their major than males. Original survey results and future research ideas are discussed.

After the feedback, the percentage of polypharmacy patients decreased from 21.5% to 20.1% and the mean number of prescribed medications per patient decreased from 8.2 to 7.7 (p<0.001, respectively). Conclusions: Providing feedback to health professionals on polypharmacy survey results may lead to a decrease in the number of polypharmacy patients. Factors contributing to polypharmacy included age (75 years or older), the number of medical institutions (2 or more institutions), and patients' difficulty with asking their physicians to deprescribe their medications. Feedback to health professionals reduced the percentage of polypharmacy patients and the number of prescribed medications.

A field study was conducted in Northern Bangladesh, Rangpur district to observe the impact of tobacco cultivation on the environment as well as to characterize contributing factors motivating tobacco cultivation. Information was collected from the tobacco cultivators by a questionnaire survey that was conducted in four mostly tobacco cultivated upazilas, namely Badargonj, Taragonj, Rangpur Sadar, Gangachara of Rangpur district. Information from non-tobacco cultivators was also collected from Mithapukur Upazila. Survey findings revealed that 62.16% of respondents believed that soil productivity is decreasing in the tobacco field and 70.27% of tobacco farmers were using more chemical fertilizer per year than the previous years. More than half (54.05%) of respondents reported that tobacco is replacing native food crops. Moreover, 51.30% of tobacco farmers were facing various health hazards due to tobacco cultivation. The present study also investigated that 97.14% of farmers got a high benefit along with 89.19% of respondent’s availed organizational support from tobacco marketing companies. The overall satisfaction rate with the price was 80.56%, which was the main contributory factor for tobacco cultivation in the study area. The survey results also identified that farmers from Mithapukur Upazila gave up tobacco cultivation due to health hazards and unnecessarily underestimating the grade of tobacco leaf by the buyer company. Performing a t-test and a chi-square test on medical fee receipts, we analyzed changes in the percentage of polypharmacy patients and the number of prescribed medications during one month before and after the feedback. Results: In the questionnaire survey, significant differences were found in the following 3 items: age (odds ratio (OR)=3.14; 95% confidence interval (CI)=2.01-4.91), number of medical institutions (OR=2.34; 95%CI=1.50-3.64), and patients' difficulty with asking their doctors to deprescribe their medications (OR=2.21; 95%CI=1.25-3.90).
