Objective: The aim of this study was to compare the clinical and radiographic success rates of pulpotomy in primary molars using formocresol versus sodium hypochlorite.Methods:  Twenty-three children aged 4-9 years with at least two primary molars requiring pulpotomy were randomly allocated into two groups. All teeth received stainless steel crown after conventional pulpotomy procedure with either NaOCl or formocresol. Clinical and radiographic signs/symptoms were recorded at six and 12 months. Outcomes were statistically analyzed using Fisher’s exact test and Chi-squaretest.Results: Clinical success rates at 6 and 12-month follow-up in both groups was 100%. At 6-month follow-up, radiographic success rate for NaOCl and formocresol groups was 100%. At 12-month recalls, in NaOCl group, 20 teeth (87%) and in formocresol group, 21 teeth (91.3%) had radiographic success. No significant difference was found in the radiographic success rates at 12 months (P=1.00). Internal root resorption was the most common radiographic pathologic finding in both groups.Conclusion:Clinical and radiographic success rates in NaOCl group was comparable with formocresol group, so NaOCL can be suggested as an alternative for primary teeth pulpotomies. However further clinical studies with long-term follow-ups are needed.

Seventy primary molar teeth, carious exposed, symptom free, without any sign of root resorption in children aged from 3 to 6 years (main age 4.3yr) were treated with conventional pulpotomy procedures. Ferric sulfate 15.5% solution (applied for 15 second for 35 teeth) and formocresol solution (five minute procedure of Buckley's formula for next 35 teeth) have been used as pulpotomy agents. In both groups, pulp stumps were covered with zinc-oxide eugenol paste. Permanent restorations were stainless steel crowns. Clinical check up was every three-months and radiographic follow-up time was six and twenty months after treatment. Our results within this period revealed 100% clinical success rate in both groups. Radiographic success rate was in both groups 97.2%, while in 2.8% cases has shown internal root resorption. On the basis of these results, we can recommend ferric sulfate as a pulpotomy agent in primary teeth in substitution for formocresol at the moment.

ABSTRACT Objective: The aim of this study is to evaluate four different pulpotomy medicaments in primary molars. Materials and Methods: A total of 147 primary molars with deep caries were treated with four different pulpotomy medicaments (FC: formocresol, FS: ferric sulfate, CH: calcium hydroxide, and MTA: mineral trioxide aggregate) in this study. The criteria for tooth selection for inclusion were no clinical and radiographic evidence of pulp pathology. During 30 months of follow-up at 6-month intervals, clinical and radiographic success and failures were recorded. The differences between the groups were statistically analyzed using the Chi-square test and Kaplan-Meier analysis. Results: At 30 months, clinical success rates were 100%, 95.2%, 96.4%, and 85% in the FC, FS, MTA, and CH groups, respectively. In radiographic analysis, the MTA group had the highest (96.4%), and the CH group had the lowest success rate (85%). There were no clinical and radiographic differences between materials (P > 0.05). Conclusions: Although there were no differences between materials, only in the CH group did three teeth require extraction due to further clinical symptoms of radiographic failures during the 30-month follow-up period. None of the failed teeth in the other groups required extraction during the 30-month follow-up period.

ABSTRACT Introduction Formocresol has been a popular pulpotomy medicament for many years. It is considered the “gold standard” in pediatric dentistry. However, concerns have been raised over its use in children. It has been reported that formocresol has toxic and mutagenic effects in cell culture, dental crypts, and precancerous epithelial cells. Therefore, additional biocompatible treatment alternatives are required to replace formocresol pulpotomy. Aims This study compared the clinical and radiographic success rates of ferric sulfate (FS) and bioactive tricalcium silicate cement (Biodentine, Septodont) as pulpotomy agents in primary molar teeth over a period of 9 months. Materials and methods Fifty primary molar teeth, symptom free, requiring pulpotomy in children aged 4 to 8 years were treated with conventional pulpotomy procedures. Ferric sulfate 15.5% solution (applied for 15 second for 25 teeth) and Biodentine (for 25 teeth) were used as pulpotomy agents. Permanent restorations were stainless steel crowns in most cases, in both groups. Patients were recalled for follow-up at 1, 3, 6, and 9 months intervals. The data were statistically analysed using chi-square test. Results At 9 months, 96% clinical success rate was observed in the FS and 100% in the Biodentine group. Radiographic success rate in the FS group was 84%, whereas 92% in the Biodentine group at 9 months. No statistically significant difference was found between the two groups. Objective: This study was conducted to compare the efficacy of 3Mixtatin (a combination of simvastatin and 3Mix antibiotic) with MTA and Formocresol for the pulpotomy of primary molars. Study design: 114 children aged 3–6 years old with 150 primary molars were randomly allocated to three groups. MTA, Formocresol or 3Mixtatin were used for Pulpotomies. Hard setting zinc oxide eugenol was used to cover these materials. The teeth were restored with amalgam. Blinded radiographic and clinical examinations were conducted at 6, 12 and 24 months after treatment for the presence of pain, tenderness to palpation and percussion, sinus tract, swelling, presence of internal or external root resorption, inter-radicular radiolucency, and periapical lesion. Results: 122 teeth were available for 24-month follow-up study. The overall success rate was 78.9% for FC, 90.5% for 3Mixtatin and 88.1% for MTA group. There was no significant difference in overall success rate among the groups after 24-month follow-up (X2=2.43, df = 2, P =0.27). Conclusion: Our findings demonstrated remarkable results of 3Mixtatin in pulpotomy of primary teeth at the 24-month follow-up. Therefore, 3Mixtatin may be considered as an effective material in pulpotomy of primary teeth because of its successful results.

Aim: The aim of this study was to undertake a comprehensive quantitative investigation of pulpal blood flow (PBF) changes in human non-carious primary molar teeth with variable degrees of root resorption by Laser Doppler Flowmetry (LDF) and Pulse Oximetry (PO) methods. Materials and Methods: Data was collected from clinically and radiographically healthy 86 mandibular primary molars which have different physiological root resorption levels (PRRLs). PRRLs for each of the teeth were assessed using periapical radiographs and teeth were subdivided into three groups. Results: The LDF values demonstrated a significant difference (p = 0.0001) between all groups although PO did not demonstrate any difference (p = 0.109). Statistical analysis of LDF values demonstrated significant differences between Groups A and C (p = 0.0001) and Groups B and C (p = 0.008). Furthermore, positive correlations were determined between LDF values and PRRL groups (p = 0.0001) and patients' ages (p = 0.0001). Conclusions: In our study, it was observed that the PBF values of human primary molars measured by LDF tended to increase with the progress of physiological root resorption and age. LDF was found to be a more effective method than PO to assess the pulpal vascularity changes of human primary molars.

ABSTRACTObjectives: The aim of this study was to evaluate the clinical and radiographic success rates of pulpotomized primary molar teeth restored with a compomer material with using United States Public Health Service (USPHS) criteria. Methods: In 173 primary molars of 156 child patients, aged within 4-9 years (mean age: 6.1±1.4 years), conventional pulpotomy treatment were performed. The teeth treated using calcium hydroxide, formocresol or ferric sulphate. After pulpotomy procedure, teeth were restored with compomer material. The teeth were evaluated as clinically and radiographically during a period of 12-24 months. Both of success of pulpotomy treatment and also restorative material (compomer material) were evaluated during follow-up period. The data were assessed with chi-square test. Results: At the end of the first year, 45% of initial treated teeth were checked, but only 18% were checked at the end of the second year. The first year success rates in the groups treated with CH, FC, and FS were 87.5%, 95%, and 79%, respectively, and, as the number of controllable patients was lower, the success rates on available teeth were determined to be 88.3% and 80%, respectively, according to the materials at the end of the second year. Restorations having been made, they were analyzed in accordance with USPHS criteria. Conclusions: At the end of the first year, 67.5% of compomer restorations were detected to be original and healthy and at the end of the second year, 57% were deemed healthy. No statistically significant relationships were found between marginal adaptation, secondary caries and pulpotomy success (chi-square test, P>.05). Among the three groups, there is no significant difference in terms of success. The least successful age group was defined as 4-6 years. (Eur J Dent 2011;5:415-422)

Purpose: The aim of this study was to compare the clinical and radiographic success rates of electrosurgical pulpotomy of human primary molars with zinc oxide eugenol (ZOE) and zinc polycarboxylate (ZPC) cements. Methods: In this randomized clinical trial study, 120 primary second molar teeth were treated by electrosurgical pulpotomy. Teeth were randomly assigned to two groups according to whether ZOE or ZPC cement was used as a sub-base. Teeth were restored with stainless steel crowns and were evaluated clinically and radiographically after 3, 6, and 12 months by two independent examiners. Clinical treatment outcomes and radiographic findings were statistically analyzed using Fishers' exact test with statistically significant differences defined for P < 0.05. Results: At 12 months, the clinical and radiographic success rates in the ZOE group were 98.2% and 84.2% and in the ZPC group were 96.2% and 75%, respectively (P ≯ 0.05 for all). Conclusions: The outcomes of this study suggested that either ZPC or ZOE sub-base have similar clinical and radiographic success in electrosurgical pulpotomy.

Aim: The purpose of this study was to evaluate the clinical success of three fissure sealants(FSs) with different contents on primary teeth. Study design: Three FSs were used to seal 150 primary molars in 75 children aged 4–7 years. All FSs were placed on occlusal surfaces in a split-mouth and randomized clinical trial. For patients in Group1,amorphous calcium phosphate(ACP) containing resin-based sealant(RBS)(Aegis) was applied to a primary molar tooth on one side ,and non-fluoride RBS(Helioseal) FS was applied to symmetrical molar tooth. For patients in Group2, fluoride-containing RBS(Helioseal F) was applied to a primary molar tooth on one side, and Helioseal FS was applied to symmetrical molar tooth. For patients in Group3,Helioseal FS was applied to a primary molar tooth on one side, and Aegis FS was applied to symmetrical molar tooth. Clinical evaluation of FSs was carried out to assess retention, marginal discoloration, marginal adaptation,and the presence of caries in months 1,3,6,12,18 and 24 after FS application. Results: There were no significant differences for all criteria in groups 2 and 3(p≯0.05). In group 1,cumulative success rates according to 24 months’ follow-up were statistically insignificant during the comparisons performed in terms of retention, marginal adaptation, and presence of caries(p≯0.05). Marginal discoloration was found to be statistically significant(p<0.05). Conclusion: RBS containing ACP or fluoride may be more effective than conventional RBS for caries prevention.

Objectives: Clinical and radiographic evaluation of the premedicated direct pulp capping using formocresol(PDC) versus conventional direct pulp capping using calcium hydroxide (CDC) in human carious primary molars. Study design: A total of 120 vital primary molars with pinpoint exposure during caries removal in 84 patients aged 4-5 years were selected. In the PDC group (n = 60), 20% Buckley's formocresol solution, and in the CDC group (n = 60), calcium hydroxide powder were applied to the exposure sites followed by placement of zinc oxide-eugenol base. Teeth were restored with preformed stainless steel crowns. Clinical and radiographic evaluations of the treatment outcomes were performed at regular intervals of 6 and 12 months, respectively, for two years post-operatively. Results: The prevalence of spontaneous pain,sensitivity on percussion, and fistula were significantly higher in the CDC group compared to the PDC group (P < 0.05). The number of teeth exhibiting periapical/furcal radiolucency or external/internal root resorption was also higher in the CDC group (P < 0.05). The clinical success rate of the PDC was 90% compared to the 61.7% of the CDC (P < 0.05). The radiographic success rates of the PDC and CDC groups were 85% and 53.3%, respectively (P < 0.05). Conclusion: It seems formocresol premedicated direct pulp capping could safely be used as a substitute for conventional direct pulp capping.

AIM: The aim of this study is to present severe root resorption following obturation of the primary molar with zinc oxide eugenol (ZOE) at different follow-up periods. CASE REPORT: We present the case of a 6-year-old boy with decayed lower primary molars. Pulpectomy for pulpally involved primary molars was performed. ZOE has been used as root canal filling material and follow-up was done at 3, 6, and 9 months. CONCLUSION: External root resorption was detected at 3 months and the resorptive process continued till 9 months. This finding emphasizes the need for alternative filling material with less irritating properties.
