Enteric fever involves reticuloendothelial system. Most common complications are intestinal perforation, hepatitis, encephalopathy and bleeding ileal ulcers. Apart from known complications we present an interesting case of middle aged female with uncontrolled diabetes presenting with empyema of gall bladder secondary to Salmonella typhi. Both blood and gallbladder tissue bacteriological cultures had Salmonella typhi confirming our diagnosis. It was noteworthy that empyema was found intraoperatively. Patient recovered after cholecystectomy and antibiotics. Considering this rare complication we advise Typhoid conjugate vaccine in tropical countries. This case report paves way to recognize this complication to watch for. Key words: Enteric fever, Empyema, complications, Typhoid vaccine, gall bladder.

Ectopic gall bladder is a rare entity. It can often be misdiagnosed causing diagnostic dilemma leading to various complications. Ultrasonography is the most common investigation for evaluating gall bladder pathologies. However, the confirmation of ectopic gallbladder is not easily possible by this method. On the other hand, hepatobiliary scan (HIDA scan) plays an important role in evaluating the presence and position of ectopic gallbladder. Here we present a case of sonographically suspected ectopic gallbladder confirmed by hepatobiliary scan highlighting the importance of HIDA scan in ectopic gallbladder.Bangladesh J. Salmonella typhiusually produces enteric fever and gastroenteritis. The infection may spread through blood stream and present as local suppurative lesions which may involve any site including the bone and joints. We report a case of septic arthritis of hip in a patient with systemic lupus erthematosis. The case is presented for its rarity and to highlight the atypical manifestations ofSalmonella typhiin endemic regions.

Intestinal perforation is a rare complication of induced abortion but it is not rare in our country. It is most commonly seen in countries in which abortions are performed by people without proper training and with sharp pointed instruments without knowledge of anatomy. Bowel perforation occurs when the posterior vaginal wall or uterus is violated, allowing the instrument to pierce underlying structures. The ileum and sigmoid colon are the most commonly injured portions of bowel due to their anatomic location. Bilateral proliferative retinopathy is a rare complication of chronic myeloid leukemia (CML) as a few case reports have been published to date. In this case report, a 32-year old diabetic female presented with history of bilaterally decreased vision. Ophthalmologic examination showed bilateral proliferative retinopathy (i.e., retinal detachment, vitreous hemorrhage, pre-retinal fibrosis and the presence of bilateral peripheral capillary dropout with multiple retinal sea fan neovascularization) for which panretinal laser photocoagulation and vitrectomy were planned. During the preoperative workup, complete blood count revealed hyperleukocytosis. Later on, the karyotype analysis identified Philadelphia chromosome, confirming the diagnosis of CML. Hence, it was an interesting case where bilateral proliferative retinopathy directed to the diagnosis of CML. Therefore, proliferative retinopathy may be the first presentation of CML.

Salmonella Typhi is a causative agent of enteric fever, which is an ongoing public health problem in urban areas of the Kathmandu valley in Nepal. It is estimated that approximately 5 % of people in an endemic enteric fever area may asymptomatically carry Salmonella Typhi in the gall bladder. Here we describe a rare clinical manifestation of chronic Salmonella carriage. A 40 year old male resident of Kathmandu underwent an emergency cholecystectomy for acute cholecystitis and gall bladder perforation. No stone was identified but Salmonella Typhi was cultured from the gall bladder. Key Words: Acalculous cholecystitis, perforation, Salmonella Typhi. Typhoid fever is an endemic infection in developing countries like India. Oral manifestations of enteric fever and typhoid tonsillitis have rarely been reported in the literature.   We report a 20 year old male, showing tonsillitis to be an integral part of clinical presentation of typhoid fever.The patient presented with fever, throat pain and oral ulcers. Blood culture and swab from tonsils showed significant growth of Salmonella typhi.This case report adds light to tonsillitis as a rare clinical manifestation of typhoid fever.

Commonly seen in the emergency department, diabetic ketoacidosis is a potentially lethal sequela of uncontrolled diabetes mellitus. In the adult population, a rare complication of diabetic ketoacidosis is cerebral edema. This case report discusses a 26-year-old male with new onset diabetes mellitus who developed cerebral edema leading to death.

Enteric fever is a systemic disease characterised predominantly by fever and abdominal pain, caused by dissemination of Salmonella Typhi or Salmonella Paratyphi. Enteric fever can affect many organ systems including liver, gastrointestinal tract, kidney and brain. Neurological manifestations occur in 2%–40% of patients and include meningitis, Gullian-Barré syndrome, neuritis and neuro-psychiatric symptoms. Spastic quadriparesis is a rare complication.
