Long-term clinical results of the Metasul metal-on-metal total hip arthroplasty: 12.6 years follow-up of 128 primary total hip replacements

Introduction: The purpose of the present study is to report the long-term clinical results of an uncemented total hip arthroplasty (THA) using a Metasul metal-on-metal (MoM) 28-mm bearing and to evaluate the long-term serum cobalt levels. Methods: At an average of 12.6 years following primary THA, we retrospectively reviewed the clinical results of the first 116 consecutive patients (128 THAs) in our institution who underwent 28-mm Metasul MoM THA. Of the 78 patients who were able to visit our outpatient clinic, serum cobalt levels were evaluated. Results: The overall survival rate of the cohort was 96.1% (95% confidence interval [CI], 93.2-99.6), 12.6 years (95% CI, 12.3-12.7 years) following surgery. 3 patients had undergone revision due to aseptic loosening of the stem and 2 patients sustained a periprosthetic fracture. The average modified Harris Hip Score was 90 (72-97) and the average Oxford Hip Score was 56 (48-60), representing both excellent outcome scores. The average serum cobalt of the entire cohort was 20.1 nmol/L (range 8.5-227.7 nmol/L). Serum cobalt levels of patients with a bilateral MoM THA were significantly higher (35.0 nmol/l, p<0.01). No relation between serum cobalt levels, subjective outcome, radiolucent lines on radiographs and survivorship of the implant was noted. Conclusions: Long-term results of the metasul MoM bearing articulation in THA seem to be excellent, although cobalt serum levels should be monitored closely.

Abstract Purrpose Metal-on-metal (MoM) total hip arthroplasty (THA) has been a subject of recent discussion and concern due to the early failures caused by local and systemic adverse reactions related to specific designs. The aim of this study is to analyze the outcomes and survival rates of a single brand of MoM implants implanted in a consecutive series of patients at a single institution. Methods Between 2007 and 2012, 116 (118 hips) patients were evaluated at a mean follow-up of 6.6 years after primary THA. The diagnosis leading to surgery was osteoarthritis (80 patients) and proximal femoral fracture (36 patients). A single design of THA was implanted. All patients were evaluated before surgery and postoperatively at 1, 3, 6, and 12 months by clinical scores and radiographic studies. The data analysis was made using Student's t-test. Results The minimum follow-up was of 4 years, with a mean follow-up of 6.6 years. Two aseptic loosenings of the acetabular component were recorded (one per group), which were not associated with local or systemic complications related to metal ion release. Both were revised by an isolated acetabular cup substitution with metal-on-polyethylene couplings. Nonprogressive radiolucency lines < 2 mm in zone 2 were observed in other six patients around the acetabular component without clinical manifestation (four in the arthritis group and two in the fracture group). Postoperative Harris Hip Score and SF-36 (36-Item Short Form Survey) score improved in both groups. Conclusion Despite several MoM implants showing early complications and failures, a specific MoM design may be associated with good clinical results at a mid- to long-term follow-up. Objective: In this study we aimed to investigate the long-term clinical and radiological results, revision rates and causes, and the rate of implant survival in total hip arthroplasty performed using CLS® expansion cup and Spotorno® cementless femoral stem. Methods: Clinical results of total hip arthroplasty performed on 131 hips of 114 patients in Istanbul Training and Research Hospital between 1993 and 2003 were retrospectively evaluated according to the Harris Hip Score. Revision rates were determined and implant survival rates were identified using the Kaplan-Meier estimator. Results: Of the patients, 39 were males and 75 were females. The average age of the patients at surgery was 48.7±11.3 years. Patients were followed up for a mean period of 13.9±2.4 years. The mean Harris Hip Score was 34.35±6.09 preoperatively and 88.20±7.11 at the final follow-up (p<0.001). The Kaplan-Meier survivorship estimate for the cup at 13.9 years, taking revision for any reason as the end point was 95.6% (95% CI), while the 15th and 17th year survival rates were 90% and 85%, respectively. Conclusion: In total hip arthroplasty using a cementless expansive acetabular cup, a 95.6% survival rate is achieved after an average of 14 years, whereas the rate decreases to 85% after 17 years. Abstract Background A high rate of complications due to metal-on-metal (MoM) revisions has been relatively commonly documented. The purpose of this retrospective study was to compare the long-term outcomes of patients who had undergone uncemented or cemented total hip arthroplasty (THA) revision for prior primary MoM THA failure.Methods Data from 234 patients (234 hips) who underwent uncemented or cemented THA (UTHA or CTHA) for prior primary MoM THA failure during 2007 - 2018 were retrospectively analysed. Follow-up occurred 3 months, 6 months, 1 year, 2 years, and then every 1 year after conversion. The mean follow-up time was 84.15 months (range, 67 - 101 months). The primary endpoint was the modified Harris Hip Score (HHS). The secondary endpoint was the major orthopaedic complication rate.Results The HHS demonstrated statistically greater differences in the CTHA group than in the UTHA group 12 months after conversion. From the 12th month after conversion to the final follow-up, CTHA yielded better functional outcomes than UTHA. There were significant differences in the rates of re-revision, aseptic loosening, and periprosthetic fracture between the groups (10.3% for UTHA vs 2.5% for CTHA, p = 0.015; 16.3% for UTHA vs 5.9% for CTHA, p = 0.011; and 12.0% for UTHA vs 4.2% for CTHA, p = 0.045, respectively).Conclusion In the setting of revision for primary MoM THA failure, we found definite evidence of the superiority of CTHA over UTHA in terms of improving functional outcomes and decreasing the major orthopaedic complication rate.

Abstract Background: Congenital pubic diastasis in combination with a dysplastic situation of the hip is a rather rare malformation. Two cases of young female patients prompted us to report the results of total hip arthroplasty (THA) due to secondary osteoarthritis. Our treatment strategy and short- as well in one case long-term results are described in regard to the rather rare literature. Methods: We describe a case of a 39 -year-old female patient with a pubic diastase and severe hip dysplasia on both sides treated with a primary total hip arthroplasty (THA). High overweight of more than 275 pounds, diabetes mellitus and multiple prior operation aggravate the operative procedure. Main problem based on the fixation strategy of the cup in this retroverted acetabulum presented in CT-analysis with elevated rotation center due to the dysplastic situation. In the other case a 52-year old female lady presented dysplastic osteoarthritis of the left hip. Results: In the first case both hips were treated with a cementless osteoconductive cup (TM, Zimmer-Biomet) and a cementless stem (Alloclassic SL, Zimmer-Biomet). A 10° elevated rim liner of the cup was used in order to avoid dislocation. Both hips were 9 and 8.5 years postoperative radiologically still in place without any signs of loosening. There was an improvement of quality of life documentated with an amelioration of the Harris Hip Score from preoperative 14 points to 68 points. In the second case a conventional hemispherical cup (Alloclassic-Allofit, Zimmer-Biomet) was placed in the retroverted acetabulum combined with a cementless stem (Fitmore A, Zimmer-Biomet) attached at the metaphseal proximal femur bone. 6 months postoperative, the patient was free of symptoms with an increased quality of life. Conclusion: Sufficient long-term results highlight the possiblity of total hip arthroplasty of patients with osteoarthritis even in congenital pubic diastasis. Retroversion of the dysplastic acetabulum, rotation of the posterior part of the pelvis and instability of the pelvic ring in these circumstances might be the most feared aspects in the treatment of these rare cases.

Aims Second-generation metal-on-metal (MoM) articulations in total hip arthroplasty (THA) were introduced in order to reduce wear-related complications. The current study reports on the serum cobalt levels and the clinical outcome at a minimum of 20 years following THA with a MoM (Metasul) or a ceramic-on-polyethylene (CoP) bearing. Methods The present study provides an update of a previously published prospective randomized controlled study, evaluating the serum cobalt levels of a consecutive cohort of 100 patients following THA with a MoM or a CoP articulation. A total of 31 patients were available for clinical and radiological follow-up examination. After exclusion of 11 patients because of other cobalt-containing implants, 20 patients (MoM (n = 11); CoP (n = 9)) with a mean age of 69 years (42 to 97) were analyzed. Serum cobalt levels were compared to serum cobalt levels five years out of surgery. Results The median cobalt concentration in the MoM group was 1.04 μg/l (interquartile range (IQR) 0.64 to 1.70) at a mean of 21 years (20 to 24) postoperatively and these values were similar (p = 0.799) to cobalt levels at five years. In the CoP control group, the median cobalt levels were below the detection limit (< 0.3 μg/l; median 0.15 μg/l, IQR 0.15 to 0.75) at 20 years. The mean Harris Hip Score was 91.4 points (61 to 100) in the MoM group and 92.8 points (63 to 100) in the CoP group. Conclusion This study represents the longest follow-up series evaluating the serum cobalt levels after 28 mm head MoM bearing THA and shows that serum cobalt concentrations remain at low levels at a mean of 21 years (20 to 24) after implantation. Traditionally, Charcot arthropathy has been considered an absolute contraindication for total hip arthroplasty (THA). However, some recent reports have shown that good short- to mid-term results can be achieved by improving the durability of the implant. This paper reports the mid- to long-term results of THA in two patients with Charcot hip joints caused by congenital insensivity to pain with anhydrosis. Both patients suffered multiple posterior dislocations in the six months immediately following surgery. However, with the continuous use of a hard abduction brace, one patient was eventually able to walk with a lofstrand cane and the other with the use of one crutch. Although one patient experienced a dislocation five years after surgery, X-rays taken after nine years and five years, respectively, revealed no clinical signs of implant loosening. We conclude that, with careful planning and appropriate precautions, THA may be a viable treatment option for Charcot hip joints caused by congenital insensivity to pain with anhydrosis.
