Background: Direct Pulp Capping technique has been in use as a treatment therapy to sustain a vital pulp. The substitute procedure to the endodontic therapy or extraction is precise & successful pulp capping. Calcium hydroxide (CaOH) is considered as gold standard agent for pulp capping because of antibacterial characteristics & ability to form dentinal bridge. Calcium hydroxide has also shown great results on peri-apical healing process. Aim: To evaluate success rate of direct pulp capping when CaOH is used as a direct pulp capping agent Setting: Multan Dental College Multan Methodology: Sample size 60 cases. Patients of 15-40 years age group were selected with the exposure of the vital pulp by caries or Trauma in permanent lower molars clinically. Those cases with pinpoint (precise) exposure of pulp were selected. Flow of blood from site of exposure should be ceased inside two to three minutes after the applying the soaking cotton pallet with the saline. Results: 25 were woman (41.67%) and 35 were men (58.33%). Overall percentage of success is 80% which means that 48 patients had showed effectiveness, while twelve patients had shown failure of pulp capping Conclusion: We conclude that Calcium hydroxide is a choice for the direct pulpal capping. When a site of exposure is sealed/ wrapped by means of calcium hydroxide, the odds of the failure may decrease, and prognosis might be good. The aim: The research was to evaluate the effectiveness of acute traumatic pulpitis treatment with the use of bioceramics according to the results of a clinical study. Materials and methods: Acute traumatic pulpitis of 25 teeth in 25 patients aged 18-25 years were treated by biological direct pulp capping. In 13 patients of the main group, bioceramics was placed on the exposed pulp, and in 12 patients of the control group, calcium hydroxide paste was applied on the exposed pulp. Treatment effectiveness was evaluated according to the following criteria: the absence of clinical signs of pulpal inflammation, on X-ray – signs of dentinal bridge formation, the absence of periapical changes. Results: The use of bioceramic material in patients of the main group provided a positive dynamic of treatment in 12 teeth. The need for re-treatment was diagnosed only in 1 clinical case (7.7%), in the control group – in 4 cases (33.3%). Conclusions: Pulpitis treatment with the use of bioceramics provides preserving the functional properties of pulp, creation of a calcified barrier – dentin bridge and prevents the periodontal complications compared to the pulp capping with a material based on calcium hydroxide. The effectiveness of treatment in the main group was 92.3%, in the control – 66.7%.

Summary There are two considerations for direct pulp capping - accidental mechanical pulp exposure and exposure caused by caries. Mineral trioxide aggregate (MTA) was used as pulp-capping material to preserve the vitality of the pulpal tissues. Follow-up examinations revealed that treatment was successful in preserving pulpal vitality and continued development of the tooth. On the basis of available information, it appears that MTA is the material of choice for some clinical applications. Material and methods: Cases 18 - 8 teeth with grey MTA, 10 teeth with white MTA; diagnose: Pulpitis chronica ulcerosa, Electro pulpal test (EOD) - 30-35 μA, pre-clinical X-ray - without changes in the structures, follow ups for 4 years. Successful treatments: without clinical symptoms and changes in the X-rays: 5 teeth with grey MTA, 8 teeth with white MTA for period of 4 years. Unsuccessful treatments: Clinical symptoms and sometimes changes in the X-ray: 3 with grey MTA, 2 with white MTA. MTA is an appropriate material for pulp-capping and follow-up examinations revealed that the treatment was successful in preserving pulpal vitality.

The objective of this study was to measure temperature changes in the pulp chamber induced by polymerization of resin-based dental restoratives following a simulated procedure of direct pulp capping. Class I cavities with a microperforation at the pulp horn were prepared in extracted human molar teeth. The complete procedure of direct pulp capping and cavity restoration was performed with the root part of extracted teeth fixed in a water bath at 37 ?C. Mineral trioxide aggregate, bioactive dentin substitute or calcium-hydroxide paste were used as pulp capping materials. Cavities were restored with a light-cured or chemically-cured resin-modified glass ionomer, universal adhesive and a bulk-fill composite, cured with a high-intensity LED unit. Pulp capping materials caused a slight temperature decrease. Lower temperature increase was recorded during light-curing of the glass ionomer liner after direct capping with mineral trioxide aggregate and calcium-hydroxide than that recorded for the bioactive dentin substitute. Adhesive light-curing increased temperature in all groups with higher mean temperatures in groups with chemically-cured as compared to those for the light-cured glass ionomer liner. Direct pulp capping with mineral trioxide aggregate or calcium-hydroxide followed by the light-cured resin-modified glass ionomer liner and a bonded bulk-fill composite restoration induced temperature changes below the potentially adverse threshold of 42.5?C.

Summary Introduction Direct pulp capping is an important therapeutic method that has goal to provide formation of dentin bridge and healing process of the pulp. The aim of this study was to investigate the effects of new nanostructural materials based on calcium silicate systems and hydroxyapatite on exposed dental pulp in Vietnamese pigs. Material and Methods The study was conducted on 30 teeth of two Vietnamese pigs (Sus scrofa verus). On buccal surfaces of incisors, canines and first premolars, class V cavities were prepared with a small round bur and pulp horn was exposed. In the first experimental group (10 teeth) the perforation was covered with new nanostructural material based on calcium silicate systems (CS). In the second experimental group, the perforation was covered with compound of calcium silicate systems and hydroxyapatite (HA-CS) (10 teeth). In the control group, exposed pulp was covered with Pro Root MTA® (Dentsply Tulsa Dental, Johnson City, TN, USA) (10 teeth). All cavities were restored with glass ionomer cement (GC Fuji VIII, GC Corporation, Tokyo, Japan). Observation period was 28 days. After sacrificing the animals, histological preparations were done to analyze the presence of dentin bridge, inflammatory reaction of the pulp, pulp tissue reorganization and the presence of bacteria. Results Dentin bridge was observed in all teeth (experimental and control groups). Inflammation of the pulp was mild to moderate in all groups. Neoangiogenesis and many odontoblast like cells responsible for dentin bridge formation were detected. Necrosis was not observed in any case, neither the presence of Gram-positive bacteria in the pulp. Conclusion Histological analysis indicated favorable therapeutic effects of new nanostructural materials based on calcium silicate systems and hydroxyapatite for direct pulp capping in teeth of Vietnamese pigs.
