BAAI is a rare but challenging traumatic lesion. Since BAAI is difficult to suspect and diagnose, frequently lethal and associated to multiorgan injuries, its management is objective of research and discussion. REBOA is an accepted practice in ruptured abdominal aortic aneurysm. Conversely, blunt aortic injuries are the currently most cited contraindications for the use of REBOA in trauma, together with thoracic lesions. We reported a case of BAAI safely managed in our Trauma Center at Maggiore Hospital in Bologna (Italy) utilizing REBOA as a bridge to endovascular repair, since there were no imminent indications for laparotomy. Despite formal contraindication to placing REBOA in aortic rupture, we hypothesized that this approach could be feasible and relatively safe when introduced in a resuscitative damage control protocol.

The aim of the study was to evaluate the role of virtual bronchoscopy (VB) in traumatic rupture of the main bronchus Material and methods. Тhe data of virtual bronchoscopy of multispiral computed tomography (MSCT) with multiplanar and 3D reconstructions of 10 patients with traumatic separation of the main bronchus were analyzed. MSCT was carried out by the computer tomograph AquilionONE (320-slice) according to the previously described technique. Results of the study. VB MSCT allowed to determine the presence of a complete or partial rupture of the main bronchus, its distance to the bifurcation of the trachea, the state of the collapsed lung, the presence of fluid in the hemithorax, secondary changes in the bone structures of the chest. The World Bank played an important role in monitoring the adequacy of reconstructive measures on the damaged bronchus, excluding the occurrence of postoperative stenosis. The conclusion. Virtual bronchoscopy of multispiral computed tomography with the capabilities of multiplanar and volumetric reconstructions, postprocessing image processing is an optimal non-invasive method for determining the traumatic lesion of the main bronchi and monitoring the success of the reconstructive surgical manual.

Pyoinflammatory complications remain an acute problem in the post-operative period of traumatic lesions of the auxiliary apparatus of the eye (AAE). Silver both in the ionic form and in composition of chemical compounds is highly toxic for microorganisms, and as a result, it shows bactericidal effect to many bacterial strains, including gram-negative microorganisms. The peculiarity of AgNPs is efficiency of influence on the wide array of microorganisms, significant anti-biofilm effect and absence of resistance reaction. The aim of the research. To study the influence of the colloidal nano silver on formation of biofilms by microorganisms discharged from the wounds of patients with traumatic lesions of the auxiliary apparatus of the eye. Materials and methods. During 2018-2019, we examined 60 patients with traumatic lesions of the auxiliary apparatus of the eye. For evaluation of the influence of colloid nano silver solution on the processes of formation of the biofilm, we selected microorganisms which were cultured most frequently (Staphylococcus aureus, Acinetobacter spp., Klebsiella ozenae) from the patients. Results. The obtained data suggest that colloid nano silver inhibits efficiently formation of biofilms at the early stages (initiation, the 0 day of incubation) of their formation by all the three microorganisms, and the degree of inhibition of the biofilm formation did not depend on the silver concentration. The effect of colloid silver in the concentrations used by us at later stages of biofilm formation (the 3rd and the 7th day) with respect to К. ozenae is less efficient – the growth of cell biomass was observed (p≤0.05), and it did not depend on the silver concentration. At the same time, the effect of the colloid nano silver on S. aureus and Acinetobacter spp. on the 3rd and the 7th days was more efficient than at the early stage (p≤0.05). Conclusions. Nanoparticles of colloid silver are an efficient means to combat biofilms, as well as to prevent their formation.

Background Congenital abnormalities of the triangular fibrocartilage complex (TFCC) are rare and could be mistaken for a traumatic lesion. It is important to recognize these anatomical variations and to realize they do not always require treatment. Case Description An incidental finding of an atraumatic bucket handle abnormality of the TFCC in a 15-year-old male, who was treated arthroscopically for dorsal wrist pain. This structure was resected, thus obtaining a normal looking peripheral TFCC. Literature Review Literature regarding congenital abnormalities of the TFCC is limited to a meniscoid articular disc or a congenital perforation. To our knowledge, an atraumatic bucket handle abnormality has not been described yet. Clinical Relevance This congenital abnormality of the TFCC could be mistaken for a traumatic lesion on MRI, or during wrist arthroscopy; therefore, it is important to realize that this entity may occur and does not require treatment.

The study of ancient skeletons is of high interest, as the relationship between men and animals can be reconstructed. In dogs, head injuries frequently result from direct physical traumas. The excavation of a prehistoric well (Genomi, Sardinia) brought to light sixty canine bones of the Age Roman (2nd century AD), although this well was built during the Nuragic Age. (1st millennium BC). Two canine skulls showing three traumatic lesions underwent computed tomography in order to study the endocast of the brain cavity and indirectly locate possible brain lesions. In the first case, a traumatic lesion was found in the left parietal bone with depression of the outer surface. This lesion determined compression of the left frontal cortex and was compatible with life, as can be deduced by the inflammation of the periostenum. In the second case, two different traumatic lesions were found. The former was a wide fracture of the right frontal bone near the orbital cavity. The injured area showed clear signs of bone remodeling which took place many months before death. The latter was a severe comminuted fracture involving both the outer and inner surface of the left frontal bone, which provoked a deep wound in the left frontal cortex. The sharp outline of bone splinters with no signs of bone remodelling indicates that the trauma was not compatible with life. All lesions described were consequent to severe blows from humans and testify mistreatment of dogs in the Roman Age.

Abstract OBJECTIVE To analyze retrospectively the outcomes of primary as well as secondary functional reconstructions in 49 patients with traumatic brachial plexus lesions from a single service. Guidelines for treatment might be extracted from this analysis. METHODS Among 152 cases of traumatic lesion of the brachial plexus presented to our clinic, 58 underwent primary brachial plexus reconstructive surgery. On exploration, all patients showed stretching and scarring of plexus elements; root avulsions were found in 28 patients (48%). Outcome evaluation was carried out in 49 of these patients with a follow-up period of 1 year or longer (mean follow-up, 27.9 mo; range, 12–72 mo). A total of 43 secondary reconstructive procedures to improve functionality of the involved arm were performed at a later stage in 25 of 58 patients. Outcomes of the secondary functional restorative procedures were evaluated (mean follow-up, 11.5 mo; range, 3–60 mo in 43 procedures). RESULTS Patients with neurolysis as a stand-alone procedure (11 patients) showed an outcome grade of 4 or 5. The average outcome of the 19 patients with C5, C6, and C7 grafting was Grade 3, the same as in patients with nerve transfers to the upper plexus elements (C5–C6 root avulsions, 13 patients). Patients with multiple root avulsions (five cases) showed an overall poor outcome (Grades 0–2). Secondary functional restorative surgery was performed in 43% of the patients and helped improve individual outcomes, providing a favorable effect on the general functionality of the arm. Among the restorative operations performed, the Steindler procedure, wrist extension restoration, claw hand correction, and free functional muscle flap transfer to the arm and forearm were the most rewarding. CONCLUSION A combination of primary brachial plexus reconstruction and carefully evaluated, selected, and planned function-restorative secondary procedures might offer favorable outcomes in patients with partial or total brachial plexus lesions.

Cutaneous flaps are used for closing wounds caused by traumatic accidents, oncological surgery (tumor removal), and burns (thermal, chemical, radiations). Skin grafting has the advantages of requiring just only one surgery for closing the defects once the wound bed is adequately prepared.The objective of the study was  to describe and asses the eficiency  of local cutaneous flaps for closing  skin defects in dogs.Seven dogs  underwent reconstruction of soft tissue wounds resulted from traumatic lesion or  after large tumors removal. Skin defects were located on the trunk and limbs. Cutaneous local flaps(advancement and rotational) were created by surgical preparations and mobilization the full tickness skin fold to enabling closure of adjacent defects. After wound debridment or tumoral removal a very carefull atraumatic and aseptical preparation of the flaps  were performed to preserve vascularization for adecquate blood supply.Cutaneous local flaps  proved effective for closing large defects in all dogs. Partial marginal necrosis of a portion of the flap occurred in one dog because of  procedure and technique errors, but the concurent remanent defects were adequate  to primary closure.  The wounds ultimately healed , without major complications.The skin local flaps(advancement and rotational) are a versatile technique that could be  use in a variety of locations, depending on skin defects shape and localization. The clinical results are comparable with those reported for  advanced reconstructive procedure.
