Aim: To review the role of Health Related Quality of Life (HRQL) assessments in the management of patients with venous ulceration. Method: A search of electronic databases and reference lists delivered numerous articles containing a variety of HRQL assessments for venous ulcer treatment. Synthesis: The salient points from the literature were collaborated in order to establish which outcome measures best reflect the quality of care delivered. The treatment of chronic venous leg ulceration is often prolonged and a permanent cure is frequently unattainable. An improvement in a patient's condition needs to be demonstrated in order to determine the benefit of a surgical treatment. The assessment of HRQL is increasingly recognised as a valuable surgical outcome measurement. Conclusion: The combination of ‘standard’ clinical outcome measures, the generic SF-36 and a specific ‘venous ulcer’ HRQL questionnaire can produce a more complete treatment outcome assessment in patients with venous ulceration.

Skin ulcers are a common complication of systemic sclerosis (SSc) that can significantly impact the quality of life. There have been recent reports on the use of hyperbaric oxygen therapy (HBO2T) in the management of nonhealing systemic sclerosis skin ulcers. The effect of HBO2T on pulmonary arterial hypertension (PAH), another common and potentially life-threatening complication of SSc, is unclear with literature on the subject lacking. We present the case of a 65-year-old female with limited SSc complicated by severe PAH and a nonhealing left lower extremity venous ulcer. HBO2T was successfully used as an adjunct in her management resulting in complete resolution of the venous ulcer and improved quality of life without any adverse effects on her pulmonary arterial hypertension.

A prospective nonrandomized cohort study on consecutive diabetic patients with foot ulcer was undertaken to assess the factors associated with the healing process or limb salvage and evaluate the impact of their treatment on their quality of life. Quality of life was evaluated using Diabetic Foot Ulcer Scale–Short Form (DFS-SF) questionnaire before and after treatment. A total of 103 diabetic patients with ulcer (mean age 69.7 ± 9.6 years, 77% male) were treated and followed up for 12 months. Ulcer healing, minor amputation, and major amputation rates were 41%, 41%, and 18%, respectively, while the mortality rate was 18%. Ulcer healing was associated with University of Texas wound grade 1 and the Study of Infections in Diabetic feet comparing Efficacy, Safety and Tolerability of Ertapenem versus Piperacillin/Tazobactam trial’s diabetic foot infection wound score. Limb loss was associated with nonpalpable popliteal artery, longer in-hospital stay, and delay until referral. Quality of life was improved in all domains of DFS-SF ( P < .0001) throughout the cohort of our patients regardless of their outcome, and no outcome (healing, minor amputation, or major amputation) was superior to other. Significant improvement was observed in all domains of hygiene self-management after consultation during the follow-up period.

Varicose ulcer are the wound caused due to improper functioning of venous valve. Venous ulcer develop mostly along the medial distal leg can be painful, can bleed & get infected with negative effect on quality of life .When valve gets damaged,it prevents the backflow of the blood, increases local venous pressure &which turns into venous ulcer. After conservative & surgical treatment ,recurrence of venous ulcer is common ranging about 54-78% by the 5th year after wound healing. In Ayurveda varicose vein correlated with Sirajanyadushtyavrana.Acharya Sushruta more scientifically described wound & its management with help of these specific Ayurvedic adjuvant therapies this kind of ulcer may be treated. A 45 yrs male patient diagnosed with varicose ulcer daily dressing done with Unprocessed Haridra,Ghruta& Madhu for 5weeks.The symptom like Pain, Ankle flare, Discharge, Hyperpigmentation,Size of ulcer & Granulation tissue were assessed. Trial showed that unprocessed Haridra,Ghruta,Madhu is effective in the management varicose ulcer. Hence this study conclude Ancient Management is safe, easily available , devoid Complication & Hence better Acceptability.
