.................... The access to palliative care in ends of life is one of the patients’ rights. Therefore it should be delivered into every level of health care for patients and family members. In some countries, palliative care is more frequent delivered in hospitals compare to primary health care, even though primary health care has a significant role in providing palliative care. Most families prefer to care for patients at home rather than in the hospital................................... The implementation of palliative care in Indonesia is still very limited to certain hospitals, even though doctors in primary care have great potential to offer such care to people in the community. Some of the factors contributing to the implementation are cultural and socioeconomic factors, patient and family perceptions, attitudes of service providers, lack of trained personnel, distribution of palliative care units, lack of consolidation and limited funds. As a result, patients with end-stage disease die in hospitals without receiving palliative care or dying at home with inadequate support................

ABSTRACT Objective: Evaluate the effects of clown activities on quality of life, depression, stress, anxiety, aid, and social support in patients eligible for palliative care (PC) attended in Primary Health Care (PHC). Method: A quasi-experimental study, with pre-intervention and post-intervention evaluation, carried out with 16 patients eligible for early PC in PHC in a Midwestern city. Patients received 24 visits with home-based clown activities. Results: The interventions evidenced improved quality of life and social support, with significant results for the Social Activities dimension (p = 0.023). Increased scores for Anxiety (p = 0.007) and Depression (p = 0.023) were also observed. Conclusion: Clown activities can bring positive results for the quality of life and social support of patients eligible for PC at home. They should be encouraged to interact with family knowledge and enhance humanized care, integral and centered on human relationships in PHC.

Objetivo: Analisar a produção científica de Programas de Pós-graduação stricto sensu brasileiros sobre Cuidados Paliativos na Atenção Primária à Saúde. Método: Revisão bibliométrica, realizada por meio da busca de Dissertações e Teses, defendidas entre 2006 e 2017, disponíveis nas bases de dados Portal Domínio Público, BDTD, Banco de Teses e Dissertações da USP, UFRJ, UFSC e Portal de Teses e Dissertações da CAPES. Resultados: A amostra foi constituída por 33 trabalhos, sendo 23 Dissertações e 10 Teses. Os estudos encontrados são, em sua maioria, dissertações defendidas em 2015, provenientes da USP, realizadas em Estratégias de Saúde da Família (ESF), tendo como sujeitos de pesquisa os profissionais de saúde. Conclusão: Ainda que incipiente, a produção científica de dissertações e teses produzidas pelos Programas de Pós-graduação stricto sensu pode colaborar para a discussão, entre gestores e profissionais de saúde, sobre o desenvolvimento dos Cuidados Paliativos na Atenção Primária de Saúde brasileira.

Introdução: A atuação da enfermagem ao indivíduo em cuidados paliativos (CP) na Atenção Primária à Saúde (APS) visa a promover a qualidade de vida dos indivíduos e da sua família como garantia da assistência integral, para um cuidado humanizado e digno, melhorando a maneira de enfrentar a doença e minimizando o sofrimento. Objetivo: Analisar e sintetizar a produção científica relacionada à assistência do enfermeiro ao indivíduo em CP nas APS. Método: Revisão integrativa da literatura realizada nas bases de dados Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Medical Literature Analysis and Retrieval System Online (MEDLINE) via PubMed, Base de Dados de Enfermagem (BDENF) e Scientific Electronic Library Online (SciELO), utilizando os descritores: Palliative Care, Nursing e Primary Health Care. Resultados: Foram analisados 17 artigos após seleção sistemática, sintetizados em um quadro com seus principais resultados e agrupados em três categorias: capacitação em CP: uma barreira para atuação do enfermeiro na APS; percepções, experiências e práticas dos enfermeiros nos CP; o papel do enfermeiro na equipe multiprofissional de CP. Conclusão: Notou-se que os enfermeiros possuíam conhecimento superficial acerca dos CP na APS, evidenciando a necessidade de educação continuada para promover a sua atuação em CP. Ademais, estudos com maior rigor metodológico com o foco no enfermeiro como agente disseminador da prática são necessários.

Objective: to map the available evidence on the main topics investigated in palliative care in primary health care. Method: scoping review type study carried out in five databases, including original articles, based on the descriptors palliative care, palliative care at the end of life, terminal care, terminal state, primary health care and their respective acronyms and synonyms, totaling 18 publications. The extraction of data from primary studies was performed using an instrument produced by the authors and which allowed the construction of the categories presented. Results: 18 publications were included in this review. Among the most studied themes are the difficulties of the teams regarding the continuity of care in the health network; the importance of in-service education by the multidisciplinary team; professional unpreparedness; bioethics; the validation and application of scales for prognosis and care for some pathologies such as cancer and diabetes; among others. Conclusion: it became evident that palliative care in primary health care has been gradually developed, but it is necessary to consider the organization of primary health care and the social policies that support or weaken it, being considered a complex challenge.

This study investigated Indonesian primary health care providers’ knowledge and comfort towards palliative care. A descriptive cross-sectional design was used. From August 2017 to February 2018, the research team approached 70 primary care centres in the Yogyakarta province of Indonesia and invited health care providers to complete the Palliative Care Quiz for Nursing – Indonesia and describe their comfort in caring for terminally ill patients. Data were obtained from 516 health care providers. The mean (±s.d.) score of palliative care knowledge was low (7.8±3.3 of a possible score of 20). Current comfort level in providing palliative care was also low (1.6±2.7 of a possible score of 10). Only 11.3% of palliative care knowledge was explained by respondents understanding of palliative care definition, their education levels and experience in providing palliative care in hospital. However, 82.9% of provider comfort was explained by their experiences for caring for terminally ill patients in primary healthcare centres, palliative care training and years of work experience in primary healthcare centres. Indonesian evidence-based palliative care standards and guidelines must be established with education offered to all providers.
