The aims of this research was to know how far program of football athlete development was effectively successful, how was situation of sport school, whether input is complete supporting this program, whether outputs of this program produced properly without challenge and whether outcomes of this program regenerated correctly without matter, To evaluate this program properly and intensively, so it was used logic model program which consists of situation of sport school, input of this program, outputs of this program and outcomes of this program. Methodology of this research used technique of collecting data by observations, interviews, questionnaires, and documentations study. Analysis of collecting data use triangulation technique which gotten by data resources, namely; organizer/coach and staff of sport school, the athlete, athlete’s parents, and society. The results of this research was found and can be concluded that situation of sport school, input and outputs of this program have been being done fairly well, whereas outcomes of this program have been extended impact very well. Adult day care (ADC) provides various services for meeting clients’ needs. Based on the mini-Delphi method with 46 ADC staff and the discussions with eight ADC administrators, this study developed and finalized a comprehensive logic model to represent the elements of ADC. For the three basic structures of a logic model—inputs/activities, outputs, and outcomes—the model contained seven core categories and 23 sub-categories. The ADC inputs/activities consisted of two core categories: “Place to stay” and “Intervention from staff”. These inputs/activities caused two kinds of outputs: “Clients’ experiences” and “Families’ experiences”. “Accumulating experiences” with repeated ADC visits was established as the link between the ADC outputs and outcomes, which were “Clients’ change” and “Families’ change”. ADC centers provide various experiences for their clients and their caregivers, ranging from the fulfillment of needs for fundamental care to psychiatric care and self-actualization. Improving various model-related inputs/interventions can produce better experiences and outcomes. The model can guide ADC administrators, policymakers, and researchers in the evaluation of a heterogeneous ADC service program that is community-based, thereby ensuring optimal care for clients with an efficient use of resources.

Background: Best practices comprise recent, relevant, and helpful nursing practices, methods, interventions, procedures or techniques based on high-quality evidence. Best practices should be implemented to improve individual patients’ health outcomes the overall quality of health care, and to strengthen the health system at large. Four facilitators (benefit levers) to effectively roll-out best practices in healthcare organizations were identified: alignment, permeation plans, leadership for change, as well as supporting and reinforcing structures. Purpose of the research: To develop and review a guide for an operational plan to facilitate the roll-out of best practices in healthcare organizations. Methodology: The content of the guide was based on findings of an integrative literature review and interviews. This article discusses the development of the guide using the logic model as format (phase One) as well as its review including five key informants using the Delphi method (phase Two). Results: The Logic Model provided a feasible format for the guide. Two Delphi rounds were required to review the guide’s design and content. This guide addresses pre-roll-out resources; pre-roll-out activities, best practices (outcomes) during and after roll-outs; and impact. Conclusion: The guide should be tested in different healthcare organizations and adapted accordingly to have an impact on improved patient and health outcomes. The guide can be used by managers wishing to roll-out best practices in their healthcare organizations to achieve improved health outcomes for individual patients.
