Venous ulcers are common in drug addicts and, although the management of these wounds is the same as in other patients, there are differences in the approach and the outcome of treatment. Those injecting drugs are at risk of serious infections, such as necrotizing fasciitis, wound botulism, and cutaneous anthrax due to the nature of the substances being injected. Herein, we present two cases of venous ulcers in drug addicts in their thirties. Both patients had been suffering from a venous ulcer for several years and the final result differed in the two patients. We concluded that it is necessary to raise awareness of the importance of treatment and lifestyle changes. The multidisciplinary approach in these patients may contribute to the improvement of wound healing.

Sushruta has explained various methods for Vrana Chikitsas. Virechana is vital to remove the vitiated Dosha from the body in turn normalize them and prevent recurrence of the wound. The local application of Vranabasti is one of the best substitutes for of chronic wound healing and Jathyadi Taila has better Shodhana & Ropana effect. Case: A case of 45 year old man, who presented with complaints of two venous ulcers in medial and lateral aspect of right ankle associated with pain, discharge, slough, oedema and discolouration of the skin since 2 years has been taken for the current study. Method: After initial Deepana and Pachana, Snehapana was administered with Mahatiktaka Ghrita. After Abhyantara and Bahya Snehana and Svedana, Virechana was administered with Avipattikara Churna. Vranabasti, was done every day for 30 minutes after the Samsarjana Karma Results: There was significant improvement in the size of the wound and nature of the surrounding tissues. Conclusion: The combination of Virechana and Vranabasti can be used as a line of treatment for the management of Chronic Venous Ulcer.

Venous ulcers are a more severe complication of chronic venous insufficiency, significantly compromising patient quality of life (QoL). Compressive stockings are still the gold standard treatment method with alternative therapies currently being evaluated. In this perspective, we investigate the influence of compressive stockings impregnated with hesperetin-based nanocapsules in the healing process of venous ulcers. Compressive stockings impregnated with hesperetin-based nanocapsules were applied to a consenting patient for 6 months following all relevant ethical principles for patient studies. The patient was evaluated at baseline (T0), 3 months (T3), and 6 months (T6), using photographic register (healing) probes to measure skin melanin, erythema and hydration parameters, and venous diameters, followed by questionnaires regarding QoL and pain perception. Healing was observed at the 3-month time point and with 91.6% and 93.1% of retraction area in larger ulcers of the right leg and lateral portion of the left leg, respectively. The deepest ulcer in a medial portion of the left leg healed 47.3%. A reduction of all measured skin parameters was observed, indicating a possible hesperetin effect. The scores of QoL and pain were, respectively, in the ranges of 91.6 to 31.2 and 7 to 0. Reduction in venous diameters also indicates healing function. These preliminary findings suggest that compressive stockings impregnated with hesperetin nanocapsules enhance venous ulcer healing. Further clinical trial controlled by placebo, involving a greater number of patients, is required to confirm the findings of this case report.

<p class="abstract"><strong>Background:</strong> Venous leg ulcers are a common cause of significant morbidity leading to compromised quality of life and a huge financial burden on patients. Apart from the systemic therapy, many topical agents are available to accelerate healing in these ulcers. Topical phenytoin is one such agent which has been studied for promotion of wound healing. The aim of the study was to study the efficacy of topical phenytoin in healing of chronic venous ulcers.</p><p class="abstract"><strong>Methods:</strong> 56 patients of chronic venous ulcer were randomly divided in two groups of 28 each. First group was given daily dressing of phenytoin in normal saline (group A) and second group was given only normal saline dressing (group B) for a period of 8 weeks. After 8 weeks the reduction in size of ulcer was measured, on the basis of which results were calculated.<strong></strong></p><p class="abstract"><strong>Results:</strong> The mean age of the patients was 50.29±13.37 years. The mean duration of ulcer was 10.13±4.66 weeks. Compared to the conventional normal saline dressing, patients given phenytoin and normal saline dressing showed far greater tendency to heal (p=0.02). 35.7% patients in phenytoin group showed excellent response out of which 21.42% achieved complete healing, while in normal saline group only 14.28% patients showed excellent response and none of patient showed complete healing. No significant adverse effects were observed in either of the group.</p><p class="abstract"><strong>Conclusions:</strong> Topical phenytoin appears to be a safe, easy, effective and inexpensive option to promote wound healing and can be used as an adjuvant for treatment of chronic venous ulcers. Further studies with larger population are merited.</p>

Identificar e analisar evidências científicas adotadas na prevenção de recidivas de úlceras venosas. Trata-se de um estudo de revisão integrativa da literatura, descritivo, de abordagem qualitativa, realizado entre outubro e novembro de 2017, a partir de consulta de bases de dados nacionais e internacionais. Os descritores utilizados foram “úlcera venosa”, “úlcera varicosa”, “recidiva” associados à palavra chave “prevenção”. Recorreu análise temática de conteúdo, emergindo duas categorias. Dos 305 artigos encontrados, 10 foram selecionados atendendo aos critérios de seleção. As principais evidências observadas foram “medidas para a prevenção de recidivas de úlceras venosas” e “facilidades e dificuldades da implementação de medidas preventivas”. Evidenciou-se que as medidas são diversas para prevenir as recidivas de úlcera venosa, no entanto, a adesão ainda é fator limitador as práticas devido à complexidade que a envolve.Descritores: Úlcera Venosa, Recidiva, Prevenção. Measures to prevent recurrence of venous ulcersAbstract: To identify and analyze scientific evidences adopted in the prevention of venous ulcers recurrences. Orofacial clefts (OFC) are one of the most common congenital problems seen with a very high incidence. It imparts a negative effect on the overall health of the child by hindering in his feeding practices, normal facial growth, development of dentition and hence speech. Infants born with orofacial clefts have oronasal communication which creates a problem with the creation of negative pressure inside the oral cavity required for suckling.The treatment for such patients is with the multidisciplinary approach but the preliminary  concern for the neonate is to help with the feeding for which a feeding appliance is given. This case report presents a case of a 3-day old infant to whom a feeding appliance was given to aid in suckling.

Abstract Traditional medicine credits yarrow (Achillea millefolium) with the ability to accelerate wound healing. The purpose of this research was to determine the effects of yarrow on the epithelization of the lower leg venous ulcers. The study included 39 patients with venous leg ulcers. They were divided into two groups: the first (experimental) group of patients were treated with an ointment containing 7.5% of yarrow extract. In the second (control) group, saline solution dressings were applied to ulcers, within the period of three weeks. In the experimental group, at the beginning of the therapy, the total surface of all the ulcers was 44736 mm2. After three weeks, the total surface of all the ulcers was 27000 mm2 (a decrease of 39.64%). In the control group, at the beginning of the therapy, the total surface of all the ulcers was 46116 mm2. At the end of the study (21 days) the total surface of all the ulcers was 39153 mm2 (a decrease of 15.1%). Herbal preparations are suitable for application in the therapy of venous ulcers, but their efficiency in wound healing is still to be investigated.
