COVID-19 has taken up the world like a storm. It has placed unusual demands on healthcare delivery systems, including physiotherapy services worldwide. This challenge is even larger than the one placed by Severe Acute Respiratory Syndrome (SARS) or Ebola because of its sheer virulence. Times are changing; we need to do the same and accept the challenge head on. The role of a physiotherapist in the management of the COVID-19 patients is well established but an important issue is the management of non-COVID-19 as well as COVID-19 patients judiciously during these times. The various barriers to practice have increased manifold due to the condition as well as the associated problems which arose due to it. This commentary will comment on the likely impact of this pandemic over the existing physiotherapy delivery structure, challenges arising out of it and their probable solutions, ending with a brief note on India.

The world is currently going through a disastrous event and a catastrophic upheaval caused by the coronavirus disease 2019 (COVID-19). The pandemic has resulted in loss of more than 150000 deaths across the globe. Originating from China and spreading across all continents within a short span of time, it has become a matter of international emergency. Different agencies are adopting diverse approaches to stop and spread of this viral disease but still now nothing confirmatory has come up. Due to lack of vaccines and proper therapeutic drugs, the disease is still spreading like wild fire without control. An Old but very promising method- the convalescent plasma therapy could be the key therapy to stop this pandemic. This method has already proven its mettle on several occasions previously and has been found to be effective in curing the pandemics induced by Ebola, severe acute respiratory syndrome coronavirus (SARS-CoV) and Middle East respiratory syndrome coronavirus (MERS-CoV). The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) belongs to the same group of β-Coronavirus that has resulted in the above diseases. Therefore, the role of plasma therapy is being explored for treatment of this disease. In this review, we have mainly focused on the role of convalescent plasma therapy and why its use should be promoted in fight against COVID-19, as it could turn out to be a game changer.

Russian political emigration from the beginning of its birth in the first half of the nineteenth century was constantly in search of forms and methods of struggle with royal power in the homeland. Detachment from Russia, the feeling of isolation that was inherent in emigration to the early twentieth century, were an important factor in the ongoing conflicts that took place in its environment. We note the conflicts between the «old» and the «young» emigration in the late 1860’s, between the Marxists and the populists of the 1880’s, between the revolutionary Marxists and the «economists» at the end of the 1890’s. All of these, as a rule, were due to excessive the ambitions of some leaders, the attempt to become the «rulers of ideas» for revolutionary youth, due to significant financial problems. In the list of these and similar conflicts there are events of 1870, when in the environment of political emigration there are two serious confrontations between the leader of anarchists M. Bakunin on the one hand and S. Nechaev or «Russian section of the First International» - on the other. These conflicts significantly influenced the situation in emigration, disorganized it, weakened the ability to fight the tsarist regime. They were accompanied by sharp accusations, searches for compromising materials, attempts to get support from leaders of the world revolutionary movement. The ambitions of young revolutionaries such as S. Nechaev or M. Utin were also connected with the attempt to take the main place among the emigrants, moving to the background of former leaders M. Bakunin, M. Ogarev, P. Lavrov. All this led to split in emigrant colonies, which consisted mainly of student youth. Violent discussions, accusations, boycotts became a hallmark of emigrant life. Basically, all these events took place in Switzerland, which at that time already became the center of not only Russian, but also international political emigration. Conflicts were directed at the political annihilation of the opponents, which subsequently resulted in the arrest and extradition to the Russian government of S. Nechaev in 1872, the cessation of the activities of the Russian Section of the First International and the return of M. Utin to Russia and the cessation of revolutionary activity in general. The positive side of these conflicts was the rallying of emigrants around their leaders, better information on the state of affairs in their environment, the development of new forms and methods of interaction and the strengthening of the role of revolutionaries from Russia itself.

Since December 2019, a novel beta coronavirus has spread around the world. This virus can cause severe acute respiratory syndrome (SARS). In this study, we reviewed proteases of SARS-CoV-2 based on related articles published in journals indexed by Scopus, PubMed, and Google Scholar from December 2019 to April 2020. Based on this study, we can claim that this coronavirus has about 76% genotype similarity to SARS coronavirus (SARS-CoV). Also, similarities between these two viruses have been found in the mechanism of entry into host cells and pathogenicity. ACE 2, the angiotensin convertase enzyme 2, plays a role in the Renin-Angiotensin-Aldosterone system (RAAS) and blood pressure regulation. Some mechanisms have been reported for the role of ACE 2 in the pathogenicity of SARS-CoV-2. For example, the interaction between the ACE 2 receptor and spike protein mediated by TMPRSS2, Cathepsin B/L, and other enzymes is responsible for the entry of the virus into human cells and pathogenicity. Some host cell endosomal enzymes are necessary to cleavage coronavirus spike protein and cause binding to their common receptor. So, we conclude that molecules like antibodies or small molecules like ACE 2 antagonists and soluble ACE 2 can be used as a good therapeutic candidate to prevent SARS-CoV-2.
