This study aimed to investigate the application of positron emission tomography- (PET-) computed tomography (CT) image information data combined with serous cavity effusion based on clone selection artificial intelligence algorithm in the diagnosis of patients with malignant tumors. A total of 97 patients with PET-CT scanning and empirically confirmed as serous cavity effusion were retrospectively analyzed in this study. The clone selection artificial intelligence algorithm was applied to register the PET-CT images, and the patients were rolled into a benign effusion group and a malignant effusion group according to the benign and malignant conditions of the serous cavity effusion. Besides, the causes of patients from the two groups were analyzed, and there was a comparison of their physiological conditions. Subsequently, CT values of different KeV, lipid/water, water/iodine, and water/calcium concentrations were measured, and the differences of the above quantitative parameters between benign and malignant serous cavity effusion were compared, as well as the registration results of the clone algorithm. The results showed that the registration time and misalignment times of clonal selection algorithm (13.88, 0) were lower than those of genetic algorithm (18.72, 8). There were marked differences in CT values of 40–60 keV and 130–140 keV between the two groups. The concentrations of lipid/water, water/iodine, and water/calcium in basal substances of the malignant effusion group were obviously higher than the concentrations of the benign effusion group ( P < 0.05 ). Benign and malignant effusions presented different manifestations in PET-CT, which was conducive to the further diagnosis of malignant tumors. Based on clone selection artificial intelligence algorithm, PET-CT could provide a new multiparameter method for the identification of benign and malignant serous cavity effusions and benign and malignant tumors.

Abstract Lactate dehydrogenase (LDH) isoenzymes may be useful in the differential diagnosis of pleural effusion (PE) and ascitic fluid (AF) etiologies in cats since tissue damage induces their release, thereby changing the pattern of their activity. The present study aimed to determine the diagnostic utility of measuring LDH levels and its isoenzyme activities in PE or AF in cats with malignancy. We measured LDH levels and its isoenzyme activities in serum, PE, and AF in 29 cats and compared the results between malignant, infectious diseases, and non-malignant, non-infectious diseases. Receiver operating characteristic (ROC) analysis was performed to assess the accuracy of the tests in diagnosing feline malignancy. In PE or AF, significant differences were found in LDH levels and LDH isoenzymes activities among the three groups. The combination of LDH level and LDH-1 activity in PE or AF had the highest area under the ROC (AUC) value of 0.874 for discriminating malignant effusion from non-malignant effusion. The sensitivity and specificity of using the combination of LDH level (cutoff: <2,269 U/L) and LDH-1 activity (cutoff: <4.8%) in PE or AF for predicting malignancy with the highest AUC value were 94.4% and 72.7%, respectively. Our results suggest that the combination of LDH level and LDH-1 activity in PE or AF can potentially diagnose for malignancy. Considering that LDH isoenzymes can be measured inexpensively and easily, LDH tests can be readily accommodated in veterinary clinical practice.

ABSTRACT Surgery and platinum-based chemotherapy are highly efficacious for treating advanced ovarian cancers in humans, but their efficacy is less known in dogs. We evaluated the long-term treatment outcomes of seven dogs with malignant ovarian tumors with malignant abdominal effusion. Ovariohysterectomies (OVHs) were performed on all dogs; four had ovarian adenocarcinoma (AC) with gross dissemination in the peritoneum (two with pleural effusion), and three had a granulosa cell tumor (GCT) with no gross dissemination in the peritoneal cavity, although one showed pleural effusion. Effusion resolved after the OVH in all dogs. Six dogs (three ACs, three GCTs) received postoperative IV carboplatin therapy. Two dogs with GCT had no postoperative recurrence or metastasis, and one dog with GCT had recurrence 1811 days postoperatively. All dogs with AC developed recurrent effusion 171–584 days postoperatively, which resolved after intracavitary administration of cisplatin or carboplatin, with a subsequent disease-free interval of 155–368 days. Overall survival was longer for dogs with GCTs (822–1840 days) than for those with ACs (617–841 days). These results suggest that dogs with ovarian tumors with malignant effusion can survive relatively long after platinum-based chemotherapy in addition to OVH, with a more favorable prognosis for GCT than AC.

BackgroundThe differential diagnosis of malignant effusion remains a clinical challenge. We aim to summarise all relevant literature studies in order to determine the overall clinical value of E-cadherin in the diagnosis of malignant effusion by meta-analysis.MethodsPubMed, the Cochrane Library Database, Medline (Ovid), Web of Science, CNKI, WANFANG and WEIPU databases are thoroughly searched up to 15 March2018. The calculated pooled sensitivity, specificity, likelihood ratio (LR), diagnostic OR(DOR) and the summary receiver operating characteristic (SROC) curve were plotted.ResultsA total of 15 studies were included in the analysis. The sensitivity and specificity of E-cadherin in the diagnosis of malignant effusion were determined to be high, with a sensitivity of 0.83(95%CI0.79 to 0.87) and a specificity of 0.96(95%CI0.90 to 0.98). The positive LR was determined to be 21.10(95%CI 8.54 to 52.11), the negative LR was determined to be 0.17(95% CI 0.14 to 0.22) and the DOR was determined to be 121.34(95%CI 49.11 to 299.80). The SROC curve exhibited a high overall diagnostic, with the area under the curve measured to be 0.91(95% CI 0.89 to 0.93). Subgroup analysis showed the method (cell blocks or smears), sample size (≥100 or<100), geographical location (Asia, Europe or USA) and impact factor of each article (≥3 or<3) were not the sources of overall heterogeneity.ConclusionE-cadherin exhibits very good diagnostic accuracy for the diagnosis for malignant effusion; thus, it can be helpful in the process of clinical decisions.
