The present article correlates with a fuzzy hybrid technique combined with an iterative transformation technique identified as the fuzzy new iterative transform method. With the help of Atangana-Baleanu under generalized Hukuhara differentiability, we demonstrate the consistency of this method by achieving fuzzy fractional gas dynamics equations with fuzzy initial conditions. The achieved series solution was determined and contacted the estimated value of the suggested equation. To confirm our technique, three problems have been presented, and the results were estimated in fuzzy type. The lower and upper portions of the fuzzy solution in all three examples were simulated using two distinct fractional orders between 0 and 1. Because the exponential function is present, the fractional operator is nonsingular and global. It provides all forms of fuzzy solutions occurring between 0 and 1 at any fractional-order because it globalizes the dynamical behavior of the given equation. Because the fuzzy number provides the solution in fuzzy form, with upper and lower branches, fuzziness is also incorporated in the unknown quantity. It is essential to mention that the projected methodology to fuzziness is to confirm the superiority and efficiency of constructing numerical results to nonlinear fuzzy fractional partial differential equations arising in physical and complex structures.

In the analysis in this article, we developed a scheme for the computation of a semi-analytical solution to a fuzzy fractional-order heat equation of two dimensions having some external diffusion source term. For this, we applied the Laplace transform along with decomposition techniques and the Adomian polynomial under the Caputo–Fabrizio fractional differential operator. Furthermore, for obtaining a semi-analytical series-type solution, the decomposition of the unknown quantity and its addition established the said solution. The obtained series solution was calculated and approached the approximate solution of the proposed equation. For the validation of our scheme, three different examples have been provided, and the solutions were calculated in fuzzy form. All the three illustrations simulated two different fractional orders between 0 and 1 for the upper and lower portions of the fuzzy solution. The said fractional operator is nonsingular and global due to the presence of the exponential function. It globalizes the dynamical behavior of the said equation, which is guaranteed for all types of fuzzy solution lying between 0 and 1 at any fractional order. The fuzziness is also included in the unknown quantity due to the fuzzy number providing the solution in fuzzy form, having upper and lower branches.

A case of successful heart transplantation from a donor that suffered an out of hospital cardiac arrest lasting approximately one hour is presented. The recipient, a 27-year-old female nurse, was diagnosed with arrhythmogenic right ventricular cardiomyopathy in 2012. The donor was a 24-year-old male who after consuming an unknown quantity of alcohol and cocaine aboard a yacht was found floating face-down in the sea. The patient to be pulseless and CPR commenced after 7 minutes. The length of time that he spent in the sea was unknown. Return of spontaneous circulation (ROSC) occurred after 20 minutes of CPR on site. However, the patient arrested again in the ambulance and CPR was performed until he arrived Hospital, still in cardiac arrest. The patient arrested three more times before achieving a stable circulation. He was transferred to the intensive care unit on high doses of Noradrenaline and Adrenaline to maintain an adequate mean arterial blood pressure. An initial echocardiogram (ECHO) revealed a hypocontractile left ventricle (LV) with an estimated ejection fraction (EF) of 30%.Over the following 3 days the patient’s cardiac function improved. He was  weaned off inotropic support and a repeat ECHO showed a normal LV with an EF of 70%. A brain MRI showed diffuse swelling consistent with global hypoxic injury with wide areas of cortical and basal ganglia infarction. The patient’s parents gave their consent and he was offered for organ transplantation on day 6. The operation was successful, with the recipient making an uneventful recovery. She received immunosuppressive treatment with cyclosporine, prednisolone and azathioprine and experienced one episode of early mild rejection with full resolution. She remains well 8 months later.

[English]: Generational, transformational, and global meta-level are three typical activities of algebraic thinking students engage in school. Several studies show that students involve more in the generational activity than the other activities. However, some students still have difficulties solving problems in the generational activity. Therefore, this study focused on the generational activity, aiming to analyze student's algebraic thinking and difficulties in the generational activity. It involved ninety-five 7th-grade students given an initial test to measure their abilities in the generational activity. The analysis of students’ answers and interviews follow three steps; data condensation, data display, and drawing and verifying conclusions. This study indicates that, in the generational activity, the students can generalize statements from patterns better than forming statements and equations containing an unknown quantity. Students’ difficulties in the generational activities, including (1) understand the problems and turn them into mathematical forms and (2) generalize the patterns to the nth term. These are due to the students’ incomprehension of the relationships of some conditions in the given problems and understanding of the meaning of variables.    [Bahasa]: Kegiatan generasional, transformasional, dan meta-level global adalah tiga kegiatan berpikir aljabar yang dilakukan siswa di sekolah. Beberapa penelitian sebelumnya menunjukkan bahwa siswa lebih banyak terlibat dalam kegiatan generasional dibandingkan kegiatan berpikir aljabar lainnya. Namun, terdapat beberapa siswa yang mengalami kesulitan dalam menyelesaikan masalah pada kegiatan generasional. Oleh karena itu, penelitian ini akan difokuskan pada kegiatan generasional, dengan tujuan menganalisis pemikiran aljabar siswa dalam aktivitas generasional dan kesulitan yang dialami siswa. Penelitian ini melibatkan 95 siswa kelas 7 yang mengikuti tes awal untuk mengukur kemampuan dalam kegiatan generasional. Jawaban siswa dan hasil wawancara dianalisis melalui tiga tahap, yaitu kondensasi data, penyajian data, serta penarikan dan verifikasi kesimpulan. Hasil penelitian ini menunjukkan bahwa dalam kegiatan generasional, siswa dapat mengeneralisasi pernyataan dari pola lebih baik daripada membuat pernyataan dan persamaan yang mengandung variabel. Siswa menunjukkan berbagai kesulitan dalam kegiatan generasional, diantaranya (1) memahami masalah dan mengubahnya menjadi bentuk matematis dan (2) menggeneralisasikan pola suku ke-n. Hal ini disebabkan siswa tidak memahami hubungan beberapa kondisi dalam masalah yang diberikan dan arti variabel.

Abstract Organophosphate pesticide (OP) poisoning is quite common and can cause cardiovascular complications and even direct myocardial injury. However, no guideline has included an acute poisoning as a potential cause for a type 2 myocardial infarction (MI) so far. Here we present a case of a 61-year-old woman brought by ambulance to emergency department one hour after accidental ingestion of an unknown quantity of a solution she used against flea infestation. The patient presented with dizziness, myosis, excessive sweating, hypersalivation, sphincteric incontinence, muscle fasciculation, tremor of the extremities, pale skin, alcoholic and pesticide breath odour. Even though we had no guidelines to fall back on, we successfully treated the patient with low-molecular-weight heparin, antiplatelets, statin, diltiazem, antidote therapy, and supportive care. Physicians should be aware that OP poisoning can induce type 2 MI as a complication within a few hours since exposure, and emergency management should always include close cardiac monitoring.
