Childbearing culture: a prominent context in the process of maternal role attainment in Iranian mothers with preterm neonates

Transition to the role of mothering is one of the most important events in a woman’s life. While childbirth is a biological event, pregnancy and the experiences around it are more influenced by social structure, which is shaped by cultural perceptions and practices. The aim of this study was to explore cultural context during maternal role attainment in neonatal intensive care units (NICUs) in Iran. The study was part of a grounded theory study on how the mothers of preterm neonates go through maternal role attainment. Data collection was carried out by purposeful sampling from 20 participants (15 mothers of preterm neonates and 5 NICU nurses). Data were analysed according to Corbin and Strauss’s (2015) approach. Four categories of childbearing culture emerged: ‘The necessity of childbearing’, ‘Childbearing rituals’, ‘Maternal persistent presence’ and ‘Attitudes and religious beliefs’. The findings showed that the special beliefs and practices in Iranian culture affected all of the participants’ reactions to mothering process. Culture is one of the most important factors affecting the development of motherhood in Iran. In order to provide sensitive and culturally appropriate care, nurses should be aware of the general impact of cultural norms and values on the process of maternal role attainment and strive to meet the cultural needs of all mothers.

STUDI KASUS PADA PRIMIPARA DENGAN BAYI GEMELLI PREMATUR

Persalinan prematur atau preterm adalah kelahiran hidup dengan usia gestasi kurang dari 37 minggu. Penyebab terjadinya persalinan preterm diantaranya kehamilan gemelli. Asuhan keperawatan yang diberikan pada ibu primipara diharapkan perawat membantu proses pencapaian peran ibu serta mengajarkan kemandirian dalam merawat diri dan bayinya. Metode yang digunakan adalah studi kasus dengan fokus penerapan teori “self care” Orem dan “maternal role attainment” Mercer. Aplikasi teori Orem dan Mercer efektif diterapkan pada kelima kasus yang dikelola, perawat melakukan observasi dan wawancara pada perilaku pasien serta kebutuhan yang diharapkan dari perawat. Intervensi keperawatan yang sesuai dengan kebutuhan pasien membantu dalam pencapaian peran menjadi ibu lebih optimal.

Prenatal Attachment pada Kehamilan Remaja dengan Gangguan Penyesuaian dan Retardasi Mental Berat

Prenatal attachment is an abstract and unidirectional bond between parents and the fetus that develops during pregnancy. It is closely associated with how the mother utilizes her abilities and knowledge to organize and perform the duties related to her maternal role attainment. We report a case of 15 years old girl admitted to the emergency room with an official request of visum et repertum in January 2018 due to becoming suspected sexual violence victim. The history of present illness revealed a 15 y.o. girl G1P0A0, singleton, at 34-36W Estimated Gestational Age with condyloma acuminata. During her pregnancy, she described relatively negative feelings about her pregnancy including shame, anxiety, and anger. She felt lack of energy and motivation since she became aware of her pregnancy, therefore spent her most of her time on the bed. Her IQ-test showed a profound intelligence rate followed by a total score of Strength and Difficulty Questionnaire (SDQ) 16, and scored low in Prenatal Attachment Inventory (PAI) at 31. We concluded that PA scoring can be utilized as a predictor of mother-infant bonding quality after birth, thus the instrument will be beneficial as initial screening before choosing appropriate management for vulnerable population mother.

Life Experience of Pregnant Woman with Gestational Diabetes Mellitus in Maternal Role Attainment in Special Region of Yogyakarta

Introduction: Gestational Diabetes Mellitus (GDM) is any abnormal carbohydrate intolerance that begins or is first recognized during pregnancy. The presence of GDM has important implications for both the baby and the mother. Objective: This study explored life experience of pregnant woman with GDM in maternal role attainment. Methods: This was a phenomenological approach qualitative research. Participants were 12 mothers who gave birth to a maximum of 1 year with a history of GDM selected by purposive sampling technique with sampling criteria. Data analysis used the Colaizzi method (2011). Results: There were eight categories, e.g. the experience of the mother in GDM diagnosis; the influence GDM of the pregnancy; mother's experience in trying hard to keep her pregnancy; a variation of life experience of mothers during pregnancy, childbirth, and post-delivery; the perception of mother of her role being a mother, the factors that support the development and achievement of the maternal role attainment; the mother's experience in integrating the identity of her role as mother; gap and the desire of the mother with GDM in receiving health services. Life experience of GDM mothers in achieving maternal role attainment has a diversity. Discussion: Pregnant mothers with GDM experience various maternal role attainments. They cannot attain the role optimally.

GAMBARAN KARAKTERISTIK REMAJA PEREMPUAN YANG MELAKUKAN PERNIKAHAN DINI DI BANTUL, YOGYAKARTA, BERDASARKAN MODEL MATERNAL ROLE ATTAINMENT

Prosen pencapaian identitas peran ibu bagi remaja perempuan yang melakukan pernikahan dini tidaklah mudah. Data menunjukkan bahwa prevalensi pernikahan dini di Kabupaten Bantul adalah yang tertinggi kedua setelah Kabupatan Gunung Kidul di Yogyakarta. Penelitian ini dilaksanakan dengan tujuan untuk mengetahui gambaran karakteristik remaja perempuan di Kabupaten Bantul, Yogyakarta, yang telah menikah di bawah umur dengan menerapkan model Maternal Role Attainment. Penelitian dilakukan pada 60 remaja perempuan yang telah menikah di bawah usia 18 tahun, dan telah memiliki anak berusia 0-12 bulan. Hasil penelitian menunjukkan bahwa tingkat pencapaian identitas peran ibu berada pada kategori sedang. Direkomendasikan agar dilaksanakan promosi kesehatan periodik agar membantu ibu mencapai kepercayaan diri dan kepuasan menjalankan peran ibu yang optimal serta ikatan tali kasih yang adekuat di antara ibu dan bayi.
