Leiomyoma of the uterus is the most common benign tumor arising from uterine smooth muscle. This is a case of a woman with an exophytic uterine leiomyoma mimicking an ovarian neoplasm. The potential for uterine leiomyomas to grow to an extreme size before causing symptoms is quite remarkable. This is likely due to the relatively large volume of the abdominal cavity, the distensibility of the abdominal wall and the slow growth rate of these tumors. In our case report, despite a fibroid of size reaching up to 34 weeks of gestation, our patient had minimal symptoms except for abdominal discomfort. There were no symptoms related to abnormal uterine bleeding or abnormal bowel or bladder habit. Also, the history and clinical examination of the patient favored a diagnosis of ovarian malignancy.

INTRODUCTION: Giant uterine leimyofibromas delivered through the uterine cervix are rare. MATERIALS AND METHODS: We report 3 observations in order to describe our diagnostic and therapeutic approach. RESULTS: Three patients aged 40, 49 and 41 years were seen for intense pelvic pain associated with metrorrhagia and a feeling of pelvic heaviness. The average delay of consultation was three years. They were all multiparous. The gynecological examination revealed a firm tissue mass with a smooth and regular surface of eight cm on average, occupying the vaginal cavity while respecting the vaginal walls. Speculum examination was not possible because of the size of the mass which was flush with the lower third of the vagina. Magnetic resonance imaging concluded to a myoma delivered through the cervix in all patients. A total hysterectomy was performed. Histology of the total hysterectomy specimen concluded a uterine leiomyoma of the uterus. CONCLUSION: These giant uterine leiomyomas are rare. Magnetic resonance imaging describes them well. The treatment is most often a total hysterectomy in women who do not wish to have pregnant.

Benign metastasizing leiomyoma (BML) is a rare spindle cell neoplasm seen in middle-aged women who have a history of leiomyoma of the uterus. The most common sites of metastases are the lungs; however, other sites of spread have been documented. These tumors by definition have no malignant features on histology and tend to be estrogen and progesterone positive. We present a middle-aged woman who was incidentally found to have multiple pulmonary nodules and a mass on her sternum after she was involved in a motor vehicle accident. She had a history of uterine leiomyoma and had undergone a hysterectomy ten years prior to the accident. Biopsies were performed of the lung nodules and sternum mass and compared to her hysterectomy specimen, and they were identical, and hence, she was diagnosed with BML. Due to the growing tumor of her sternum, she was started on tamoxifen with stability of her tumors. These tumors, since they are benign, tend to have an indolent course. However, in the instances that treatment is warranted, options include surgery or antiestrogen therapy. We will be discussing the pathogenesis, histological findings, and treatment options of this rare condition. Our case is unique because BML in general do not tend to spread to multiple organs and tend to be limited to one site of disease.

Meigs’ syndrome is a rare condition characterized by the presence of a benign fibroma of the ovary, ascites and pleural effusion. Other benign cysts of the ovary (such as struma ovarii, mucinous cystadenoma, serous cystadenoma and teratomas), leiomyoma of the uterus, and secondary metastatic tumours to ovary if associated with hydro thorax and ascites are referred to as ‘Pseudo‐Meigs” syndrome. It very uncommon and diagnosis is made difficult by symptoms that usually mimic disseminated malignancy or tuberculosis. The gold standard treatment is laparotomy and, by definition of the syndrome, after tumor removal, the symptoms resolves and the patients become asymptomatic. We presented an 18 years old girl with giant ovarian serous cystadenoma with associated pseudo-meigs syndrome, successfully managed in a low resources setting.

The combined pathology of the uterus, leiomyoma and adenomyosis, is one of the pressing problems of modern medicine, and is accompanied by a deficiency of vitamins, and the main macro-, trace elements. The purpose — to study the content of folic acid, vitamins B1, D, E, ferritin in blood, zinc, copper, calcium in the blood and remote tissues of the uterus. 150 women were examined: 30 practically healthy women, and 120 patients with combined uterine pathology with leiomyoma and adenomyosis. Group I — practically healthy women; Group ІІА — patients with asymptomatic leiomyoma of the uterus and adenomyosis nodular, diffuse, diffuse-focal І, ІІ, ІІІ, ІV degrees of severity of active clinical course; group IB — patients with symptomatic leiomyoma of the uterus and adenomyosis diffuse, diffuse-focal І, II degrees of severity of inactive clinical course. The content of folic acid, vitamins B1, D, E, ferritin in blood, zinc, copper, calcium in blood and remote tissues of the uterus was studied, the hormonal status of women with combined uterus pathology. Determined that the level of folic acid was significantly lower in relation to the control group by 1.5 times, vitamin B1 — by 3.6 times, vitamin D — by 2,3 times, vitamin E — by 1.5 times. In groups ІІА, II B, the concentration of ferritin in comparison with group І was by 11.9 and by 13.4 times lower, respectively. In relation to the control group І, the content of Zn was lower in the group ІІА by 1.9; Cu — by 1.25 times more, in group IIB — by Zn 2.0 times less, Cu — by 1.2 times more, respectively. In group ІІА, the calcium content did not significantly differ from the indicators of the control group, in group IIB there was a significant increase in calcium levels by 1.2 times. The content of estradiol in patients of group IIB was by 1.5 times higher than in women of group ІІА, progesterone — by 1.15 times lower, luteinizing hormone — by 1.3 times less, follicle-stimulating hormone — by 1.2 times lower respectively. Thus, Determining the content of zinc, copper, calcium in the blood and remote tissues at case of a combined pathology of the uterus with leiomyoma and adenomyosis is of practical importance from the point of view of timely prevention of diseases.
