ZusammenfassungDerzeit existieren für Kopf- und Halskarzinome (HNSCC) keine evidenzbasierten Leitlinien für das Re-Staging oder die Tumornachsorge nach primärer Radio(chemo)therapie. In Deutschland ist die hochauflösende Sonografie mit Verwendung von Farbduplexverfahren neben der Computertomografie (CT) und der Magnetresonanztomografie (MRT) ein bewährtes Verfahren zur Therapiekontrolle (Re-Staging) im Rahmen der Tumornachsorge nach der Primärtherapie von Kopf-Hals-Karzinomen. Die PET/CT gilt seit Kurzem als Standard. Es fehlt an klaren Empfehlungen, wie sich die unterschiedlichen in der Routine angewendeten Verfahren ergänzen. Außerdem fehlt es an validen Selektionsalgorithmen für die Indikation zur PET/CT. Indikationen für und wider eine Salvage Neck Dissection werden häufig als Einzelfallentscheidung getroffen. Dieser Artikel soll die gängigen Re-Staging Modalitäten darstellen und ein aktuelles Studienkonzept kurz erläutern.

Mucosal malignant melanoma of the head and neck is a rare diagnosis. The safety and efficacy of salvage neck dissection following carbon–ion radiotherapy with concurrent chemotherapy are not well described, and carbon–ion radiation protocols have not been fully developed. A 77 year old woman with crT0N1M0 mucosal melanoma of the head and neck achieved a complete response following initial treatment with carbon–ion radiotherapy and concurrent chemotherapy. She was treated with salvage neck dissection for as a cervical lymph node metastasis 16 months after initial treatment. She experienced neither Clavien-Dindo Grade 3 or 4 postoperative complications nor subsequent recurrence of disease at 3 months following salvage neck dissection. Surgical specimens may be useful for future precision oncology based on the molecular biology of recurrence melanoma with poor prognosis.

Tracheostomy is a common procedure in otorhinolaryngology and it is not without its own complications. Tracheoinnominate artery fistula is one of the late complication of tracheostomy. It commonly results in fatality if not detected and treated early. Herein, we present a rare case of a patient with underlying nasopharyngeal carcinoma post radiotherapy, cryotherapy and salvage neck dissection with tracheoinnominate artery fistula as a consequence of tracheostomy who defied statistics and was successfully stented. Objective of this case report is to create awareness regarding the differential diagnosis of massive bleeding from tracheostomy and immediate life saving measures the can be undertakenin addition to theneed to be vigilant in a patient with multiple risk factors that predisposes to the occurrence of tracheoinnominate artery fistula.International Journal of Human and Health Sciences Vol. 03 No. 04 October’19 Page : 238-240
